หลังเกิดเหตุขางอผิดรูป
• ไป รพช.หนองบุญมาก ABCD – ok
• Refer มา รพ.มหาราชนครราชสีมา

3. • A: can talk, patent airway, not tender midline
at neck
• B: equal breath sound, CCT negative
• C: no active external bleeding, no sign of poor
tissue perfusion, v/s stable
D: E4V5M6, pupil 3mm RTLBE
• E: No major external wound, significant
deformities at right thigh

7. • Dx: Close fracture right femoral
shaft

12. ตรวจร่างกายเบื้องต้น
• ATLS!!
• High energy mechanism
• Blood loss : close fx1000-1500ml
open fx may double
• Spine, pelvis, knee

13. Film
• AP, lat entire femur + hip + knee
• Rule out femoral neck fx

15. Treatment
• Operative treatment
• Traction : to reduce pain and deformities
before operation

16. Operation
• Intramedullary nailing
• Plate fixation
• External fixation

17. Intramedullary nailing
• Gold standard
• Most stable
• ↓ infection

18. Plate fixation

19. • Pavlik harness splint
• Spica casting
• Internal fixation
