A 5-year-old Thai boy presented with left elbow pain for 2 hours after falling and having his friend fall on his outstretched left arm. On examination, his left elbow was tender on the lateral side with limited range of motion. X-rays showed no fractures. The diagnosis was nursemaid's elbow (radial head subluxation), which occurs when longitudinal traction is applied to an extended arm in young children. The radial head was reduced by gentle manipulation and the boy could immediately use his arm normally again with low risk of recurrence.

Physical Exam
pain and tenderness localized to the lateral
aspect of the elbow.
22. Post treatment
Recurrence occurs in 5% to 39% of cases, but
generally ceases after 5 years of age.
immobilization is not necessary and the child
may immediately resume use of the arm.
follow up is only needed if the child does not
resume normal use of his arm in the following
weeks.
