This document provides instructions for creating forms and surveys using Google Forms in 5 steps: 1) Enter Google Docs and create a new form, 2) Create questions and customize the form, 3) Choose a theme, 4) Share the form via email or link, and 5) Edit existing forms by pulling down the form menu to edit questions or theme.

4. Explain the purpose

5. Create your Questions

6. Enter comments about your question, if needed.

7. Choose the type of responses you expect from a drop down menu of choices.Question TypesText: If you expect a brief reply. IE: name, user id

8. Paragraph Text – If you want more descriptions, comments

9. Multiple Choice – If you want the user to select one answer from options. There is an option for ‘other’ so they can input a choice, and an option to go to a different page, depending on response

10. Checkboxes –allow for more than one response
