1,5-Anhydroglucitol (1,5-AG)
- Giúp theo dõi thay đổi glucose máu ở BN ĐTĐ có HbA1C
bình thường hoặc gần bình thường.
-1,5-Anhydroglucitol là monosaccharide có trong hầu hết
các loại thức ăn.
- 1,5-Anhydroglucitol giảm khi glucose máu trên 180 mg/dL,
và trở lại bình thường sau khoảng 2 tuần nếu glucose máu
không tăng.
-Theo dõi 1,5-AG giúp đánh giá glucose máu ở ĐTĐ týp 1,
ĐTĐ týp 2 trên 180mg/dL cả trong các trường hợp HbA1C
khá tốt, hoặc blood glucose monitoring.
-FDA công nhận.

SINH LÝ
- Gần 100% 1,5-AG không chuyển hóa, tương đối
hằng định ở trong máu và các mô.
-1,5-AG vào máu được lọc ở cầu thận, rồi được tái
hấp thu ở ống lượn gần trở lại dòng máu.
-Một lượng nhỏ được thải trong nước tiểu, bằng lượng
được hấp thu nhằm duy trì trị hằng định trong máu và
các mô.
-Diễn tiến này xảy ra ở người bình thường khi glucose
máu không cao quá 180 mg/dL

-BN ĐTĐ khi glucose máu > 180 mg/dL, thận không thể hấp thu
hết glucose sẽ xuất hiện glucose niệu
-Glucose niệu sẽ chẹn sự tái hấp thu 1,5-AG, làm xuất hiện 1,5-
AG niệu ở mức cao hơn bình thường.
-1,5-AG máu giảm tức thì, và tiếp tục giảm cho đến khi glucose
máu xuống dưới 180 mg/dL.
-Khi sự tăng glucose máu được điều chỉnh, 1,5-AG lại được tái
hấp thu.
-Nếu glucose máu duy trì dưới 180 mg/dL trong thời gian 2- 4
tuần, 1,5-AG sẽ trở về trị số bình thường.

-Tính bằng µg/mL. Nồng độ thấp chứng tỏ kiểm soát
đường huyết kém, > 180 mg/dL nhiều đợt và kéo dài.
-Nồng độ 1,5-AG = 10 µg/mL tương đương glucose sau ăn
185 mg/dL, mức yêu cầu của BN ĐTĐ.
-Nếu > 10 µg/mL tương đương glucose trung bình <
180 mg/dL.
-BN ĐTĐ có trị < 10 µg/mL cần tiết thực, vận động và dùng
thuốc khống chế glucose sau ăn như: pramlintide,
exenatide, sitagliptin, saxagliptin, repaglinide hoặc rapid
acting insulins.

Fasting plasma glucose, the 2 hour plasma glucose after a 75-g oral glucose tolerance test, and A1C are equally appropriate diagnostic tests for diabetes.
These diagnostic criteria are:
Fasting plasma glucose (FPG) ≥126 mg/dL

2-hour plasma glucose ≥200 mg/dL during an OGTT

A1C ≥6.5%
Or in a patient with classic symptoms of hyperglycemia a random plasma glucose ≥ 200 can also be used.
In the absence of unequivocal hyperglycemia, the result should be confirmed by repeat testing.

Characterization of the underlying pathophysiology of diabetes is much more developed in type 1 diabetes than in type 2 diabetes. Three distinct stages of type 1 diabetes can be identified and serve as a framework for future research and regulatory decision making. The rate of progression is dependent on the age at first detection of antibody, number of antibodies, antibody specificity, and antibody titer. Glucose and A1C levels rise well before the clinical onset of diabetes, making diagnosis feasible well before the onset of DKA.

This is new this year– in order to clarify the relationship between age, BMI, and risk for type 2 diabetes and prediabetes, the Association revised the screening recommendations to now recommend testing all adults beginning at age 45 years, regardless of weight. [CLICK]
Testing is also recommended for asymptomatic adults of any age who are overweight or obese and who have one or more additional risk factors for diabetes. [CLICK] If tests are normal, the Association recommends repeat testing at least every 3 years.

There is strong and consistent evidence that obesity management can delay progression from prediabetes to type 2 diabetes (1,2), and benefits type 2 diabetes treatment.
Weight loss induced improvements in glycemia are most likely to occur early in the natural history of type 2 diabetes when obesity-associated insulin resistance has caused reversible β-cell dysfunction, but insulin secretory capacity remains relatively preserved
Many studies document other benefits of weight loss in patients with type 2 diabetes including improvements in mobility, physical and sexual functioning, and health-related quality-of-life.
And just a reminder that this entire section pertains to the treatment of type 2 diabetes specifically.

Glycemic goals should be individualized, based on a patient’s duration of diabetes, age, life expectancy, comorbid conditions, known cardiovascular disease (CVD) or advanced microvascular complications, unawareness of hypoglycemia, and other considerations
Many studies have shown that diabetes prevention is cost effective, so the Association maintains that such programs should be covered by third party payers.
Consider metformin in your patients with prediabetes, especially in those with BMIs over 35, who are younger than 60 years old, and women with a history of gestational diabetes.

Here’s a quick summary of recommended statin dosing for high and moderate intensity therapy. Note that these are all based on once-daily dosing.

Here is a summary of recommendations for statin treatment in people with diabetes. All of these recommendations are in addition to lifestyle therapy, as indicated by the asterisk by Recommended Statin Intensity.
For your patients less than 40 years old without ASCVD risk factors, no statins are recommended. If they do have risk factors-- which, as indicated by the double asterisk there, include LDL ≥100 mg/dL (2.6 mmol/L), high blood pressure, smoking, overweight or obesity, and family history of premature ASCVD–moderate or high dose statin therapy is recommended. For patients with overt ASCVD, a high dose is recommended.
For your patients aged 40-75 with no risk factors, moderate dose statin therapy is recommended in addition to lifestyle. For patients in this age group with ASCVD risk factors, a high dose is recommended, and for your patients with acute coronary syndrome and LDL over 50 who can’t tolerate high dose statin therapy, a moderate dose plus ezetimibe is recommended (along with lifestyle intervention).
And finally, for your patients over 75 years old with no risk factors, a moderate dose is recommended. With ASCVD risk factors, a moderate or high dose, and with overt ASCVD, a high dose along with that lifestyle therapy. And again for your patients in this age group with acute coronary syndrome and LDL over 50 who can’t tolerate high dose statin therapy, moderate dose plus ezetimibe is recommended.

Dapagliflozin clinical development program continued
In a 24-week, randomised, double-blind, placebo-controlled, parallel-group, multicentre, international trial, dapaglifozin was tested as add-on to glimepriride in type 2 diabetic patients achieving inadequate glycaemic control with sulphonylurea monotherapy.4 Adult patients with uncontrolled diabetes (HbA1c 7%-10%) receiving sulphonylurea monotherapy (n=597) were randomised to placebo or dapagliflozin (2.5, 5, or 10 mg/day) added to open-label glimepiride 4 mg/day for 24 weeks. Dapagliflozin significantly improved HbA1c, reduced body weight and was generally well tolerated.
The efficacy and safety of dapagliflozin as add on to insulin in patients with poorly controlled type 2 diabetes (mean baseline HbA1c: 8.5%) with insulin (mean baseline insulin dose: 77 IU/d) was evaluated in a 24-week randomised, placebo-controlled study.5 Patients (n=807) received once-daily placebo or dapagliflozin 2.5, 5, or 10 mg in addition to their unchanged background insulin therapy with or without concomitant oral antidiabetic drugs. Once-daily dapagliflozin therapy improved glycaemic control and body weight. No clinically relevant increase in hypoglycaemia was seen in the dapagliflozin group.
Dapaglifozin was also evaluated as add-on to pioglitazone in type 2 diabetic patients (n=420) inadequately controlled on pioglitazone alone (baseline A1c: ≥7-≤10.5%) in a 24-week randomised placebo-controlled study.6 Following run-in periods of 2 weeks (those on previous pioglitazone therapy, 48%) or 12 weeks (those starting pioglitazone, 52%), patients on pioglitazone ≥30 mg were randomised to DAPA 5 or 10 mg or placebo plus open-label pioglitazone. Compared with placebo, dapagliflozin significantly reduced HbA1c (a benefit maintained at week 48) and mitigated weight gain with low incidence of hypoglycaemia.

Nghiên cứu 014: nghiên cứu pha 3, ngẫu nhiên, mù đôi, so sánh song song, đối chứng với giả dược tại 80 trung tâm, được thực hiện trên 546 bệnh nhân ĐTĐ típ 2, tuổi từ 18-77, dùng Met với liều ≥ 1500mg, mức HbA1c ≥7.0 và ≤10.0%, C-peptide ≥0.34 nmol/L và BMI ≤45 kg/m2.
Nghiên cứu gồm 2 giai đoạn. Giai đoạn ngắn: kéo dần 24 tuần mù đôi đối chứng giả dược.Giai đoạn kéo dài:các bệnh nhân kết thúc giai đoạn ngắn, đủ điều kiện sẽ tiếp tục nghiên cứu kéo dài thêm 78 tuần mù đôi, đối chứng giả dược.
Sau giai đoạn dẫn nhập kéo dài 2 tuần sau đó các bệnh nhân được phân ngẫu nhiên thành 4 nhóm sủ dụng lần lượt Dapa 2,5mg, 5mg, 10mg và placebo.
Tiêu chí chính: Sự thay đổi HbA1c so với ban đầu tại tuần thứ 24.
Tiêu chí phụ: Sự thay đổi FPG ( đường huyết đói) tại tuần 24, tỉ lệ bệnh nhân đáp ứng với điều trị, HbA1c &lt; 7% tại tuần 24, Thay đổi tổng cân nặng cơ thể tại tuần 24.

Tại tuần 24, Dapagliflozin giúp giảm đường huyết đói có ý nghĩa so với giả dược, và duy trì ổn định trong suốt 102 tuần.
Hiệu quả giảm đường huyết đói được ghi nhận từ sớm, chỉ 1 tuần sau khi dùng dapagliflozin và tiếp tục giảm tới đỉnh ở tuần thứ 8 và duy trì đến tuần 24.
At week 24, DAPA 2.5 mg, 5 mg and 10 mg significantly reduced FPG from BL compared with PBO; decreases in FPG were notable by week 1 in the DAPA groups (significant for dapa 5 mg and 10 mg vs pbo)
Trong 546 bệnh nhân chia ngẫu nhiên thành 4 nhóm, thì số lượng bệnh nhân hoàn thành luôn nghiên cứu giai đoạn kéo dài 78 tuần ở nhóm placebo (63,5%) thấp hơn nhóm dùng dapagliflozin ( 68,3 – 79,8%). Nghuyên nhân chủ yếu là do trong nhóm placebo bệnh nhân rút khỏi nghiên cứu vì thuốc điều trị không hiệu quả.
Tại tuần 102 tất cả các nhóm dùng Dapagliflozin đều cho hiệu quả kiểm soát HbA1c tốt hơn nhóm placebo.
Nhóm placebo: HbA1c +0.02% so sánh với các nhóm dùng Dapa 2,5mg: - 0.5% (p=0.0008); Dapa 5mg: -0.6% (p&lt;0.0001) và Dapa 10mg: - 0.8% (p&lt;0.0001).
Hiệu quả kiểm soát HbA1c của nhóm Dapa+Met ổn định hơn suốt 4 năm khi so sánh với nhóm Gli+Met.
Đay cũng là dữ liệu lâm sàng của thuốc ức chế SGLT2 duy nhất có thời gian theo dõi tới 4 năm.

Early weight loss might be partly due to a mild osmotic diuresis caused by dapagliflozin. However, the gradual progressive reduction in body weight thereafter, seen in all studies,1-7 accompanied by a decrease in waist circumference as shown in the study evaluating dapagliflozin as an add-on to metformin,2 is consistent with a reduction of fat mass. This reduction is potentially attributable to the loss of excess energy through glucose excretion in the urine, an effect clearly evident at week 24, as shown by the increased urinary glucose/creatinine ratio in patients randomised to dapagliflozin.

The results presented include data obtained after the administration of rescue medication and were calculated according to the last observation carried forward (LOCF) principle.
Data are adjusted mean changes from baseline using ANCOVA, including data after rescue (LOCF).
Potential sequences of antihyperglycemic therapy for patients with type 2 diabetes are displayed, the usual transition being vertical, from top to bottom (although horizontal movement within therapy stages is also possible, depending on the circumstances). The figure denotes relative glucose lowering efficacy, risk of hypoglycemia, effect on body weight, other side effects and approximate relative cost for each drug class.
In most patients, begin with lifestyle changes; metformin monotherapy is added at, or soon after, diagnosis, unless there are contraindications.
