5. Guidelines during the examination:
1. You are required to wear your school uniform.
2. You are required to supply the needed information both for
the Examinee Identification Sheet (EIS) and Examination Answer
Sheet, and shade the letters/numbers corresponding to the
supplied information.
3. In accomplishing the Examinee Answer Sheet, you are given
utmost 15 minutes to fill out the boxes for the School Code and
the Examinee Number and write in full in the box provided the
subject to be taken.
6. 4. Books/notebooks/instructional materials should be
placed under your seats/tables front/back part of the
classroom.
5. Calculators that are PROGRAMMABLE are NOT allowed.
6. The use of scratch paper is NOT allowed. You may use
the back page of the question sheets for their solutions.
