This study aims to systematically compare and
contrast the two most commonly used techniques of
tonsillectomy- Cold tonsillectomy and cobilation tonsillectomy.
Three different age group of patients were examined and
operated. The total number of patients was 104. There were 52
patients each of cobilation and cold tonsillectomy. The
specifications being following: 35 patients of age between 3 to 7
years, 6 patients of age 7-12 years and 11 patients of age greater
than 12 years.
The result of this study showed that there is no significant and
noticeable difference between the two procedures of operating
tonsils. However slight differences in the post operative pain and
primary and secondary bleeding was seen. The operation time
was considerably lower in patients of younger age.

Dressing A (Mepilex Border Post-Op) absorbed the largest volume of blood before reaching the dressing edges or leaking, with good dispersion throughout. The results indicate Dressing A will require fewer dressing changes and better protect wounds from contamination compared to the other dressings tested.

It found that over 50% of patients reported experiencing wound pain based on formal pain assessment tools, even though some did not report pain to their podiatrist. Pain was described as tender, brief, or intermittent. Both neuropathic and neuroischaemic ulcers had low reported pain intensity and little effect on quality of life. The study concludes wound pain may be underassessed clinically and more research is needed to understand differences in pain between ulcer types.
EWMA 2014 - EP485 EVALUATION OF MR ANGIOGRAPHY RESULTS IN DIABETIC FOOT PATIENTSEWMA
This document evaluates the results of MR angiography (MRA) in diabetic foot patients to detect peripheral arterial disease (PAD). A cross-sectional study of 100 diabetic patients with clinical symptoms of PAD found that while 32% had abnormal ankle brachial index measurements, 63.4% showed abnormalities on MRA. The conclusion is that while ankle brachial index is trustworthy for detecting severe ischemia, MRA should be used for other degrees of ischemia as it can detect more cases of abnormality than ankle brachial index alone. More patients need to be studied to determine what types of abnormalities found on MRA are associated with outcomes like healing or amputation.

This study examined the effect of diabetic foot ulcer (DFU) wound fluid pH on the presence of bacteria. Wound fluid was collected from 55 patients with DFUs and tested for pH and bacteria. A variety of bacteria were found, most commonly Staphylococcus and Streptococcus species. Higher pH wound fluid (>7.5) was associated with some bacterial species while others were found more often at neutral pH (<7.5). DFUs with clinical signs of infection had significantly higher pH than non-infected wounds. The results suggest that wound fluid pH, especially values over 7.2, could help identify silent infections and inform treatment decisions.

This document compares angiographic findings between patients with non-healing diabetic foot ulcers (DFUs) with and without Charcot foot (CF). It found that patients with CF and DFUs had less severe peripheral arterial disease (PAD) based on angiographic findings compared to DFU patients without CF, however revascularization rates were equal between the two groups. The study assessed 29 CF patients and 48 non-CF patients with DFUs who underwent lower limb angiography. It found patients without CF had significantly more frequent presence of severe PAD (Graziani class 4 and higher) versus the CF group.

This study assessed foot self-care knowledge and practices in 350 Saudi patients with diabetes referred to a specialized diabetes care center. While 42.6% had excellent knowledge of how diabetes affects foot health, only 17.7% demonstrated good ability to identify and apply appropriate foot self-care practices. Most patients acquired knowledge through media rather than primary health care clinics. The quality of knowledge from primary care clinics did not translate to better ability to practice self-care compared to other sources of knowledge. The study concludes current foot self-care education and knowledge is unsatisfactory and calls for a national diabetes control program delivering unified, evidence-based education through healthcare providers and media to prevent limb loss.

This study examined the relationship between initial transcutaneous oxygen pressure (TcPO2) measurements and healing of diabetic foot ulcers over 6 and 12 months. TcPO2 is a non-invasive method to detect peripheral arterial disease. The study found that 76% of patients had a TcPO2 of ≤30mmHg and 81% had a TcPO2 of ≤40mmHg, indicating peripheral arterial disease. Healing rates were higher in patients with a TcPO2 >30mmHg or >40mmHg. Using a TcPO2 threshold of 40mmHg rather than 30mmHg showed higher sensitivity and negative predictive value for predicting healing. The study concludes that a TcPO2 threshold

INTRODUCTION

5. MATERIAL AND METHODS
Descriptive study in 15 patients of a primary health service with venous
ulcers of degree III and signs of critical colonization and infection with the
cadexomer iodine®.
We considered, regarding the skin, painful wound, surrounding area of the
wound, exudation, colonization, necrosis, and the need to have a use full
solution. The control of the humidity, colonization and reduction of pain were
the most important goals.
Previous deciding not to use silver dressings, we reviewed guidelines SIGN
(Scottish Intercollegiate Guidelines Network), NICE (National Institute for
health and clinical Excellence) Cocrhane Library.
Finally decided to use SIGN to reflect the most recent evidence on
chronic venous leg ulceration.

9. RESULTS

A Cochrane systematic review reviewed three RCTs comparing
silver dressings with hydrocellular foam, alginate or best practice.
Two studies showed no difference between products and one
reported statistically faster healing rates but did not report complete
healing. All three studies were small and of short duration and could
not be combined for a meta-analysis.49
An RCT (n=213) comparing, silver dressings and non-adherent
dressings (in addition to standard compression), found no significant
differences in median time to complete healing or healing rates at
three, six and 12 months.
A Silver dressings are not recommended in the routine
treatment of patients with venous leg ulcers
10. RESULTS
After application for 2 weeks with wound dressing we observed a
good evolution of skin surrounding the wound, a reduction of
exudate, the removal of necrotic tissue, the reducing the size of
the ulcer, an atraumatic treatment and the supply of an adequate
humidity to the wound frequently to absorb according to the
necessities of the wound simultaneously.
After the application to 15 patients during 10 days, we observed
the reorganization of the edges of the wounds skin, exudate
control in each patient without macerating the new skin, the
reduction of the dressing changes to 2 times per week and the
easy procedure in the application by the professional as for the
patient at the time of making the cure.
Being an atraumatic treatment to provide the wound with the right
humidity frequently.

11. DISCUSSION
Our performances are had to be based on scientific
evidence to guarantee a quality attention.

We support this new technology in dressings, which
unites the characteristics of reducing costs, and
improving at the same time healing rates and the wound
treatment itself. There is no doubt that this new wound
dressing, CADEXOMER IODINE®, meets this requirements.

12. CONCLUSIONS
After this first experience with the use of these dressings
we saw a reduction of the dressing change, an excellent
exudate control with a lower risk of macerations.
The wound dressings were well tolerated and the easy
applicability and painless removability are much valued
by both treating persons and patients.
Evidence of variation in both healing rates and
recurrence rates of venous leg ulcers highlights the
need for an updated evidence based guideline to
support practice.
