Рак молочной железы по частоте занимает одно из первых мест по заболеваемости среди женского населения, а смертность от рака молочной железы достигает почти 15% от всех смертей, возникающих вследствие опухолевого процесса.
При лечении диссеминированного процесса необходим баланс между оптимизацией ответа на лечение, увеличением продолжительности жизни и максимально возможно высоким качеством жизни. Метастатическое поражение скелета встречается у 65-75% пациентов с диссеминированным раком молочной железы.
Скелетные метастазы, как правило, приводят к развитию так тяжелых осложнений как патологические переломы, компрессия спинного мозга и гиперкальциемия, кроме того, они часто служат основной причиной развития выраженного болевого синдрома . Таким образом, лечение скелетных метастазов является важным компонентом лечения диссеминированного рака молочной железы.

This document describes a case of a 72-year-old man who presented with breathlessness, cough, and recently developed easy fatigability and drooping of the left eye. Imaging showed a mediastinal mass. He was diagnosed with thymoma and myasthenia gravis. He underwent surgery to remove the mass. Histopathological examination found it to be a thymic marginal zone lymphoma. Immunohistochemistry supported this diagnosis. The surgery achieved a complete excision and the patient's myasthenia gravis symptoms improved with medication. He has been followed up for 3 months since with no recurrence.
This study aimed to determine if the concentration of 20S proteasome could serve as a novel marker for monitoring treatment in patients with multiple myeloma. The study measured proteasome concentrations in 32 multiple myeloma patients before and after 6 cycles of CTD therapy and in 15 healthy controls. Proteasome concentrations were higher in patients before treatment compared to controls. Responding patients had lower proteasome levels after treatment than non-responders. The study concluded that proteasome concentration increases in multiple myeloma patients and correlates with treatment effectiveness, suggesting it may serve as a marker for monitoring multiple myeloma treatment.
Monoclonal gammopathies can cause a variety of renal lesions beyond just cast nephropathy and amyloidosis. A study of renal biopsies from a tertiary care center in India over 3 years found 123 cases of monoclonal gammopathy, representing 2.33% of all renal biopsies. The most common lesions were cast nephropathy, amyloidosis, and monoclonal immunoglobulin/light chain deposition disease. Using immunofluorescence to detect kappa and lambda light chains helped identify monoclonal gammopathies presenting atypically or in unusual morphological patterns. Early diagnosis of monoclonal gammopathy of renal significance is important to initiate therapy and prevent future complications.
1) A patient with double-hit high grade B-cell lymphoma was successfully treated with R-DA-EPOCH chemotherapy combined with metformin.
2) The treatment resulted in a PET-negative residual mass that was further reduced in size with maintenance therapy of metformin and lenalidomide.
3) This case demonstrates the potential for R-DA-EPOCH combined with metformin to achieve complete clinical remission in double-hit lymphoma, though larger studies are still needed.
This study evaluated the quality of life, efficacy, and safety of dasatinib treatment in 37 patients with chronic myeloid leukemia in chronic phase (CP CML) in a real-world setting. The results showed a high rate of hematological and cytogenetic responses. Adverse events were mostly mild. Quality of life was initially lower in CML patients versus controls but stabilized or improved over 12 months of dasatinib treatment. The front-line use of dasatinib in this real-world study demonstrated benefits for patients' outcomes and quality of life.

1) The role of allo-HSCT in treating patients with the T315I mutation causing resistance to tyrosine kinase inhibitors (TKIs) is still debated.
2) A retrospective analysis of 53 CML patients with the T315I mutation found 5-year overall survival was 42% but was significantly worse for those in blast crisis.

Проведено комплексное обследование и лечение 40 больных генерализованным пародонтитом I степени тяжести в возрасте 19 - 23 лет. Всем больным были проведены клинические, рентгенологические и лабораторные исследования. Постановку диагноза осуществляли на основании данных анамнеза заболевания, жалоб больного, осмотра полости рта, определения индекса кровоточивости по Мюллеману, пародонтального индекса Рассела, индекса РМА, гигиенического индекса Грина-Вермильона и рентгенологических данных. Выявляли наличие соматической патологии, основных этиологических факторов. Иммунологические показатели изучали до и после лечения. В качестве тестов были использованы: определение популяций и субпопуляций лимфоцитов с помощью моноклональных антител, иммуноглобулины методом радиальной иммунодиффузии в геле по Mancini [6], ЦИК фотометрически с ПЭГ 6000, фагоцитарная активность нейтрофилов в отношении латекса.

This study aimed to determine the epidemiological characteristics and outcomes of lymphomas in HIV-positive patients in Russia. The study analyzed data from 73 patients treated between 2006-2016 at several medical centers. It found that diffuse large B-cell lymphoma was most common, comprising 34% of cases. Brentuximab vedotin is an antibody-drug conjugate used to treat Hodgkin lymphoma. It consists of an antibody linked to a cytotoxic agent. The antibody targets CD30 found on Hodgkin lymphoma cells, and the cytotoxic agent is released inside those cells to kill them. Brentuximab vedotin is used as first-line treatment, for relapsed disease, and after stem cell transplantation. 2. Цель - изучение универсальной системы маркеров метилирования
острого миелоидного лейкоза (ОМЛ) у детей.
Задачи:
формирование системы маркеров метилирования при ОМЛ у детей;
изучение изменения профиля аберрантного метилирования в
динамике ОМЛ, а также при различных терапевтических схемах ОМЛ;
анализ выявленных маркеров метилирования на предмет наличия
клинико-генетических корреляций с целью изучения прогностического
потенциала аберрантного метилирования;
оценка перспективы определения минимальной остаточной болезни с
использованием системы выявленных маркеров.
Детекция метилирования методом МЧ-ПЦР
Определение частоты аномального метилирования маркеров,
соответствующих промоторным областям генов SOX8, CXCL14, GSG1L,
TMEM176A/TMEM176B, CLDN7, EGFLAM, проводили методом
мультиплексной МЧ-ПЦР с использованием внутренних контролей.
Для определения метилирования был использован фермент
BstHHI, имеющий многочисленные сайты рестрикции в пределах
изучаемых локусов, для определения гомогенности метилирования CpG-
пар региона.

4. Выборка
В исследовании использованы 86 образцов биологического материала
костного мозга 39 детей с ОМЛ (НИИ ДОиГ им. Н.Н.Блохина) и 6
образцов донорского КМ (ФНКЦ ДГОИ им. Дмитрия Рогачева)

В клетку он попадает с помощью переносчика
нуклеозидов hENT1. Далее с помощью
дезоксицитидинкиназы фосфорилируется до
монофосфата, а следующая киназа превращает его в
нуклеотиддифосфат. После трех фосфорилирований он
включается в ДНК.

В ходе репликации ДНК-метилаза копирует метильную группу (фиолетовый
кружок) на новосинтезированную цепь ДНК. Фермент садится на встроенный
в цепь ДНК вместо цитозина динуклеотид азацитидин-гуанин (Z) (так как не
чувствителен к замене основания) и ковалентно с ним сшивается. После чего
подвергается деградации. Таким образом, в ДНК будет метилированной только
одна из цепочек вместо двух, а значит, новая клетка будет метилирована только
наполовину. Дальнейшая репликация приведет к появлению клеток, полностью
лишенных метилирования.

10. Выводы
Предложенная система из 6 маркеров аномального метилирования является
универсальной системой маркеров ОМЛ у детей.
Показано, что использование деметилирующей терапии перед курсом химиотерапии
снижает уровень метилирования ДНК, однако это снижение статистически не значимо.
После блока AIE происходит увеличение частоты метилирования у 4х изучаемых генов
(статистически значимо для гена CLDN7), а снижение - у 2х (статистически значимо для гена
SOX8), что свидетельствует о клональной эволюции опухоли.
Была определены статистически значимые ассоциации между статусом метилирования
гена TMEM176A/176B и вовлеченностью центральной нервной системы в патологический
процесс у больных ОМЛ до лечения (р = 0,0153), а также наличием у пациента
цитогенетической аномалии (p=0,037).
C помощью разработанной системы возможна оценка злокачественной прогрессии
бластных клеток.
