therapy (MT) affects conditioned pain modulation (CPM) among adults with chronic idiopathic neck
pain (CINP).
Scope of the problem: Why is the study important?
Chronic neck pain is common, costly, and a leading cause of disability. Literature suggests that
ascending pain pathways (i.e. pain pressure threshold or PPT) and/or descending pain pathways (i.e.
CPM) may be impaired in those with chronic pain [please see Appendix for illustration of ascending
and descending pain pathways]. Spinal mobilization (SM), a form of MT, is a common treatment that
has been effective in relieving neck pain according to Cochrane Review (Gross et al 2015). A recent
systematic review (Coronado et al 2015) concluded that although the pain relief from MT seems to
improve PPT; the pain relief, however, is not associated with PPT, a measure of the ascending pain
pathways. Whether the pain relief from SM is due to its effects on the descending pain pathways (e.g.
CPM) is not known at this time.
CPM is a method to test one s endogenous descending pain inhibition status. It is a simple test that
uses a second painful stimulus to identify if a person has a normal functioning CPM or not. Pressure

Rene Descartes was a French philosopher in the 17th century. Descartes was not pleased with what
philosophy was based on in during this time in history. He took it upon himself to raise questions in
the world of philosophy. He questioned well known beliefs and beliefs held by many, not just
philosophers. In his meditations, he explains his process of emptying his mind and starting completely

He argues that this is different from normal thought because one can think of something without it
being present. He uses the example of the chiligon by saying that he can think of this many sided
shape, but he cannot form a picture (or image) of it in his mind. Descartes argues that one uses effort
when they are imagining, but not while thinking. He also says that imagination is not a necessity for
being; that he could go on without it. One does not need imagination to exist, Descartes believes.
Descartes thinks that thought is the mind turning on, or arguing against, its own ideas, and that when
imagining things, the mind turns its attention to one s body. From these statements and conclusions,
Descartes makes his final conclusion by saying people cannot imagine things without the existence of
their body. He tells readers that this might be only a chance. At this point he cannot say for sure if the
body exists. He then goes on to interpret and think through the uses of senses and decides if they
deceive him. He wonders if they deceive him of the existence of material objects existing. He says that

provide self care. I would make sure to eat three times a day making sure I consume the appropriate
nutrients. I would try to maintain a minimum of seven hours of sleep each day. I would also take an
