Calcium metabolism is regulated by the interplay of calcium absorption from the intestine, renal excretion, and bone uptake/release, controlled by vitamin D, parathyroid hormone (PTH), and calcitonin. PTH acts on bone to promote rapid calcium release and long-term bone resorption by osteoclasts. It also increases calcium reabsorption and decreases phosphate excretion in the kidneys. Vitamin D promotes intestinal calcium and phosphate absorption and bone mineralization by increasing formation of calcium transport proteins in the intestine and kidney.
This document discusses gastroesophageal reflux disease (GERD). It begins by defining GERD as a condition caused by stomach contents refluxing into the esophagus and causing troublesome symptoms or complications. It then discusses the pathophysiology of GERD, noting that the lower esophageal sphincter normally acts as a barrier but can become disrupted, allowing acid to reflux from the stomach into the esophagus. The document outlines the clinical manifestations of GERD including heartburn, regurgitation, and extraesophageal symptoms. It also discusses diagnostic evaluations for GERD including endoscopy, pH monitoring, and manometry. The document concludes by covering treatment options for GERD including lifestyle modifications
Protein Calorie Malnutrition, also known as severe malnutrition, can have several causes such as worm infestations, infections, poor feeding practices, and poverty. It manifests as Marasmus, Kwashiorkor, or a combination of both. Treatment follows WHO guidelines and has three phases - initial treatment to address life-threatening issues like dehydration, a rehabilitation phase using specialized feeds, and follow-up monitoring. Dietary supplements including vitamin A, iron, and folate are also important parts of the recovery process.
Gout is a metabolic disorder resulting from elevated uric acid levels and deposition of urate crystals in the joints. It involves a spectrum of conditions including hyperuricemia, acute inflammatory arthritis attacks, tophaceous deposits of urate crystals in and around joints, renal disease, and kidney stones. Gout typically involves recurrent attacks of severe pain and swelling in joints like the first metatarsophalangeal joint. Chronic untreated gout can cause permanent joint damage and deformity from urate tophi. Diagnosis involves identifying urate crystals in joint fluid or tissues.
Excessive vitamin D intake can lead to vitamin D toxicity. The 25-OH metabolite plays a key role in vitamin D intoxication by competing for intracellular receptors, inducing responses normally caused by 1,25-(OH)2-D3. This leads to hypercalcemia through increased calcium absorption and bone resorption, decreasing PTH and kidney function and disrupting calcium homeostasis. Long-term effects include calcinosis, the deposition of calcium and phosphorus in soft tissues like the heart and kidneys. Risk depends on calcium and phosphorus intake in addition to vitamin D exposure.
This document discusses malabsorption syndrome, which occurs when there is abnormal absorption of nutrients in the gastrointestinal tract. It can be caused by digestive failures, structural defects, mucosal abnormalities, infections, or other systemic diseases affecting the GI tract. Common causes discussed include celiac disease, tropical sprue, Crohn's disease, short bowel syndrome, and bacterial overgrowth syndrome. Symptoms include diarrhea, steatorrhea, weight loss, and nutrient deficiencies. Management involves replacing lost nutrients, treating any underlying causes, and making dietary modifications.

Osteoporosis is a disease characterized by low bone mass and deterioration of bone tissue, leading to fragile bones and increased risk of fractures. It is defined by the World Health Organization as a bone mineral density score of -2.5 or below. Those at highest risk include older adults, especially post-menopausal women, and those with low calcium/vitamin D intake or other risk factors such as smoking. Symptoms include back pain from fractured or collapsed vertebrae. Diagnosis involves bone mineral density tests and other lab tests. Treatment focuses on calcium, vitamin D, bisphosphonates, and lifestyle changes to build strong bones early in life and prevent bone loss later in life.
This document provides information on osteoporosis, including its definition, classification, epidemiology, clinical features, investigations used for diagnosis, and management. Osteoporosis is defined as a reduction in bone density leading to an increased risk of fractures. It is classified based on whether it is primary/secondary and type. Diagnosis involves tests like DEXA scans, biomarkers, and imaging. Management includes lifestyle modifications, calcium/vitamin D supplementation, and medications like bisphosphonates, teriparatide, and denosumab that reduce resorption or stimulate bone formation.
This document outlines a lecture on peptic ulcer disease (PUD). It begins with definitions of PUD and ulcers, noting the typical burning pain exacerbated by fasting and relieved by eating. It then discusses gastric physiology including the mucosal defense system and role of prostaglandins. Key causes of ulcers are described as H. pylori bacteria and NSAID use. Clinical features, complications, differential diagnosis, diagnostic tests, and treatment approaches are summarized. The role of H. pylori and NSAIDs in duodenal and gastric ulcers is emphasized throughout.
Folic acid, also known as vitamin B9, is a water-soluble B vitamin that occurs naturally in many foods. It is important for cell division and growth, especially during periods of rapid cell division and growth like infancy and pregnancy. Folic acid works as a coenzyme in single carbon transfers that are critical for DNA synthesis and metabolism. A deficiency can result in health issues like megaloblastic anemia or neural tube defects in developing embryos. Good dietary sources include leafy green vegetables, legumes, liver, and fortified grains.

American Journal of Clinical
Nutrition,
Clinical Cancer Research,
International Journal of
Breast Cancer Research and
Cancer Prevention

Соя
Cancer
Epidemiology, Biomarkers
and Prevention journal, са
Cancer Causes and Control,
32. Чай
Breast
Cancer Research,
Cancer Epidemiology,
Biomarkers & Prevention,
American Journal of Clinical Nutrition,
Journal of the National
Cancer Institute,
в Journal of the
National Cancer
Institute,
