Les résidents pouvaient néanmoins se sous-spécialiser grâce à des brevets nommés capacités. Mais la refonte du troisième cycle des études médicales et l'obligation pour tous les étudiants de passer l'ECN, a tout ré-organisé sous la forme de diplômes d'études spécialisées (DES) dans un premier temps, permettant d'accéder a un panel de diplôme d'études spécialisées complémentaires (DESC) dans un deuxième temps. L'ensemble de ces deux diplômes constituent la maquette d'internat, maquette reprenant les enseignements théoriques et les stages suivis par l’interne, qui déterminera la profession future de l'étudiant.
Diplômes d’études spécialisées (DES):
Les DES sont les diplômes de base, qui aiguilleront le néo-interne dans une des filière principale de la médecine. Les DES ont une durée variable, allant de 3 ans (Médecine générale...) à 5 ans (Oncologie, Chirurgie générale...). Chacun donne l'accès a une liste déterminée de DESC qu'il sera possible de choisir une fois le DES validé.
