Farmakologi

4. Pendahuluan
Farmakologi didefinisikan sebagai ilmu
tentang bahan-bahan yang berinteraksi dengan
makhluk hidup melalui proses kimiawi,
khususnya melalui ikatan dengan molekul-
molekul pengatur (regulatory molecules) serta
mengaktifkan atau menghambat proses-proses
normal tubuh.
Bahan-bahan ini dapat berupa bahan kimia yang
diberikan untuk mencapai suatu efek terapeutik
yang berguna pada suatu proses dalam tubuh
pasien atau untuk efek toksiknya terhadap
proses-proses regulatorik dalam tubuh parasit
yang menginfeksi pasien.

Penyerapan obat dari saluran pencernaan
(kebanyakan dari ileum) dipengaruhi oleh
berbagai faktor :
- Motilitas Usus
- Muntah
- Jenis Obat Lipid penyerapan lebih baik
dari Obat non-lipid
- pH (obat asam sebagian besar diserap
dari lambung, dan obat alkali paling baik
diserap dari area basa usus)
Obat oral masuk ke vena portal hepatik
dan diangkut kehati. Hati adalah tempat
utama untuk metabolisme obat, sehingga
persentase obat yang diserap akan diubah
secara kimiawi oleh metabolisme saat
melewati hati.

9. Metabolisme awal oleh hati ini disebut Metabolisme
lintas pertama, hanya berlaku untuk pengobatan
oral, karena pemberian melalui injeksi tidak melibat
kan vena portal hepatik. Jumlah obat aktif dalam
sirkulasi sistemik, baik secara langsung (via injeksi)
atau melalui hati (dari asupan oral), disebut
ketersediaan hayati.

11. TAHAP DISTRIBUSI
 Sirkulasi Sistemik,
 Sistem getah bening
Terikat dengan protein darah,
 Yang paling umum adalah albumin
 Glikoprotein,
 Lipoprotein,
 Gammaglobulin
Tidak terikat, yaitu terlarut dalam plasma.
Ikatan dengan protein darah, yakni mengangkut obat ke tempat penggunaannya,
dan juga menciptakan reservoir obat. Saat obat menjadi terlepas dari protein,
mereka mendapatkan kembali aktivitasnya, yaitu mereka menjadi tersedia secara hayati.
Jaringan sasaran adalah sel yang memiliki reseptor ligan sehingga menjadi target
dimana obat memberikan efek.

12. TAHAPAN METABOLISME
 Perubahan kimia atau biotransformasi yang diperlukan
untuk sebagian besar obat agar dapat diekskresikan.
Terutama untuk menjadi obat yang bersifat Hidrofilik,
misalnya Obat yang berbasis lipid dieksresikan
sebagai urin,
 Untuk membuat obat aktif menjadi kurang aktif,
 Hasil akhir dari perubahan kimia ini disebut Metabolit
(produk biasa untuk ekskresi).
 Sebagian besar metabolisme obat terjadi di hati.
Sebagian lagi terjadi di sirkulasi atau di ginjal.
 Metabolisme hati menggunakan sistem enzim

13. Fase Utama Metabolisme Hati
Fase I, biotransformasi obat untuk memberikan muatan elektrostatik, yang memungkinkannya untuk
menyatu lebih baik dengan air, karena molekul air bersifat polar), sehingga dapat dikeluarkan melalui ginjal.
Fase ini menggunakan sistem enzim, dikenal sebagai sitokrom P-450 (CYP, atauP450),
yang terlibat dalam banyak metabolisme obat. Enzim ini adalah protein yang ditemukan dalam mitokondria
atau diretikulum endoplasma (ER). Sistem enzim CYP terlibat dalam sekitar 75% metabolisme obat.
Fase II, konjugasi obat atau produk akhir fase I dengan zat endogen. Konjugasi berarti menggabungkan
satu zat (obat atau produk fase I) dengan zat kedua (kebanyakan produk alami hati). Fase ini untuk
mengurangi aktivitas sebagian besar obat.

14. SIKLUS ENTEROHEPATIK
Obat yang masuk ke sistem pencernaan
secara oral melalui tahap Metabolisme
lintas pertama, menghasilkan metabolit
untuk ekskresi melalui ginjal.
Obat yang tidak dimetabolisme memasuki
sirkulasi sistemik dikenal sebagai bioavailabilitas
Beberapa metabolit diekskresikan melalui
Empedu lalu diekskresikan dalam tinja.
Beberapa re-absorpsi metabolit akan
berlangsung dari usus kembali ke hati, yang ke
mudian mendaur ulang melalui empedu kembali.
Setiap siklus enterohepatic akan berkurang dari

15. TAHAPAN ELIMINASI
Sebagian besar eksresi metabolit obat melalui system ginjal (komponen urin),
Jauh lebih jarang, beberapa obat dieliminasi melalui usus (feses),
dan sangat jarang melalui kulit melalui keringat dan air susu ibu.
Gas anestesi sebagian besar dihilangkan melalui paru-paru selama pernafasan

16. Waktu Paruh
Waktu paruh (t1/2) adalah waktu yang diperlukan
untuk mengubah jumlah obat dalam tubuh menjadi
separuhnya sewaktu eliminasi.
Laju metabolisme oleh Hati dan laju ekskresi oleh
ginjal merupakan faktor utama yang menentukan
waktu paruh obat.
Waktu paruh sangat singkat (< 1 jam),
waktu paruh pendek (1 - 6 jam),
waktu paruh intermediet (6 - 12 jam)
waktu paruh Panjang (12 - 24 jam),
waktu paruh sangat panjang (> 24 jam).

18. Obat yang Bekerja pada Reseptor
Reseptor pada permukaan sel otak terikat dengan neuro
transmitter dan Neuromodulator sehingga memicu
serangkaian perubahan di dalam sel.
Perubahan ini dapat dilakukan dengan cepat melalui ion
otropic reseptor, atau dalam periode yang lebih lama
melalui metabotropic reseptor.
Proses ini juga dapat meningkatkan aktivitas seluler
(eksitasi) atau menurunkan aktivitas seluler (inhibitori)

19. PRINSIP FARMAKODINAMIKA
Sebagian besar obat harus berikatan dengan reseptor agar dapat menimbulkan efek.
1. Reseptor menentukan hubungan kuantitatif antara dosis atau
konsentrasi obat dan efek farmakologik.
2. Reseptor menentukan selektivitas kerja obat.
3. Reseptor memperantarai kerja agonis dan antagonis farmakologik.
Sebagian obat dan ligan alami (hormon dan Neurotransmiter),
Terikat pada reseptor dan mengaktifkan/ merangsang reseptor disebut
sebagai agonis; dan antagonis farmakologik yaitu yang berikatan
dengan reseptor, tetapi tidak mengaktifkan pembentukan sinyal;
Karena obat ini mengganggu kemampuan agonis mengaktifkan reseptor.

20. Kerja Obat sebagai Agonis dan Antagonis
 Obat agonis, contohnya agonis dopamine : bromokriptin
 Dopamin adalah ligan alami untuk reseptor dopamin
 Antagonis dopamine : klorpromazin
Antagonis kompetitif adalah dua atau lebih obat yang bersaing untuk tempat berikatan yang
sama. Setiap obat biasanya memiliki 50% kemungkinan sehingga hasil terapi mungkin kurang
pasti. Karena itu, meresepkan dua obat sekaligus yang merupakan antagonis kompetitif
biasanya tidak dibenarkan dan sebaiknya dihindari.
Agonis parsial adalah obat-obatan yang mengikat reseptor dan mengaktifkan reseptor tetapi
tidak pada tingkat yang sama seperti agonis penuh atau ligan.
Dalam beberapa hal, agonis parsial bertindak dengan cara yang mirip dengan antagonis karena
dengan mencegah ligan mengikat, mereka mengurangi aktivitas ligan di otak, seperti yang dilak
ukan antagonis.

21. DOSIS & RESPONS PADA PASIEN
Untuk memilih obat yang tepat dan menentukan dosis yang sesuai,
dokter harus memahami potensi farmakologik relatif dan efikasi
maksimal obat dalam kaitannya dengan efek terapetik yang diinginkan.
Potensi merujuk kepada konsentrasi atau
dosis suatu obat yang dibutuhkan untuk m
enimbulkan 50% dari efek maksimal obat.
Potensi suatu obat sebagian bergantung
pada afinitas (Kd) reseptordalam mengikat
obat dan sebagian pada efisiensi keterkait
an interaksi obat-reseptor dengan respons

22. Obat Yang Bekerja Pada Saluran Ion
Saluran ion ada di hampir semua membran saraf dan ketika saluran ini terbuka,
mereka memungkinkan lewatnya ion masuk atau keluar sel. Kanal ion (terutama
Na+dan K+ saluran) sangat penting selama impuls saraf (potensial aksi)
transmisi sepanjang akson.

Beberapa aktivitas sel bergantung pada transportasi zat, seperti neurotransmiter,
Misalnya, penggunaan obat antidepresan tertentu yang menghambat pompa

25. Biosimilar obat adalah teknik yang sedang dikembangkan untuk membuat obat yang
sangat mirip dengan zat alami (yaitu yang berasal dari sumber organik alami dan
disebut sebagai 'referensi' atau ‘originator medicine’).
Jendela terapeutik adalah rentang dosis yang ada dari dosis efektif terendah obat
hingga dosis di mana ada lebih banyak efek berbahaya daripada efek menguntungkan.
Di bawah dosis jendela terendah obat tidak efektif dan tidak berguna.
Di atas jendela dosis tertinggi obat, potensi efek samping yang parah dan risiko over
dosis. Beberapa obat memiliki jendela terapeutik yang sempit dan harus diresepkan
dengan hati-hati terhadap dosis yang menyebabkan toksisitas

dalam praktek klinis, tidak ada
perlindungan hukum melalui merek
dagang, paten, atau
hak cipta atas nama obat ini. Dengan
demikian, nama ini mengidentifikasi
obat tetapi bukan milik
perusahaan obat tertentu.
• Nama Dagang, ada hak milik nama
yang diberikan kepada obat oleh
perusahaan farmasi tertentu
yang memproduksinya.Hak milik berarti
nama obat dilindungi secara hukum
oleh hak cipta, merek dagang,
atau paten. Nama dagang tidak
digunakan pada resep UK NHS karena
