"Looking Ahead" Post-Ebola Strategy in West Africa is the first in a series of planned webinars, where we invite knowledgeable individuals and participants to join the post-Ebola strategy in West Africa discussion.
During the webinars, experts from different backgrounds, will outline their view on the Ebola Crisis and most importantly, share their vision on what needs to be done now, and post-Ebola, to ensure aversion of further political and economic disturbances.
The fast spread of the Ebola virus has major consequences on the African countries it has hit the hardest: Guinea, Liberia, and Sierra Leone.
Besides the death tolls and associate losses, the countries are also facing great danger because of the economic consequences the virus carries.
Sierra Leone and Liberia, two of the most hit countries, have both recently come out of more than a decade of gruesome civil wars and the set back of the disease does not help with the stabilization of the economies. Their democracies are fragile and the deprivation from the Ebola crisis could be a trigger for political disruption.
The youth played a major role in those conflicts as a result of economic and social marginalization. Without a post-Ebola strategy to ensure the youth a future of economic and social stability, there may be unforeseeable instabilities.

The Indonesia HiT reports the significant improvement in the health status of the population over the last 25 years through transitional period in all fields. However, the country faces remaining and foreseeing challenges in communicable diseases and emerging NCDs. The HiT concludes with the future challenges of expanding coverage of National health insurance scheme (JKN), reducing regional disparities in health-care services, managing resources and engaging private sector.
These slides give an overview of public health and the role of local public health departments in keeping people healthy, presents housing, health and some of the vulnerable populations who are the primary focus of our work, and shows the Healthy Chicago Public Health Agenda - the blueprint for our work at the Chicago Department of Public Health. Lastly, it highlights some of our work and accomplishments with vulnerable groups.
This review outlines the main organizational, financing, human resources and service delivery features of the health-care system. Although there has been implement in overall health outcomes since the 1990’s the current levels are still below average for the country’s Pacific neighbors. The remoteness of the many rural communities has hampered improvements in health services. This is one of the major challenges that the country faces in order to achieve SDG heath targets by 2030. This Hits highlights steps taken to overcome challenges especially in the face of epidemiological change in disease burden that is slowly taking place in the country.
Thailand was the first country outside of China that reported COVID-19 infection in January 2020. At the peak of transmission during March-April 2020, it was reporting close to 200 new cases per day and yet it has been able to control the outbreak with no laboratory confirmed local transmission reported for over 100 days as of 2 September 2020.
This publication attempts to identify in a systematic way, various policies and steps that were put in place from the beginning of the outbreak to control COVID-19 transmission in the country.
The November 2020 update builds on the previous document by focusing on the challenges of balancing opening up the country and protecting the population from COVID-19 as well as preparing for the potential second wave.
As the burden of NCDs increases, various countries have introduced new and innovative modes of managing them in primary healthcare setting. APO, in conjunction with Duke Kunshan University, China, conducted a 4-country study (Bangladesh, China, Nepal and Viet Nam) to understand the different approaches used in involving CHWs in preventing and managing NCDs. The Thailand HiT reports that sustained political commitment to the health of the population since the 1970s has resulted in significant investment in health infrastructure, in particular primary health care, district and provincial referral hospitals, and strengthened the overall functioning of the Thai health system. After Thailand achieved universal health coverage in 2002, public expenditure on health significantly increased from 63% to 77% and out-of-pocket expense was reduced from 27.2% to 12.4% of the total health spending in 2011.

“Rachel Glennerster is Executive Director of the Abdul Latif Jameel Poverty Action Lab (J-PAL). HerRachel - Credit Blu Nordgren research includes randomized evaluations of community-driven development, the adoption of new agricultural technologies, and improving the accountability of politicians in Sierra Leone; empowerment of adolescent girls in Bangladesh; and health, governance, education, and microfinance programs in India. She serves as Scientific Director for J-PAL Africa, Co-Chair of J-PAL’s Agriculture Program and is a board member of the Agricultural Technology Adoption Initiative (ATAI). She is the lead academic for Sierra Leone for the International Growth Center. Between 2007 and 2010 she served on the UK Department for International Development’s (DFID) Independent Advisory Committee on Development Impact.
Rachel Glennerster helped establish Deworm the World, of which she is a board member, which has helped deworm 23 million children worldwide. Before joining J-PAL, she worked at the IMF and Her Majesty’s Treasury. She has a Ph.D. in economics from Birkbeck College, University of London. The spread of the Ebola virus disease has major consequences on the African countries it has hit the hardest: Guinea, Liberia, and Sierra Leone. The death toll from the disease and associated losses pose great economic consequences.
More than 2,600 people have died in West Africa. Transportation companies suspended their operations to the countries for fear of contamination even though the World Health Organisation advised against it. So far, the economies are experiencing adverse effects with escalating food insecurities.
Sierra Leone and Liberia, two of the most hit countries, recently came out of more than a decade of gruesome civil wars. Their institutions are still fragile and the deprivation from the Ebola crisis could trigger ever bigger problems.
The youth played a major role in both the Sierra Leone and Liberia conflicts as a result of economic and social marginalization. Without an effective strategy to assure young people a brighter future of economic and social stability, impact of the 2014 Ebola crisis on food insecurity and social livelihoods could trigger an even greater post-Ebola crises.

38. Definition of terms
• Community Volunteers: a group of individuals who has
undergone basic or no formal education at all but provides
significant community health intervention. They are
selected, trained and working for their own communities
but not necessarily part of the formal health system, they
include vaccinators, cold chain assistants, community
mobilizers and independent monitors.
• Hard to reach communities: communities who are not
easily accessible to health services due to one or more
adverse conditions: distance from the nearest health
facility, difficult terrain (mountainous areas, forested
areas), flooding, insecurity, poor or unreliable road
networks, nomadic lifestyle.

• Weak health system: a health system which is
unable to meet the basic health needs of its people.
Weakness in light of insufficient qualified health workers,
diseases surveillance, public awareness, chronic shortage
of supplies e.g. vaccines, inequality in distribution of health
workers and health facilities, limited or no community
outreach programs and engagement of the local
population.

South Sudan
• South Sudan become independent in July 2011, 10 States
and 79 counties
• Population project (2012) puts the figure for South Sudan
at 11,532,241
• Only one third of the country has access to basic health
services
• It has 1,090 functional health facilities with 37 of them
being hospitals.

• It has the worst shortage of health workers put at 1.5
doctors and 2 nurses per every 100,000 population.
• Extreme inequality in distribution of health facilities and
health workers with urban areas most favored
• Its maternal mortality ratio stands at 2,054/100,000 and
under five mortality rate was estimated at 135/1,000 live
births. (Sudan HH Survey 2006/2010)
• 2011 EPI survey shows 16.5% of children under the age of
one fully vaccinated and only 22.1% completed OPV3.
42. Background to Core Group South Sudan
• Started operations in 2011, within eight worst performing
counties in immunization mostly based at the Southern
borders of South Sudan.
• Core Group South Sudan has three partners: American
Refugee Committee, World Vision South Sudan and Amref
Health South Sudan.
• It supports comprehensive EPI services that focuses on;
1. Routine immunization both at facility and community level
2. Support Polio National Supplementary Immunization Days
3. Community-based disease surveillance

• Inadequate funding to strengthen the already improving
services and scaling to other poorly performing counties
• Due to the level of poverty in the community, the
expectation of pay among project volunteers
• Poor transport infrastructure- poor roads, flooding,
inadequate transport means.
• Insecurity in the remote areas- arrest of volunteers in
doing their work in some insecure areas.
58. Lessons learnt
• Integration of the community volunteers’ activities into the
formal health system is critical for the success of the
program.
• Collective planning with the community leaders, facility
based health workers and volunteers results to ownership
of the program
• Community volunteers play a very pivotal role in
strengthening the health system through their engagement
and participation.

• Working with community volunteers who hail from that
community leads to a lot of trust resulting in acceptance of
the program and improved uptake of vaccine which
culminated in improvement in immunization coverage in
most of the counties.
• Strong collaboration between community leaders, the local
health authorities and presence of supporting partner on
the ground
60. conclusion
• Due to the weak health system in South Sudan resulting in
worsening health indicators, there is no doubt that
involving community volunteers and linking them to the
formal health system is important if we are to reach the
unreached with lifesaving health interventions.
