Piezosurgery has been applied in dentistry for many years. This paper reviews specifically the treatment applications that have been used in surgically assisted orthodontic treatment since the last decade. Periodontally Accelerated Osteogenic Orthodontics (PAOO) is a surgical technique which results in an increase in alveolar bone width, shorter treatment time, increase post-treatment stability, and decrease amount of apical root resorption. The aim of this case report is to compare the use of micro-motor and piezoelectric surgery unit during decortication in Periodontally Accelerated Osteogenic Orthodontics technique.

Abstract: Severe atrophy of the inferior alveolar process and underlying basal bone often results in problems with a lower denture. These problems include insufficient retention of the lower denture, intolerance to loading by the mucosa, pain, difficulties with eating and speech, loss of soft-tissue support, and altered facial appearance. These problems are a challenge for the prosthodontist and surgeon. In this case report, patient with resorbed edentulous mandible was successfully rehabilitated using two dental implants placed in the interforaminal region with ball abutments opposing conventional maxillary complete denture. Partial or complete edentulism has multiple implications in relation to function, esthetics and future rehabilitative treatment. This case report illustrates the management of a patient with extreme consequences of partial edentulism in the maxillary arch and total edentulism in the mandibular arch. The main clinical findings were unopposed remaining teeth, over eruption of the remaining teeth, loss of vertical dimension of occlusion, and significant disfigurement of the occlusal plane. Following the diagnostic procedure, a well-coordinated prosthodontic treatment involving liaison with other dental disciplines was indicated. The management involved an innovative combination of fixed and removable prostheses in conjunction with intentional root canal therapy of the remaining natural teeth. Series of provisional prostheses were applied to facilitate the transition to the final treatment.

Immediate dental implants have greatly reduced the treatment time and the number of surgical intervene tions. Recently it has been noted that this treatment modality can be used in aesthetically demanding cases especially the anterior maxilla. The aim of this article is to describe a clinical case in which a fractured maxillary canine was replaced by an osseointegrated implant using a simplified technique in a patient who was a smoker and presented poor oral hygiene. The technique adopted permits a reduction of the number of implant components and consequently a lower cost of treatment, while at the same time maintaining acceptable aesthetic and functional outcomes.

Objectives: 1.To clinically evaluate the healing process following periapical surgery with Chitra granules 2.To radiographically evaluate healing, following periapical surgery with Chitra granules. 3.To compare the bone healing in the study group, clinically and radiographically following the modalities of treatment, with that of the control group (without graft). Materials: 1.Chitra hydroxyapatite granules. 2. Gutta-percha — for root canal obturation by lateral condensation. 3. High Copper amalgam used as retrofilling materials. To evaluate healing after periapical surgery using Chitra granules, 22 patients were selected from out patient section of Department of Conservative Dentistry and Endodontics, Govt. Dental College, Kozhikode,based on clinical and radiographical evaluation. The study group consisted of 13 males and 9 females. All were of the age group 15-35 years. After selection they were randomly divided into two groups A and B. In group A the Chitra granule were placed in the bony defect and in group B the defect were left as such after surgery. Results: Clinical parameters showed better early symptom free condition in group A compared to group B. But data found statistically insignificant (t-7.27,df-1.8, P >0.05). Radiographical evaluation data analysis showed statistically significant difference among group A and group B. Conclusion: Biocompatible Chitra granule not only obliterates the cavity but act as a scaffold for bone growth and prevent scar tissue formation.It is osteoconductive. In comparison to the conventional periapical surgery, the placement of Chitra granules facilitates bone regeneration more easily. The material is found to be very cost effective, easily available, easy to manipulate and involves least complication to both clinicians and patients.

Abstract: Intentional reimplantation is a procedure in which tooth extraction is performed followed by reinsertion of the extracted tooth into its own socket after performing the desired procedure. In this article, intentional reimplantation is described and discussed as a treatment approach for aperiapical lesion that is in maxillary second molar. After 15 years, the patient was asymptomatic, the tooth was still functional and a recall intraoral periapical radiograph showed an intact periodontal ligament space and lamina dura with no evidence of gross root resorption or ankylosis.

Corticotomy facilitated orthodontics
Although the art and science of orthodontics have progressed significantly over the past 100 years, relatively little has been done to enhance the rate at which tooth movement occur. Many methods have been done to enhance the rate of tooth movement. These methods include the injection of biologically active peptides, the use of magnets and even the application of electric current and corticotomy.
Corticotomy: is slight penetration through the cortical bone and did not be confused with the osteotomy. Or defined as incision made into the cortical bone.
This penetration or incision leads to decrease the resistance of the alveolar and diminish physical alveolar bone contact that accelerates the rate of tooth movement.
Several authors have described rapid tooth in conjunction with corticotomy surgery as movement by bony (Block). Kole6 was the first describe the corticotomy as a surgical procedure in which one tooth or group of teeth with the adjacent bone is repositioned in one step. But others prefer to call this osteo-corticotomy or intra alveolar segmental osteotomy, reserving the term corticotomy for a technique in which cuts are made in the buccal cortical plate of bone. So that the segment to be moved orthodontically is held only by cancellous trabeculea and palatal cortical bone.
Kole in 19596 reported combining orthodontics with corticotomy surgery and complete the active tooth movement in adult orthodontic cases in 6 to 12 weeks.
The inter-proximal corticotomy cuts were extended through the entire thickness of the cortical layer, just barely penetrating the medullary bone.
The vertical cuts were connected beyond the apices of the teeth with horizontal osteotomy cut extending through the entire thickness of the alveolus, essentially creating blocks of bone in which one or more teeth were embedded, using the crowns of the teeth as a handles. Kole believed that he was able to move the blocks of bone some what independly of each other because they were only connected by less-dense medullary bone. He found no incidence of root resorption, no loss of tooth vitality and no pocket formation.
Kole used this surgical technique for correction of some of dento-alveolar problems as:
Protruding of lower incisors: this procedure is indicated in most of cases but should be determined whether a mandibular or dento-alveolar retrusion. Buccally the cortiocotomy is performed between the incisors and canine then horizontal cut is made 1cm. below the incisors, lingual two vertical and one horizontal cut is made fig(1).
Distal displacement of a single tooth or group of teeth: correction necessitates a long period of treatment in adult patients fig(2) .
The retrusion of all six lower anterior teeth: after buccal and ligual corticotomy is perefrmed.

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

Abstract: Corticotomy-assisted orthodontic treatment involves selective alveolar decortication in the form of
decortication lines and grooves which is performed around the teeth that are desired to move orthodontically.
This procedure is done to induce a state of increased bone turnover and, which will enhance in a faster rate of
orthodontic tooth movement. This technique has several advantages, including faster tooth movement, shorter
treatment time span, safer expansion of constricted arches, enhanced post-orthodontic treatment stability and
extended envelope of tooth movement. This case report describes a surgical technique and case report
involving periodontally accelerated osteogenicorthodontics.
I. Introduction
Periodontally accelerated osteogenic orthodontic (PAOO) technique is defined as the orthodontic
treatment combined with alveolar corticotomy along with bone augmentation grafting. As a promising adjuvant
technique based on the transient nature of demineralization-remineralisation process in healthy tissues, PAOO
consists of alveolar corticotomy and bone grafting of labial and palatal/lingual surfaces, followed by orthodontic
force. Significant acceleration in orthodontic tooth movement has been extensively reported following a
combination of selective alveolar decortications and bone grafting which is responsible for increased scope of
tooth movement and long term improvement in periodontal health.
Main benefits of PAOO are reduced orthodontic treatment time, increased bone volume and post-
orthodontic stability. The average orthodontic treatment time for adults is considerably longer than for
adolescent patients, ranging from 18.7 to 31 months2-4
. It is also more likely for adult patients to experience root
resorption because of an aplastic, narrow, and less vascular periodontal membrane, as well as denser, avascular,
and aplastic bone5
. Surgically assisted orthodontic tooth movement has been used since the 1800s. Corticotomy-
facilitated tooth movement was first described by L.C. Bryan in 1893, published in a textbook by S. H.
Guilford6
. In the past 50 years, rapid tooth movement without significant root resorption has been reported7-10
.
In these cases, the total treatment time was reduced to one-third to one-fourth that of traditional non
extraction and extraction orthodontic treatments10
. The number of adult patients seeking orthodontic treatment
has been increasing in the recent years. The efficiency of orthodontic tooth movement in these patients can be
increased when well‑planned force systems are applied to bony tissues that offer less resistance against the
desired movement which results in faster orthodontic movement,treatment goals can be achieved in a shorter
period, without compromising the results.
There are several differences between orthodontic treatment of adults and adolescents, being it
psychological, biological and clinical differences. Adults have more specific objectives and concerns related to
dentofacial aesthetics, the type of orthodontic appliance and the duration of treatment. An important factor of
consideration in adult patients is that they are more prone to periodontal complications since their teeth are
confined to thick mineralised alveolar bone.
The role of periodontics in PAOO is becoming important. It is important for the periodontics to know the details
of procedure to fulfill the need of patients.
II. Case Report
The patient, a 27-year-old female, was undergoing orthodontic treatment since one and half years, but
the orthodontist couldn’t close the space between 17 and 15, 27 and 25 within this time period. A decision was
made to perform PAOO to accelerate the space closure during the treatment process.
Patient was referred to Dept of Periodontology, A.J Institute of Dental Sciences for further treatment.
Periodontal Treatment Procedure
In step one, oral prophylaxis was carried out using ultrasonic tips and recalled after 1week to perform
PAOO.
Pre-treament analysis for the space was done using cephalometric tracing (Fig.2) and cast which was
recorded in millimeters(Fig.1).

In step two PAOO was performed after 1 week.
All surgical procedures were performed after obtaining the consent of the patient. After administration
of local anesthesia 2 % lidocaine with adrenaline (1:100000), crevicular incision was given buccally extending
from 15 to 25 (Fig.4). Full thickness mucoperiosteal flap was reflected beyond the apex of the teeth (Fig.5).
Selective decortication was performed on buccal site (Fig.6). Vertical corticotomy cuts were made between the
roots from 15 to 25 using a diamond round bur size 2mm in diameter, stopping just short of the alveolar crest
(about 3mm). Cortical perforation was made at the selected areas to increase blood supply to graft. Grooves
were filled with graft material (tata bone graft) (Fig.7) and (Fig.8). Closure was done with interrupted suture
using 4-0 resorbable sutures (Fig.9). Surgical area was covered with periodontal pack (Fig.10). Post operative
instructions were given with medication amoxicillin, 500 mg t.i.d. for 5 days and chlorhexidine mouth rinse
0.2% b.i.d. for 2 weeks. Ibuprofen as needed was prescribed for postoperative discomfort. Patient was recalled
after 1 week for suture removal and revaluation. Orthodontic appliance was activated after 1 week of the
surgery.

In step three One week after surgery orthodontic treatment began.
Cephlometric tracing and cast analysis for space closure between 17 and 15, 27 and 25 was done after
four months. There was closure of 6.5mm of space.

III. Discussion
Periodontally accelerated osteogenic orthodontic technique has been commonly used in conjunction
with orthodontic treatment. The orginal technique is described by Kole7
included a combined intra radicular
corticotomy and supra apical osteotomy. later the supra apical osteotomy was replaced by corticotomy and
labial and lingual corticotomy cuts were used to circumscribe the roots of the teeth.
Reduction of orthodontic therapy time is considered to be an important goal in management of
malocclusion. Corticotomy has been proposed as an alternative to conventional orthodontic treatment in
different adult case.
Hsuan-Chen Chang et al1
reported that the complete retraction of anterior teeth combined with
corticotomy in cases with severe maxillary protrusion took less than 3 and half months. In cases performed with
PAOO it was observed dramatic changes in orthodontic time when bone responding to traumatic stimuli, bone
tissue have a initial stage called regional accelerating phenomenon(RAP) characterized by a transient increase in
the bone turn over and a decrease in trabacular bone density.
In this case study, a rapid space closure was achieved in 4 months.
According to Hajii10
, an average treatment time for the PAOO procedure was one-third to one-fourth of
traditional orthodontic treatment. Wilcko et al11
reported an average of 6.1 months of treatment time for the
PAOO procedure. The present case result agreed with these observations. The present case is also in accordance
with the procedures described by Köle and Wilcko et al4
, corticotomy was performed only at the buccal aspects
in this case. This was in agreement with Germec et al12
, who reported rapid tooth movement
when corticotomy was performed at the buccal aspects of alveolar bone.
This study adds a note on that the space closure or tooth movement which couldn’t be achieved by the
orthodontic treatment in one and half years of time period could be achieved by the PAOO technique at a shorter
time period. Also this study indicates that PAOO is the better technique when orthodontist find difficulty in
closing the space and moving the teeth.
IV. Conclusion
Corticotomy facilitated orthodontics is an effective treatment alternative in adults with severe
malocclusion to decrease treatment time and increase quality of treatment. The periodontics should choose
appropriate surgical technique according to alveolar topography.
