Effects of physical exercise in cerebrovascular mechanisms of adaptation to a...Enrike G. Argandoña
Microvascular environment plays a fundamental role in the adaptive response to increases in activity, involving an increase in energy demand. To puntual increases in demand, the response is effected by changes in local flow, but if demand remains high, angiogenic process is triggered through the formation of new capillaries from preexisting vessels. The trigger factor of angiogenesis is local neuronal hypoxia as result of increased activity, mediated by angioglioneurins whose main exponent is VEGF. There are situations in which hypoxia is not from an increased activity, but a decrease in available oxygen, as occurs in the altitude exposure.
Our goal is to study the changes underlying microvascular acute adaptation to moderate altitude and the possible effects of physical exercise.
Wistar rats bred to 400 m (P47) and transported to a height of 2800m on a journey of 35 minutes. We used the following cohorts of 7 rats each: a group was housed indoors in standard laboratory conditions, another group had free access to
an exercise wheel. As controls, we used animals born at 400m. After 48 h, animals were sacrificed and after neuropathological examination by conventional histology, capillary density was quantified using computer assisted stereology on thick sections processed using Butyryl Cholinesterase Histochemistry (Argandoña, Brain Res, 1996).
Three cortical areas were quantified, motor, somatosensory and visual cortices.
Rats with free access to exercise wheel showed higher vascular density in response to hypoxia, while the rest showed similar values. Differences were significant in all three areas, being highest in the motor cortex.
The acute adaptation to moderate altitude is an increasingly relevant issue given the increase in displacements at moderate or high altitudes, with serious consequences such as Acute Mountain Sickness, which in its most severe degree leads to
Brain edema. Our results show that exercise can play a protective role through adaptive angiogenesis.

Martin Bardsley: analysis of virtual wardsNuffield Trust
This document summarizes a presentation on virtual wards, which are a form of case management that integrates health and social care. It describes predictive modeling to identify high-risk patients for virtual ward enrollment and the multidisciplinary staffing of virtual wards. Evaluations of virtual ward programs in different locations found they enrolled complex, high-cost patients but did not consistently reduce emergency admissions or costs in the short-term. Implementing large-scale service changes takes time and hospital use is not the only impact measure of these programs.
Virtual Maternity Unit in Second Life IRVW2011Colleen McCants
This presentation covers the creation and use of the University of Nottingham's Virtual Maternity Ward in Second Life. It was created by Colleen McCants, (artist, builder) and delivered at the Innovative Research in Virtual Worlds 2011 conference in Coventry in conjunction with Jenny Bailey (midwife) and Fay Cross (programmer). Jenny Bailey has recently been nominated for a Royal College of Midwives award for her work on the VMU.
In November 2009, the IS Learning Technology Section and the Academic Division of Midwifery at the University of Nottingham began construction on a Virtual Maternity Unit (VMU) on the University of Nottingham's web campus in Second Life. In the Summer of 2010, a small group of midwifery students and educator peers took part in the pilot, and in March 2011 it began to be used by first year midwifery students on a voluntary basis. The potential benefits of immersive learning and storytelling role-play in Second Life and other virtual environments are being explored by many healthcare educators, but the implementation of the physical and interactive design varies according to the learning objectives and collaborator skill sets. The VMU was not the first Higher Educational midwifery sim to be developed, as it was created in parallel with the Second Life Education New Zealand project, (SLENZ), which was further along on the development timeline.
Midwifery education traditionally relies on sharing information by narrative means, but students can find 'real life' role play situations awkward. The Virtual Maternity Unit enables students to immerse themselves, with a certain degree of anonymity, in the role of midwife, with the teacher in the role of a woman in labour. Through being involved in a narrative over which they have choices in care and support, the student midwives can draw on their first-year training and put it into a meaningful context, learning from mistakes and reviewing the scenario afterwards with the midwife-educator. Immersion is enabled by familiar equipment and facilities, and documentation is modeled on real paperwork and sample birth plans. The teacher is able to guide the scenario by controlling feedback such as temperature, blood pressure and stage of labour via the pregnant mother's HUD, (Heads Up Display). The timescale can also be compressed to encompass a normal series of birth events. Because midwifery entails duty of care over mother and baby, student midwives must learn to communicate well, respond appropriately and act decisively, ensuring the safety and well-being of all. The VMU provides a safe environment in which to practice doing so; one in which they can become accustomed to thinking on their feet, yet have freedom to explore and question. At present, the VMU is being used in midwife educator-led sessions, taking place several times weekly, with more sessions held on our Derby campus. Role-play in the VMU is considered a useful addition to training and experience on the real-life war
A Modern Approach to Healthcare:Bridging Dentistry, Medicine, Pharmacy, and ...Brian Bergh
The document discusses trends in healthcare delivery that will require greater coordination and integration between medical professionals. As the population ages and chronic diseases increase, healthcare costs are rising significantly. This will necessitate more preventative and comprehensive care that relies on a team-based approach using all levels of healthcare providers. New technologies and data sharing will also be needed to ensure proper treatment and avoid errors from uncoordinated care.
The Concept of a Virtual Ward needs some explaining. In Seaham in County Durham we identified the most vulnerable and elderly patients through predictive risk modelling and developed a service which would improve their clinical outcomes, improve their experience, reduce costs through reducing use of unplanned care, and improve working lives.
Although this only ran for 12 months in pilot form, it has since been extended across 170,000 patients in Easington and Sedgefield in County Durham

1) Skilled nursing facilities often fail to meet Medicare discharge planning requirements, with 31% not meeting at least one requirement. Common deficiencies include lacking post-discharge plans of care and adequate discharge summaries.
2) The Office of the Inspector General recommended that CMS increase regulations on discharge planning, improve care planning and discharge processes, hold facilities accountable, and link payments to meeting requirements.
3) Poorly managed care transitions can diminish health and increase costs. Exceptional discharge planning that begins before admission and continues after discharge can improve health outcomes and decrease costs by ensuring patients and families have the knowledge and support needed for safe transitions to the next care setting or home.
Hospital Discharge Instructions using Mind MappingJosé M. Guerrero

Key changes included emphasizing high-quality chest compressions for basic life support over ventilation. The guidelines also changed the recommended compression-to-ventilation ratio for single rescuer CPR from 30:2 to 30 compressions continuous followed by 2 breaths. Additionally, the A-B-C sequence for single rescuer CPR was changed from Airway, Breathing, Chest compressions to
Parameter estimation for the truncated weibull model using the ordinary diffe...Hideo Hirose
In estimating the number of failures using the truncated data for the Weibull model, we often encounter a case that the estimate is smaller than the true one when we use the likelihood principle to conditional probability. In infectious disease predictions, the SIR model described by simultaneous ordinary differential equations are often used, and this model can predict the final stage condition, i.e., the total number of infected patients, well, even if the number of observed data is small. These two models have the same condition for the observed data: truncated to the right. Thus, we have investigated whether the number of failures in the Weibull model can be estimated accurately using the ordinary differential equation. The positive results to this conjecture are shown.

Mobile phones became multi-purpose devices supporting their users with large variety of applications for various tasks. Not only the number of available applications is increasing, also the number of applications people are using on their devices is growing, as well as the amount of time people spent on their smartphones daily is getting bigger. In this workshop paper, we briefly describe our past work on understanding mobile application usage. We explain our research tool for measuring mobile application usage, called AppSensor, and discuss possibilities to exploit the information of mobile application usage to inform the reasoning about users’ contexts. We contribute our source code to the workshop for a discussion and prototyping of use cases leveraging the information of which application a user is currently using.

Solving for x - a solution to the healthcare conundrum?Wayne Pan
This document discusses moving healthcare payment models from fee-for-service to value-based systems. It argues that fee-for-service incentivizes volume over quality and has led to rising costs. Value-based models where providers accept risk could better align incentives with the "Triple Aim" of improving outcomes, reducing costs, and improving experience. This involves measuring quality comprehensively, focusing more on wellness and managing populations' clinical and financial risks. Pharmaceutical companies may also take on more risk through accountable treatment organizations managing medication therapy. The goal is true measurement of quality to solve for value in healthcare.
The document discusses the pharmaceutical industry's growing interest in digital health technologies. It mentions that companies are looking at technologies like machine learning, genomics, wearables, home monitoring, and telehealth to better understand patients, expedite drug discovery and development, and enable new financing models. The industry is seeking to hire data scientists, bioinformaticists, behavioral specialists, and mobile developers with skills in these digital technologies. Genentech's involvement in digital health is also briefly referenced but not described.
Some thoughts about healthcare IT systems 1OCT13Wayne Pan
This document discusses improving healthcare IT systems to focus on person-centered care rather than patient-centered care. It notes that current EHR systems are designed for provider-centered care and can result in multiple patient identities for a single person across different providers. The document introduces SyntraNet as a system designed for person-centered care. It outlines the 5Cs of connecting, communicating, collaborating, coordinating, and calculating as key to achieving person-centered care through an advanced healthcare IT system.
The document announces a spring 2012 health horizons conference in San Francisco featuring Wayne Pan, MD, MBA as a speaker. Pan is the Chief Medical Informatics Officer of Health Access Solutions and Advisory Chief Medical Officer of Kavaii Analytics. The conference will take place in San Francisco.
The document discusses challenges in managing healthcare for patients in an Accountable Care Organization (ACO) model within a traditional fee-for-service system. It proposes a solution to make care coordination and quality accountability feel similar to a traditional HMO model by: 1) Tracking provider referrals and specialty visits, 2) Guiding provider selection based on quality/efficiency, 3) Incentivizing referring providers, 4) Allowing real-time patient tracking, and 5) Reinforcing quality at every patient encounter. The goal is to create a "virtual patient-centered medical home" model to manage ACO patients.

Enhancing patient care through better dataWayne Pan
This document discusses enhancing patient care through better data and care coordination. It describes how the Santa Clara County IPA Medical Group implemented a common web-based communication platform to facilitate care coordination across providers and settings. The platform allows sharing of patient data, embedding of quality reminders into workflows, and patient access to their own information. Through coordinated efforts of hospitalists, case managers, and utilization review staff using this platform, the group aims to improve the patient journey from hospital to post-acute care to primary care follow up.
Bringing patient centered health care to the patient by integrating retail ph...Wayne Pan
The document discusses bringing patient-centered health care to patients by integrating retail pharmacy. It describes SCCIPA's journey to address readmissions by improving care coordination and transitions through medication reconciliation and physician follow-up post-discharge. It outlines SCCIPA's pilot project to leverage AccessExpress and retail pharmacies to help patients understand their medications after leaving the hospital.
This document summarizes a presentation given by Dr. Wayne Pan of the Santa Clara County IPA (SCCIPA) on 5 "dangerous ideas" for improving healthcare quality. The ideas are: 1) Ask how health plans can help providers rather than what providers can do for plans, 2) Require providers to use a common EHR platform for communication, 3) Use the EHR platform to provide performance feedback, 4) Implement physician bonus programs tied to quality targets, and 5) Focus on coordinating care for the whole patient through reminders, checklists and follow-ups. The document provides examples of how SCCIPA has implemented these ideas, such as engaging office staff, using data to drive feedback, and linking

Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
