1. Современный дизайн электрода (Advanced
cochlear implant electrode) и сохранение
слуха
Новые критерии отбора пациентов с остаточным слухом диктуют
необходимость разработки новых электродных систем и новой
хирургической тактики. При этом основным условием является их
атравматичность. Любой хирургический подход должен исключать
возможность введения электрода в лестницу преддверия и
возможность повреждения сохранных структур улитки.
Сохранение остаточного слуха будет способствовать не то лько
улучшенному восприятию речи, но и восприятию музыки. В
будущем сохранение даже нескольких волосковых клеток
может иметь значение для регенерации ВК, стимулируемой
введением нейротрофинов или других веществ
непосредственно через модифицированную электродную
систему.

Basic principle of CIs is to stimulate the spiral ganglion cells within the cochlea. The spiral ganglion is shown (green) below the basilar membrane (blue).
Somata of Spiral Ganglion nerve cells spirals to 1.75 turns (12mm = middle of the 2nd turn)
If there are no surviving dendrites, then insertion depths beyond 540 degrees have no benefit. Furthermore, the characteristic frequency of firing of these cells is less than 400Hz and there is limited pitch range at these depths in the cochlea. Consequently, the goal of cochlea implants is to get the maximum number of separate sites along the 540 degrees of the cochlea that overlies the spiral ganglion.
The Contour Electrode is designed to:
Be inserted up to 540 degrees
Place 22 electrodes in close proximity to the modiolus (“modiolus kissing”)
Place no pressure on the delicate cochlear structures
Beware: ABC measures insertion depth from the apex outward – this overstates the insertion depth by 40-50 degrees compared with our reference point.
540 degrees is how for and ideal insertion of a CA reaches into the cochlea. MedEl electrodes are designed to go up to 720 degrees of insertion and with a thicker apical tip than the CA. Such a deep insertion technique seeks to insert a wider electrode into an increasingly smaller area.
The cochlea is a delicate structure - a fine lattice of bone holding the soft tissues in place. During insertion of a straight electrode a pressure point on the lateral wall is created. This pressure may cause trauma.
This is a stunning photo showing the point in the bone where straight electrodes typically contact the lateral wall and have the potential to damage the spiral laminar, basilar membrane and stria vascularis.
To illustrate quantitatively the benefit of the AOS technique, we measured the force that the electrode exerts on the walls of the cochlea model during insertion for a standard insertion and an electrode inserted with the advance–off stylet technique.

Vertical axis is the measured insertion force.
Horizontal axis is the insertion depth as the electrode is inserted into the cochlea.
Four phases of insertion are depicted:
As the electrode is introduced into the cochlea there is essentially zero force.
When the tip contacts the lateral wall, the tip of the electrode begins to pressure the lateral wall and the insertion force increases as the tip applies some pressure to this contact point.
Then as the electrode bends to go around the cochlear spiral the insertion force decreases. The malleable platinum stylet moulds to follow the shape of the cochlea.
Finally as the electrode moves completely into the cochlea, the length of the electrode contacts the lateral wall and so the total friction between the electrode increases. The smooth silicone surface helps the electrode to slide smoothly around the lateral wall and distributes the force around the length of the cochlea. The force measured here is not pressuring the cohlea at a single point. The force on the lateral wall of the cochlea is distributed over the length of the cochlea providing a low pressure at any point.
The main aspect to note is the high pressure point in phase 2 when the tip first contacts the lateral wall of the cochlear and makes a relatively high pressure point on the cochlear structures.
This slide explains the design goals of the Contour Advance electrode:
Facilitate smooth movement through the scala
Minimise lateral wall forces
Aid more consistent perimodiolar positioning
Preserve delicate cochlear structures during insertion
The primary end-goals of course are consistent positioning and preserving structures.
These are designed to
Reduce stimulus thresholds
Improve stimulus localisation
Which can provide:
Potential for increased electrode numbers
Improved spatial definition
New generation of coding strategies
Maintenance of residual hearing
The four phases of insertion as the electrode moves into the cochlea are depicted again.
Notice that in each phase the tip of the electrode is not contacting the lateral wall.
The graph demonstrates this.
There is a dramatic reduction in insertion forces from the measurement with the standard insertion technique.
The insertion force reduces to near zero.
Not that the force in phase 2 shows that the force exerted on the modiolus is extremely low. This is the part of the graph where the force is slightly negative – of the order of 0.002 Newtons = equivalent to 0.0002 grams.
The Fraysse et al study found that of a subset of 12 of the 27 subjects where there were no surgical deviations from a “soft surgical” approach 9 (75%) retained hearing sufficient to fit a hearing aid.
Use of residual low frequency hearing may enable better speech in noise and pitch perception, and may improve subjective benefits such as sound quality, listening effort and greater enjoyment of music.
The Contour Advance electrode is designed to stimulate 1.5 turns of the cochlea or 420° - this is Cochlear’s definition for the ‘full cochlea’. The Hybrid electrode is shorter than the contour advance electrode as it only needs to stimulate the high frequencies which are in the outer section of the cochlea. This removes the need for a stylet to minimise insertion forces as with the contour advance. Not having to use a stylet also means the electrode can be made thinner which further reduces electrode insertion forces. So the Hyrbid L24 electrode mismises trauma during insertion by being shorter and thinner.
For both the Countour Advance with AOS insertion and Hybrid electrodes, the insertion force is minimised.
Roland T. – A Model for Cochlear Implant Electrode Insertion and Force Evaluation: results with a new electrode design and insertion technique
The thicker the electrode the more obstructive loss is experienced and the less effective the lower frequencies (being acoustically amplified) are at reaching further into the cochlea to the functioning hair cells.
A thinner electrode reduces the amount of obstructive loss experienced allowing lower frequencies to reach further into the cochlea and be more effectively perceived.
What is PDCI?? PDCI is hearing loss which is characterized by normal or slightly elevated threshold in low frequencies and by no hearing sensitivity in high frequency region. On this slide we can see that high frequency region corresponds to this part of cochlea so we can assume that this part of cochlea doesn’t function. (Is not functional). But the rest of the cochlea functions normally
To summarize our qualification criteria we prepared a graphical representation of this so area number 6 corresponds to qualification criteria for PDCI witch is different than the historical qualification criteria for EAS proposed by prof ilberg in 1999.
There was no inflammatory tissue response observed when stem cells were transplanted into the deafened mammalian cochlea for 4 weeks. H&E.
