The document provides a health and safety risk assessment for filming a short film called "Broken" from September 20th to October 20th. It outlines various hazards that could occur during filming at different locations and rates them on a scale of 1 to 5 based on risk level. Hazards with the highest risk ratings of 5 include losing equipment, a wheelchair falling on train tracks, loose wheelchair brakes on a train platform, and an actor being hit by a car. Solutions for mitigating risks include checking equipment, having strong personnel transport gear, and clearing areas before filming dangerous scenes. The risk assessment aims to minimize chances of injury or problems during the production.

Scatter diagram is the graphical presentation of relationship between two variables. Scatter diagram is an important tool out of 7 fundamental tools of quality control. Scatter diagram helps to confirm the degree of relationship between cause and effect. Here cause is an independent variable and effect is dependent variable. Scatter diagram is an important statistical tool to analyze the relationship of two variables. To create the scatter diagram take the values of independent variable on X-Axis where as the dependent variable is taken on Y-Axis. Plot the intersection points of X and Y on the graph. Draw a straight line passing through all the points. Analyze the pattern of the points. For different degree of relationship different pattern of scatter diagram is formed. If Y increases as X increases and data points are on the straight line then there is perfect positive correlation. If Y decreases with increase of X and data points are on the straight line then there is perfect negative correlation. But when data is scattered all over the graph then there is zero correlation.

Operation “Blue Star” is the only event in the history of Independent India where the state went into war with its own people. Even after about 40 years it is not clear if it was culmination of states anger over people of the region, a political game of power or start of dictatorial chapter in the democratic setup.
The people of Punjab felt alienated from main stream due to denial of their just demands during a long democratic struggle since independence. As it happen all over the word, it led to militant struggle with great loss of lives of military, police and civilian personnel. Killing of Indira Gandhi and massacre of innocent Sikhs in Delhi and other India cities was also associated with this movement.

He discussed the concept of quality improvement, emphasizing its applicability to various aspects of life, including personal, project, and program improvements. He defined quality as doing the right thing at the right time in the right way to achieve the best possible results and discussed the concept of the "gap" between what we know and what we do, and how this gap represents the areas we need to improve. He explained the scientific approach to quality improvement, which involves systematic performance analysis, testing and learning, and implementing change ideas. He also highlighted the importance of client focus and a team approach to quality improvement.
2024.06.01 Introducing a competency framework for languag learning materials ...Sandy Millin
http://sandymillin.wordpress.com/iateflwebinar2024
Published classroom materials form the basis of syllabuses, drive teacher professional development, and have a potentially huge influence on learners, teachers and education systems. All teachers also create their own materials, whether a few sentences on a blackboard, a highly-structured fully-realised online course, or anything in between. Despite this, the knowledge and skills needed to create effective language learning materials are rarely part of teacher training, and are mostly learnt by trial and error.
Knowledge and skills frameworks, generally called competency frameworks, for ELT teachers, trainers and managers have existed for a few years now. However, until I created one for my MA dissertation, there wasn’t one drawing together what we need to know and do to be able to effectively produce language learning materials.
This webinar will introduce you to my framework, highlighting the key competencies I identified from my research. It will also show how anybody involved in language teaching (any language, not just English!), teacher training, managing schools or developing language learning materials can benefit from using the framework.

2. Name of Production: Broken
Filming Date’s: 20th September – 20th October
Task: To create a five minute short film
Personnel: Director, Cameraman, Actor and Actresses
Location’s:
• Jonathan’s household
• Oak’s Park, Park
• Newbury Park Healthcare centre
• Gants Hill Train Station
3. Outline
When we are filming it is essential that all members who are taking part, must be aware of all the
dangers which surround us. The main purpose of a risk assessment is to inform people about al the
risk which could occur when filming. This would allow the actors/actress to be more careful when
filming. I believe that doing a risk assessment it would minimise the chances of somebody actually
getting hurt or injured when filming.
4. Roles
Names Roles
Rikhel Trivedi Director/Sound
Hassan Shahid Cameraman/Sound
Thuvaragan Kuruparan Editor/Sound
Jonathan Gardner Protagonist
Donilla Actress
Manjayan Actor
Shajid Ahmed Actor
5. Hazards Risk Factor (1 to 5
with 1 = low risk
and 5 = high risk
Solution Why we gave this rating
The risk of getting fluids or
dust on the camera or the
tripod. 4
When moving the camera around to
get the different types of shots and
angles for our footage, I would be
constantly cleaning the location would
be making sure that there are no fluids
anywhere near the equipment.
I had given this rating mainly due to the fact that if the camera or
equipment gets damaged or tends to have permanent stains it
would mean that we wouldn’t be able to film on that particular
day. This is because our footage would not be at its best and there
are chances of our footage not being saved. Not only this, as a
group we would have to pay for all damages which would not be
pleasant for the group members and I.
Dropping the heavy
equipment when
transporting to location
when filming.
4
When we are transporting the
equipment to our location, it will
require some sort of physical strength.
In order, for someone not to get injured
we would need somebody who is
physically strong to carry our the
equipment.
The reason why I had given this rating is simply because moving
heavy equipment by a person who is not that strong, can injure
themselves and possibly damage the equipment. As for when
filming, if any or the equipment is damaged it will no longer mean
that we would be able to film with damaged equipment. Therefore,
the person carrying out the equipment must be able to cope in
rush hour conditions as well as holding heavy object for a long
period of time.
Losing any of the
equipment when travelling
to different locations.
5
To prevent any cables being lost whilst
the journey to our location. We could
perform a checklist with all the
equipment included. With having a
checklist and going over it before we
being transporting, it would reduce the
chances of losing cables or equipment.
As there will be only the cinematographer and I on the day of
filming, the chances of forgetting equipment or cables is high. With
losing equipment on the day we film, it may result to not allowing
us to film. The smallest things are important to us when filming
such as the SD card and battery’s. With losing equipment, it will
mean that it will be a waste of a day. We also would have to pay for
all lost equipment which may be high regarding what the losses.
6. Hazards Risk Factor (1 to 5
with 1 = low risk
and 5 = high risk
Solution Why we gave this rating
Getting dirt on the
camera and equipment.
3
To prevent any dirt getting on the
equipment, I will make sure that I clean
the camera from time to time. By
cleaning the camera and equipment
when moving it around it would reduce
the chances of any permanent stain from
getting on the equipment. Also I will
ensure that I clean the surroundings of
where the equipment is placed.
Obviously when filming at the park, the chances of dirt and mud
getting on the camera are high. If mud and dirt are on the camera it
may affect my captured footage. With the camera or equipment
having dirt on them it may result with the equipment getting
damaged.
Tripping over when
running to cast members
and to the camera .
2
Falling while running may hurt, however
the best way to avoid falling and injuring
myself is to make sure that I look where I
am running. I could also make sure that
when running around the set, that my
shoelaces are tied correctly. Although, I
will be rushing on the day, I must make
sure that I understand the circumstances
on the day.
If I fall whilst running around the set it would just lead to me being
more stressed on the day of filming. This could then result to me not
capturing correct footage. This would just make my day worse, also
this could lead to me actually getting injured on the day of filming.
With me being hurt it would mean that I wouldn’t be in the right
condition to film for my coursework.
Pets coming towards the
camera and scratching
the lens
5
When placing the camera down steadily, I
should make sure that there are no pets
or people walking past unless I plan to
have them for my short film. With being
patient for every person or pet walking
past, I would be able to avoid the camera
being damaged.
As I will be filming this scene at the park, I am bound to see his pets
loose with their owners. With the pets being loose the chances of
them actually coming towards the camera are high as the camera
may attract attention. Pets such as dogs tend to have sharp claws and
if they approach the camera when unattended it could result to the
camera lens becoming damaged. This would then lead to not being
able to film with the cameras condition.
7. Hazards Risk Factor (1 to 5
with 1 = low risk
and 5 = high risk
Solution Why we gave this rating
Moving the wheelchair
underground and falling
down the escalators
5
To prevent any personnel members actually
falling I would need to make sure that the
wheelchair is held stiff by two people. This
would be most likely be the actors and me, I
believe that this would help get down to the
underground easier. I would also make sure that
all members are going down the escalators at
the same time as if one member is left behind
this may become confusing.
The reason on why I had given this rating for this potential hazard is simply due
to the fact that going down the escalators is difficult when going down with
heavy equipment. As I would be carrying the wheelchair prop with me it would
be hard to move around when I have no access to no roads or pavements. When
going down and up the escalators I would need to make sure that I do not drop
the wheelchair. If the wheelchair does fall it could result to the main prop being
damaged.
Managing to fall onto the
railway tracks
5
Managing to fall onto the railway tracks is a
situation. This could result with a train coming or
my leg being stuck on the tracks. To make sure
that I do not fall onto the railway tracks I could
make request that the actors and other
personnel members look out for me just in case.
Also I could constantly look at the arrival board
for the incoming trains. This would help me
know, when trains are coming therefore,
knowing the times when I should film and when
I could plan more.
Filming at a train station platform is highly dangerous. The obvious reason is if I
or the other members fall on the railway tracks. This could be an issue as if there
is an incoming train it could conclude to somebody’s life being taken. By looking
at the news throughout the year, there have been many incidents where people
have accidently slipped onto the railway tracks. To make sure that none of this
happens to any of the members I could be aware of the yellow line which
demonstrates the limit on how close to get to the edge of the platform.
Brakes on wheelchair become
loose when at the edge of the
platform
5
This is similar to previous hazard. However, if the
wheelchairs brakes become loose when he is at
the end of the platform this would affect the
entire scene. For instance, the actor may feel
uncomfortable to act on the day knowing that
the brakes on the main prop where he will be
sitting on is loose. With filming at the edge of
the platform it builds on fear within the
personnel’s including myself.
Same reasons for the hazard above, the reason on why I had given the maximum
risk factor for this hazard is understandable. If the brakes become loose and the
actor is acting, there are chances that he may either tilt forward or backwards.
Before filming it is best that I make sure that all equipment including props are all
still working for filming. The risks for this scene are high however, this may build
fear within the actors and they may decide not to act for my production.
Therefore, I would need to make sure that no silly situations happen or occur
when filming this scene.
8. Hazards Risk Factor (1 to 5
with 1 = low risk
and 5 = high risk
Solution Why we gave this rating
The tires being punctured
when driving up and down
3
To avoid any tires being punctured when filming
the flashback of the accident, I could make sure
that I clear the roads before the car is in use
repeatedly. This would help reduce the chances
of the tires actually being punctured. Having a
punctured tire would cause a problem on the
day I film as I would need to wait until the drive
is ready to repair his tires. Therefore I would
need to make sure that no ties are punctured
during filming.
If tires are punctured when filming it could create problems related to my short
film. The driver wouldn’t be able to proceed with a punctured tire and this may
be time consuming which may become irritating for both myself and actors. As
much as I would like to continue with a punctured tire, the risks could result to
the car losing control which most likely lead to a car accident. Furthermore, the
reasons for why I chose this rating is understandable yet irritating.
Actor getting hit by the car
5
If the actor accidently gets hit by the car whilst
performing the scene, we would need to call the
ambulance immediately regarding the damage
caused. To make sure that this wouldn’t happen I
would need to tell the driver to watch carefully
when driving.
The reason on why I gave this hazard this rating is simply due because using a car
for my production is going to be long. This increases the chances of hazards
happening on the day of filming. Any problems relating the car could occur which
could lead to an accident happening. With making sure that my driver has a full
MOT service, I would still need to make sure that no accidents happen. Also, I
would need to confirm with my actor if they would be willing enough to film with
involving a car.
Driver braking too hard and
crashing into another vehicle
5
To prevent from any accidents involving other
vehicles, I would need to make sure that no cars
are incoming from both sides of the road. If the
driver happens to use the emergency brake and
crashes into a nearby vehicle, we would need to
react quick making sure everyone is okay. After
this we would need to contact both insurance
companies to make them aware of the situation.
We would speak to the two drivers and advise
them on what actions to take from now on.
Damaging another person’s vehicle’s is a big incident. It would be mainly our
productions fault and we may have to pay the following bill. Therefore to prevent
from any accidents happening, I would need to make sure that all cars are not
coming towards the scene. If we have to wait for cars to go then we will. Our
production doesn’t want bad reputation or to explain to an insurance company,
therefore we would need to make sure no accidents happen.
