Humidification during Mechanical Ventilation
HMV 1.0 PROCEDURE:
The addition of heat and moisture to inspired gases delivered to the patient during mechanical ventilatory support via an artificial airway
HMV 2.0 DESCRIPTION/DEFINITION:
When the upper airway is bypassed, humidification during mechanical ventilation is necessary to prevent hypothermia, inspissation of airway secretions, destruction of airway epithelium, and atelectasis.(1-7) This may be accomplished using either a heated humidifier or a heat and moisture exchanger (HME). (HMEs are also known as hygroscopic condenser humidifiers, or artificial noses). The chosen device should provide a minimum of 30 mg H2O/L of delivered gas at 30°C.(8,29) Heated humidifiers operate actively to increase the heat and water vapor content of inspired gas.(11-14) HMEs operate passively by storing heat and moisture from the patient's exhaled gas and releasing it to the inhaled gas.(I5-25)
This is a checklist with the criteria the learners needed to demonstrate to verify competency in accessing, using, and de accessing central venous access devices with implanted ports.
The sterilization of surgical instruments is a process that removes all microorganisms from medical instruments before a surgery can take place. Proper sterilization ensures that all equipment has been thoroughly cleaned, sanitized and sterilized, and minimizes the risk of preventable surgical site infections. This process should be completed by a certified central sterilization technician.

카테터를 유지하는 경우
말초혈관과 중심정맥관을 통해 채혈하여 혈액배양을 의뢰한다.
카테터를 제거하는 경우
카테터가 삽입된 부위의 피부표면을 소독제로 잘 닦은 후 중심정맥관을 제거
하여 카테터의 끝 부위 5cm를 멸균된 가위로 잘라 멸균 튜브에 담아 배양검
사를 의뢰한다. 카테터를 제거한 후에는 멸균드레싱을 한다.
필요 없는 경우에는 빨리 제거한다.
단지 감염을 줄이기 위한 목적으로 중심정맥관을 정기적으로 교환하지 않는다.
삽입부위에 농이 나오면 단기간 사용하는 중심정맥카테터는 어떤 종류이든 바로 제거한다.
카테터 감염으로 카테터를 교체하는 경우, 반드시 다른 부위에 삽입하며 동일한 부위에 가이드
와이어를 이용하여 삽입하지 않는다.
무균술을 철저히 지켰다고 확신할 수 없을 경우(예, 응급 상황에서 카테터를 삽입한 경우)에는 가
능한 빨리 (48시간 이내) 교체한다.

Cross-transmission, the transfer of micro organisms between humans, which occurs
directly via an environmental source, occurs at all time in hospitals. It is the
antecedent factor to cross infection that can result in negative clinical outcomes.
Overviews of epidemiological evidence conclude that hand cross contamination is a
major contributing factor in the current infection episodes in hospital in – patients
(Pratt et al. 2001) Therefore all health care workers must be aware of their
responsibility in prevention of infection. An effective hand washing technique is
essential and is demonstrated below
One way to decrease the likelihood of central line infections is to apply maximal barrier precautions in preparation for line insertion. This is an integral part of the IHI Central Line Bundle and has been correlated with reduction in the rate of central line infection.
For the operator placing the central line and for those assisting in the procedure, maximal barrier precautions means strict compliance with handwashing, wearing a cap, mask, sterile gown and gloves. The cap should cover all hair and the mask should cover the nose and mouth tightly. These precautions are the same as for any other surgical procedure that carries a risk of infection.
For the patient, maximal barrier precautions means covering the patient from head to toe with a sterile drape with a small opening for the site of insertion.

The cause of most catheter-related blood stream infections are those micro organisms
that colonise catheter hubs and the skin surrounding the catheter insertion site.
(Mermal, 2000). Skin antisepsis is regarded as one of the most important measures for
preventing catheter-related infection and appropriate preparation of the insertion site
will reduce this risk.
Maki et al. (1991) conducted a trial to compare the effectiveness of 2% chlorhexidine
compared to either 10% povidone iodine or 70% alcohol. They found that 2%
chlorhexidine was superior and an additional study by Mimoz et al (1996) has since
confirmed the superior efficacy of chlorhexidine 2%.

Daily review of central line necessity will prevent unnecessary delays in removing lines that are no longer clearly necessary in the care of the patient. Many times, central lines remain in place simply because of their reliable access and because personnel have not considered removing the line. However, it is clear that the risk of infection increases over time as the line remains in place and that the risk of infection is decreased if removed.
