Peripheral Arterial Disease (PAD) is the progressive obstruction of arteries below the aortic bifurcation due to atherosclerosis. It has a prevalence of 5-20% in those over 50 years old. Symptoms range from intermittent claudication to critical limb ischemia with rest pain and tissue loss. Late presentation is common due to asymptomatic or atypical symptoms. Diabetes significantly increases the risk and severity of PAD. Treatment involves risk factor modification, endovascular or surgical revascularization, wound care, and in severe cases amputation. Regular screening and multidisciplinary care can help prevent amputation in those with PAD and foot ulcers.

Physiological changes during pregnancy can be extensive. The uterus grows dramatically in size and the cervix softens. The breasts enlarge and darken. Throughout pregnancy, the body retains more fluid and blood volume increases. Respiration increases to support higher oxygen needs. The heart works harder pumping more blood. The kidneys and liver increase in size. Many hormonal changes prepare the body for childbirth and nurturing a baby.
ESC guidelines on Cardiovascular diseases during pregnancy
Pregnancy poses risks for women with cardiac conditions due to physiological changes that increase cardiac output and blood pressure. Careful pre-pregnancy evaluation and individualized management plans are needed to minimize risks which can include arrhythmias, heart failure, and aortic dissection. Medical therapy aims to control symptoms while minimizing risks to both mother and fetus, and mode of delivery depends on maternal cardiac status and obstetric factors.

Oxygen therapy involves administering oxygen at concentrations greater than room air to treat hypoxemia. The purpose is to increase oxygen saturation in tissues where it is too low due to illness or injury. Oxygen can be delivered via various low or high flow devices like nasal cannulas, masks, tents or venturi masks to maintain adequate oxygen saturation. Close monitoring of oxygen saturation levels via pulse oximetry or arterial blood gases is needed to properly titrate oxygen therapy.
Coronary circulation
The coronary circulation consists of the arterial supply, venous drainage and lymphatic drainage of the heart. The heart receives its blood supply from the right and left coronary arteries. The right coronary artery supplies the right atrium and ventricle while the left coronary artery supplies the left atrium and ventricle. Coronary blood flow is regulated by physical, chemical, neural and hormonal factors to meet the metabolic demands of the heart. Coronary artery disease can lead to conditions like angina pectoris and myocardial infarction due to reduced blood supply to the heart muscle.
