While the aetiology of the condition is not completely understood, it is believed that a number of factors (rather than one in isolation) are most likely to lead to its development. Background. Reduced functional capacity and postexertion fatigue after physical
activity are hallmark symptoms of chronic fatigue syndrome (CFS) and may even
qualify for biomarker status. That these symptoms are often delayed may explain the
equivocal results for clinical cardiopulmonary exercise testing in people with CFS.
Test reproducibility in people who are healthy is well documented. Test reproduc-
ibility may not be achievable in people with CFS because of delayed symptoms.
Objective. The objective of this study was to determine the discriminative validity
of objective measurements obtained during cardiopulmonary exercise testing to
distinguish participants with CFS from participants who did not have a disability but
were sedentary.
Design. A prospective cohort study was conducted.
Methods. Gas exchange data, workloads, and related physiological parameters
were compared in 51 participants with CFS and 10 control participants, all women,
for 2 maximal exercise tests separated by 24 hours.
Results. Multivariate analysis showed no significant differences between control
participants and participants with CFS for test 1. However, for test 2, participants
with CFS achieved significantly lower values for oxygen consumption and workload
at peak exercise and at the ventilatory or anaerobic threshold. Follow-up classification
analysis differentiated between groups with an overall accuracy of 95.1%.
Limitations. Only individuals with CFS who were able to undergo exercise
testing were included in this study. Individuals who were unable to meet the criteria
for maximal effort during both tests, were unable to complete the 2-day protocol, or
displayed overt cardiovascular abnormalities were excluded from the analysis.
Conclusions. The lack of any significant differences between groups for the first
exercise test would appear to support a deconditioning hypothesis for CFS symp-
toms. However, the results from the second test indicated the presence of CFS-related
postexertion fatigue. It might be concluded that a single exercise test is insufficient
to reliably demonstrate functional impairment in people with CFS. A second test
might be necessary to document the atypical recovery response and protracted
fatigue possibly unique to CFS, which can severely limit productivity in the home and
workplace.

Gene Therapy For Cystic Fibrosis
Gene therapy aims to treat cystic fibrosis by delivering functional copies of the defective CFTR gene to the airway epithelium using viral or non-viral vectors. Several clinical trials have been conducted using adenovirus, adeno-associated virus, and non-viral vectors to deliver the CFTR gene with varying levels of success. While progress has been made, gene therapy has yet to provide a cure for cystic fibrosis and new vector and trial designs are still needed to achieve sufficient gene expression and correction.
Review Paper-Gene Therapy for the Treatment of Cystic Fibrosis
Cystic fibrosis is a genetic disease caused by mutations in the CFTR gene. Gene therapy aims to insert a normal copy of this gene to restore function. While conceptually simple, gene therapy for cystic fibrosis has proven difficult in practice due to biological barriers and challenges with delivery vectors. Early clinical trials used adenoviral vectors administered to the nose with limited success. Subsequent trials delivered vectors directly to the lungs but were also unsuccessful. Recent trials have focused on non-viral vectors which allow for repeated administration and show promise, though further development is still needed to achieve an effective treatment through gene therapy.

- 2. Problem Do changes in the functionality of the HPA axis cause the onset of chronic fatigue syndrome?
- 3. Objectives Asses the functionality of the HPA axis on patients diagnosed with Chronic Fatigue Syndrome. Determine if the activity of the HPA axis on patients with CFS is different than that of a control group.
- 4. Background Information Chronic fatigue syndrome, or CFS, is a debilitating and complex disorder characterized by profound fatigue that is not improved by bed rest and that may be worsened by physical or mental activity. Despite an intensive, nearly 20-year search, the cause of CFS remains unknown.
- 10. Nutritional DeficiencyThere is no known cure for CFS. Treatment is aimed at symptom relief and improved function.
- 11. Hypothesis I believe that alterations in HPA axis activity are responsible for the development of Chronic Fatigue Syndrome.
- 12. Methodology Obtain permission from the IRB to investigate with human participants.
