1. Can you describe to me in your own words how you are feeling today and of late? This is an open-ended question to provide some insight on the patient, such as feelings, attitudes, and thoughts, mood, and how he perceives his well-being expressing his anxiety, any depression, and fatigue which should be taken into consideration.
2. Do you feel your medications all work well for you? This gives an insight to if the client is taking his medications as prescribed. Individuals with attention-deficit/hyperactivity disorder (ADHD) may usually be non-compliant with their medication because of symptoms associated with the disease such as hyperactivity, impulsivity, and inattention (Stahl, 2013).
3. Can you tell me what you mean by “torn in many directions”? This will help understand and assess if the client is pressured form work or from home and how it’s interfering with his social life.
4. Do you have thoughts of hurting yourself or others? Because the client expressed feeling like he is “torn in many direction,” in addition to his increased anxiety and high energy levels, it is important to assess the client for suicide ideation and depression.
Identify people in the patient's life you would need to speak to or get feedback from to assess the patient's situation further. Include specific questions you might ask these people and why.
To get feedbacks to further assess my client, I would speak to his wife to provide some history in regards to his moods over the years, any past triggers, his routine sleep/wake cycle, and also any information on how he was in his earlier years. From the information given, the client did show signs of ADHD as a kid. If the mother was present, I would ask about family history of ADHD and any other history of mental health disorder when the client was young. According to Starck, Grünwald, and Schlarb (2016), about 40% of ADHD children have at least one parent with clinical ADHD symptoms.
Explain what physical exams and diagnostic tests would be appropriate for the patient and how the results would be used.
Diagnosis of ADHD is based on clinical evaluation; therefore, no laboratory-based medical tests are available to confirm the diagnosis; however, basic laboratory studies that may help confirm diagnosis and aid in treatment are as serum CBC count with differential, electrolyte levels, thyroid function tests, and liver function tests before beginning stimulant therapy (Soreff, 2018). Other sources for exams or diagnostics are ADHD symptom checklists and a standardized behavior rating scales (Children and Adults with Attention-Deficit/Hyperactivity Disorder [CHADD], 2019). Not only do children and adults have different presentations for ADHD, but males and females may also have vastly different clinical presentations. Different people may also respond to medication therapies differently. For example, some ADHD medications may cause children to experience stomach pain, while others can be highly addictive for adults. In your role, as a psychiatric nurse practitioner, you must perform careful assessments and weigh the risks and benefits of medication therapies for patients across the life span. Attention deficit hyperactivity disorder is a lifelong neurodevelopmental disorder that affects the brain and results in a variety of inappropriate and maladaptive behaviors. ADHD is not a disease (Kajander 1995) but is a processing deficit that results in children having difficulty with inhibitory control. That is, children with ADHD lack self-control, something they cannot help. In children with ADHD, the parts of the brain that control attention and stop inappropriate behavior are underdeveloped (Barkley 1996; Kajander 1995). ADHD occurs three times more frequently in male students than female students (Reeve et al. 1995) and commonly occurs with other disorders. For example, ADHD occurs in 20 percent to 50 percent of the students with learning disabilities, in 65 percent of the students with oppositional defiant disorder, and in 20 percent to 30 percent of the students with conduct disorder (Reeve et al. 1995; Barkley 1990).

Using Second Life Avatars and Machinima to Introduce Sustainability into the University Curriculum: Evidence from Two Funded Pro-Environmental Behaviour Studies.
In 2011 the Higher Education Academy (HEA) in the UK funded seven projects to look at Education for Sustainable Development (ESD) in the Professional Curriculum. Cyberspace technologies featured strongly in utilising a wide range of professionally-accredited undergraduate degrees to explore the ways in which interdisciplinary awareness of sustainability issues is encouraged or prevented by professional requirements. The first project reported here “Problem-based Learning in Virtual Interactive Educational Worlds for Sustainable Development” (PREVIEW-Sustain) exploited the distinctive properties Second Life by using problem-based teaching methods with digital avatars in (the virtual) world.
In a previous JISC-funded project led by the author with Aston University and the ‘HEA Psychology Network’ we previously validated, transferred and disseminated immersive cyber-activities and materials adapted from Coventry University’s Second Life ‘PREVIEW’ project for use in mental health awareness and Psychology teaching. The follow-on PREVIEW-Sustain project reused and transferred these teaching methods to introduce Sustainability education to academic staff and students across two university subject groups (Psychology and Geography) by using customised online problem-based scenarios.
This presentation reports the virtual world methods developed and redeployed for the Sustainability agenda. The work existed entirely in the online virtual world populated by highly personalised 3D digital avatars. We conducted a series of learning scenarios with University staff and students highlighting motivational and behavioural factors that impact on real-world environmental sustainability (e.g., resources, recycling and energy efficiency). These materials are available to the wider teaching community. The project’s ‘virtual’ infrastructure is in place within the virtual world Second Life.
The online videos of the Second Life avatar interactions we developed, to further engage the community, later provided the basis for a follow-on research project which will also be reported here. We used filmed avatar interactions to assess changes in real world pro-environmental behaviour. We embed these videos in the University’s online teaching ‘virtual Learning Environment’ and assessed students before and after exposure to the environmental messages in the machinima.
Innovative cyberspace teaching and learning techniques offer flexible, cost-effective and rapidly deployed Higher Education solutions. Further research using similar virtual world techniques is planned that will explore Disability Awareness.
