2. SCRUB TYPHUS
R.australicaused bya mite called O. tsutsugamushi.
Family – rickettisiae
This family contains 2 genera ticks ( orentia) and mites (rickettisiae)
Under orentia the following species are present. R.typhus- endemic typhus
R.rickettisiae-rocky mountain spotted fever.
R.conori-boutnneuse fever.
s Australian tick fever.
R.sibera –Siberian tick typhus
RICKETTISIAE CONTAINS - O.TSUTSUGAMUSHI(scrub typhus)

3. INTRODUCTION
Caused by a mite called O. Tsutsugamushi.
The word tsutsuga means dangerous “mushi means bite.
Disease transferred fro larval form of mite.incubation period 7-10
days.
Case fatalityis 10-60%
In laraval forms these mites are known as chiggers
They live in places where there is a lot of moisture and scrub

4. MODE OF TRANSMISSION
normall y the larva feeds on small mammals
or ground feeding birds
engorged larva
nymph (both nymph and adult free
living in soil)
adults( where these adults lay eggs
and enters into larval stage infects humans via mites

5. SYMPTOMS
Symotoms usually appears after 10 of mite bite
Fever and chills
Head ache
Body ache and muscle pain
Eschar –a dark scrab like region at the site of chigger bite
Confusion
Enlarged lymph nodes
Bleeding and organ failure -severe

6. COMPLICATIONS
5-8 days after onset of fever mucopapulary rash appears on the trunk ad
later extended to arms and legs
Complications of scrub typhus
Pneumonia
Acute respiratory distress
Myocarditis
Encephalitis
Hepatitis
AKD
Acute pancreatitis and transient adrenal insufficiency

7. DIAGNOSIS
Weil-felix ox-k Aggulitination reaction
The weil felix isan agglutination test for diagnosis of rickettiseal
infections.
The weil felix antibody was recently found to target rickettisea LPS O –
ANTIGEN
Main purpose of this test is diagnosis of rickettisealinfections.
The weil felix can be done as either slide or tube test
Antigens –OX2,OX19,OX-K
Indirect flurascence antibody test

8. TREATMENT OF SCRUB TYPHUS
Drug of choice – doxycycline 200mg -BD (1-0-1) for 7 days
Azithromycin 500mg – OD - (0-1-0 ) for 7 days
Symptoms based treatment can be according to patient condition
