2. PATIENT PROFILE
• ผู้ป่วยชายไทย อายุ 66 ปี
• อาชีพ ทานา
• ภูมิลาเนาจังหวัดนครราชสีมา

3. CHIEF COMPLAINT
อุบัติเหตุมีดฟันนิ้วโป้งขวา 2 ชั่วโมงก่อน

4. PRIMARY SURVEY
• A – Able to talk, not tender along C-spine, full ROM of C-spine
• B – Equal breath sounds both lungs, CCT negative
• C – vital signs BP 174/74, HR 66
• D – E4V5M6, pupil 3 mm RTLBE
• E – no life threatening wound

5. SECONDARY SURVEY
• A – no drug or food allergy
• M – metformin(500) 1*1 po pc
• P – T2DM
• L – 18:00 (4hr PTA)
• E – 2 ชั่วโมงก่อนมารพ. ขณะกาลังสับปลา โดนมีดที่ใช้สับปลาสับบริเวณนิ้วโป้งข้างขวา มีแผลเปิด เลือดไหล เห็นมี
ตาแหน่งเนื้อที่แหว่งออกไป ดังรูป

6. PHYSICAL EXAMINATION
• Vital signs – BP 147/74 mmHg, HR 66 bpm
• GA - A Thai man, alert, well cooperative, no pallor
• HEENT – no pale conjunctivae, anicteric sclerae
• CVS – normal S1S2, no murmur
• Lungs – clear, equal both lungs
• Extremities – avulsion wound size 1 x 1.5 cm. at distal phalanx of Rt.
Thumb with oozing blood and nail bed injury
• Neuro – intact distal neurovascular

8. DIAGNOSIS
• Open fracture distal phalanx of Rt. Thumb with nail bed injury

9. OPEN FRACTURE

10. GUSTILO ANDERSON CLASSIFICATION

11. TREATMENT
• Antibiotics
• Tetanus vaccination
• ER management
• OR management

12. ANTIBIOTICS
• Gustilo type I&II – 1st generation cephalosporin
• Gustilo type III – 1st generation cephalosporin and
aminoglycoside
• If anaerobic bacteria is suspected – add metronidazole or
penicillin
• ATB should be given as soon as possible (<3 hr)

13. ER MANAGEMENT
• Primary survey
• Antibiostics
• Bleeding control
• Wound dressing
• stabilization

14. OR MANAGEMENT
• Aggressive debridement and irrigation
• Fracture stabilization
• Avoid pinning

15. STABILIZATION
• Gustilo type I and II – internal fixation
• Gustilo type IIIa – controversial
• Gustilo type IIIb a d IIIc – external fixation

16. COMPLICATION
• Infection
• Neurovascular injury
• Compartment syndrome
