is a medication used to manage angina
(chest pain) , high blood pressure

called Raynaud's phenomenon, and
premature labor.

➢It is recommended that Nicardia Retard
(Nifedipine Tablets) be administered orally
once daily on an empty stomach.
➢Nicardia Retard is a sustained release
dosage form and tablets must be swallowed
whole, not bitten or divided.
➢Normally, titration must take place over a 7
day to 14 days period starting with 30 mg
once daily. Upward titration must be based on
therapeutic efficacy and safety.

➢The normal maintenance dose is
30 mg to 60 mg once daily. Titration
to doses above 90 mg daily is not
recommended.

Nicardia (Nifedipine Tablets) have to be
stored at controlled room temperature
i.e. from 20°C to 25°C (68°F to 77°F)
with excursions permitted between 15°
to 30°C (59° to 86°F). The tablets
should be protected from light and
moisture.
Keep this as well as all other medicines
away from children and pets.

Excessive Hypotension: Normally in most patients
the hypotensive effect of Nifedipine is modest and
well tolerated, occasional patients have had
excessive and poorly tolerated hypotension.
Increased Angina and/or Myocardial Infarction:
Rarely, patients, specially those who have severe
obstructive coronary artery disease, have
developed well-documented increased frequency,
duration and/or severity of angina or acute
myocardial infarction upon starting therapy with
Nifedipine tablets or at the time of dosage
increase.

Beta-Blocker Withdrawal:
When stoppind a beta-blocker it is important
to taper its dose, if possible, rather than
stopping abruptly before starting treatment
with Nifedipine. Patients recently withdrawn
from beta blockers may develop a withdrawal
syndrome with increased angina.
Congestive Heart Failure: Rarely, patients
have developed heart failure after starting
Nifedipine.

Contraindications
Nicardia Retard is contraindicated in patients
with a hypersensitivity to Nifedipine or any other
component of this medication. Concomitant
administration of Nicardia Retard with strong
P450 inducers, such as Rifampin, is
contraindicated.
Nifedipine should not be used in cases of
cardiogenic shock.

➢The most common side effect reported with Nifedipine
(Nicardia Retard Tablets) was peripheral edema. The other
common side effects are Headache, Flushing/heat sensation,
Dizziness, Fatigue/asthenia, Nausea and Constipation.
➢The following Nifedipine side effects were reported with an
incidence of 3% or below in daily doses up to 90 mg:
➢Body as a Whole/Systemic: chest pain, leg pain
➢Central Nervous System: paresthesia, vertigo
➢Dermatologic: rash
➢Gastrointestinal: constipation
➢Musculoskeletal: leg cramps
➢Respiratory: epistaxis, rhinitis
➢Urogenital: impotence, urinary frequency

Symptoms observed with severe Nicardia
Retard (Nifedipine Tablets) overdosage
include loss of consciousness, drop in
blood pressure, heart rhythm disturbances,
metabolic acidosis, hypoxia, cardiogenic
shock with pulmonary edema.

Generally, overdosage with Nifedipine (Nicardia
Retard Tablets) leading to pronounced hypotension
calls for active cardiovascular support including
monitoring of cardiovascular and respiratory function,
elevation of extremities, judicious use of calcium
infusion, pressor agents as well as fluids.
After oral ingestion, thorough gastric lavage is
indicated, in case necessary in combination with
irrigation of the small intestine.
In cases involving overdose of a slow-release product
like Nicardia Retard, elimination must be as
complete as possible, including from the small
intestine, to prevent the subsequent absorption of the
active substance.

16. Nifedipine During Pregnancy

Nifedipine (Nicardia Retard Tablets) has been
classified by the US FDA as Pregnancy
Category C.
During animal studies Nifedipine has been
observed to have a variety of embryotoxic,
placentotoxic, teratogenic and fetotoxic effects,
including stunted fetuses , digital anomalies, rib
deformities, cleft palate, small placentas and
underdeveloped chorionic villi, embryonic and
fetal deaths, prolonged pregnancy and
decreased neonatal survival.
There are no adequate and well-controlled
studies conducted in pregnant women.
