III. ENTRANCE AND SEATING ARRANGEMENT (WAITING ROOM)
1.Inspect the seating arrangement before instructing the examinees to enter the waiting
room. There should be 5 columns and 4 rows of armchair. Check if the first and last rows
are close to the walls.
2.Instruct the examinees to line up outside the room and observe proper social distancing.
3.Ensure that the prescribed COVID-19 Health Protocols are implemented by checking the
learner’s body temperature & spraying alcohol on their hands upon entering the testing
room by the school health personnel.
4. Examinees shall fill out the health declaration form to be facilitated by the school health
personnel.
5.Instruct the students to proceed to the testing room.

1.Inspect the seating arrangement before instructing the examinees to enter the testing
room. There should be six rows and 5 columns of armchair. Check if the first and last rows
are close to the walls.
2.Instruct the examinees to line up outside the room and observe proper social distancing.
3.Call out the names of the examinees by alphabetical order of their surnames. Let the first
five examinees occupy the front line first, then the second, until the last line as shown in
the next page.
4.Check again the seating arrangement by calling out the names of the examinees from the
list prepared by the School Head/Testing Coordinator.

5. Check the identities of the examinees inside each examination room by all means using the
following procedure:
Countercheck their identity through their ID.
If an ID card is not available, ask the examinee to present a notebook or any
belonging bearing his/her name.
In the absence of requirements for A & B identification materials, ask his/her
respective classmates inside the Examination Room to attest the identity.
6. Cellular phones and other gadgets should be kept inside their bags. Instruct them that all
belongings shall be placed in front of the room underneath the blackboard; except the
pencils, sharpeners and clean sheets of paper. Make sure that only pencil, ballpen,
rubber eraser, and a clean sheet of paper are on the chair/desk of each examinee.

IV.ORIENTATION

V. DISTRIBUTION OF TEST MATERIALS (FOR GRADE 3 ONLY)
1.Open the envelope (one containing the TQs and the other the AS). Check their quantity
and see to it that these are clean and no unneccesary markings. In a pack, there are
twenty (20) TQs. Each examinee will have one TQ and one AS.
2.In the distribution of the TQs and AS follow the numbering of examinees. Start with the
examinee in Number 1,
3.Seats of late comers / absentees should be left vacant. Test Questionaires and Answer
Sheets allocated to the said examinees should be placed inside the original envelope by
the Room Examiner pending their arrival.

VI. FILLING OUT THE INFORMATION
1. Instruct the examinees on how to fill out the informations in the answer sheet (for
Grade 3) and computer-based exam (Grade 6, 10, & 12).
2.Give the Examinees enough time to fill out the needed information.
3.Check and double check that all Examinees have responded to all the required
information in the EDQ. Go around and inspect each Examinee’s Answer Sheet /
Monitor. (THIS IS VERY IMPORTANT).

I. ADMINISTERING THE TEST
1.Instruct examinees to accomplish the Part I- Examinee's Descriptive Questionnaire
2.Each examinee shall fill out all the needed information.
3.Administer the Part II- Test Questionaire I
4.The Examiner will read the questions in English up to item 5 only. The examinees will continue
answering all the remaining items.
5.After the English test, Examiner gets the Filipino and reads up to item no. 5 only. The
examinees will continue answering all the remaining items.
6.The same process indicated in Step 4 & 5, will be done to all the other subjects.
7.Record on the board the time started and the time to end this test. Go around and see to it that
the examinees answered the correct part of the test. Note on the board the item to end. The
indicated time to end is from the time started, excluding the 15-minute snacks break.

8. The unused Answer Sheets shall be collected by the Room Supervisor/Chief Examiner at the time when the
Examinees are answering the last subtest. Likewise, the filling out of the remaining data in Form 4 should be
done at this time.
9. Collect and count all used and unused test booklets and answer sheets and place these in the envelope.
Make sure that all materials have been retrieved. In case a Test Questionaire is missing expend all means to
find it.
III. DISMISSAL
1.Dismiss examinees only after accounting the test materials.

The Chief Examiner does the following:
Collects and accounts all ETREs, arranges and bundles them accordingly seeing to it that the
number of ETREs tallies with the number of testing rooms. A report on the number of ETRE’s
should accompany the bundle.
Counts the Test Quetionaire retrieved from each Examiner.
