A 30-year-old female came in for a routine obstetrical examination at 17 weeks of gestation. An ultrasound revealed her fetus had exencephaly, a lethal brain anomaly where the skull vault is absent and brain tissue protrudes out. Exencephaly occurs when the neural tube fails to close properly early in development. It is characterized by absence of the skull and herniated brain tissue dangling in the amniotic fluid. As exencephaly is essentially a lethal condition, the parents were advised to terminate the pregnancy.
