This document presents the program theory for evaluating a Swedish national project called "Learning Project - Care on Equal Conditions". The project aims to identify and develop methods to facilitate more equitable care in primary care units. The evaluation will assess the process, outcomes and health economics of the project from the perspectives of patients, caregivers and healthcare payers. It will examine activities, outputs, and short and long-term outcomes. The program theory provides a framework for the different types of evaluations and can be used as inspiration for the participating care units.

Title: Massachusetts Patient-Centered Medical Home Initiative (MA PCMHI): Impact on Clinical Quality at Midpoint

Research Objective:
To assess the impact on clinical quality of practices’ participation in a Patient-Centered Medical Home (PCMH) demonstration. The MA PCMHI is a statewide, three-year, multi-payer demonstration of PCMH implementation in 45 primary care practices. Practices receive technical assistance including learning collaborative, coaching provided by external facilitators, and feedback of aggregated data, to support their implementation of PCMH processes. This study aims to assess the overall impact of this approach to transformation on a practice’s delivery of selected clinical services, including preventive care, care coordination and care management, and its processes and outcomes of care related to the initiative’s targeted conditions of diabetes and asthma at the midpoint of the initiative.

Embracing GenAI - A Strategic ImperativePeter Windle
Artificial Intelligence (AI) technologies such as Generative AI, Image Generators and Large Language Models have had a dramatic impact on teaching, learning and assessment over the past 18 months. The most immediate threat AI posed was to Academic Integrity with Higher Education Institutes (HEIs) focusing their efforts on combating the use of GenAI in assessment. Guidelines were developed for staff and students, policies put in place too. Innovative educators have forged paths in the use of Generative AI for teaching, learning and assessments leading to pockets of transformation springing up across HEIs, often with little or no top-down guidance, support or direction.
This Gasta posits a strategic approach to integrating AI into HEIs to prepare staff, students and the curriculum for an evolving world and workplace. We will highlight the advantages of working with these technologies beyond the realm of teaching, learning and assessment by considering prompt engineering skills, industry impact, curriculum changes, and the need for staff upskilling. In contrast, not engaging strategically with Generative AI poses risks, including falling behind peers, missed opportunities and failing to ensure our graduates remain employable. The rapid evolution of AI technologies necessitates a proactive and strategic approach if we are to remain relevant.

Lärandeprojekt - Vård på lika villkor (Learning Project -
Care on equal conditions) which is a national project
from the Swedish Association of Local Authorities and
Regions (SALAR) in cooperation with the Swedish
Government.
●In this project SALAR will, together nine primary care
units and the research group from MDH, identify and
develop methods and procedures that facilitate a more
setting based equitable care.

Mälardalen University (MDH).

● health economic evaluation
● Focus on stakeholders and external factors.
