This document discusses the management of patients with xerostomia (dry mouth) in prosthodontics. It defines xerostomia and lists its common causes such as medications, medical conditions, and radiation therapy. Signs and symptoms include dry mouth, mouth cracks, tongue burning, and taste alteration. Problems for denture patients include reduced retention and increased friction. Management involves symptomatic relief, addressing underlying causes, stimulating residual gland function, using saliva substitutes, encouraging hydration, and optimizing oral hygiene. Prosthodontic treatments aim to improve retention, stability, and comfort through techniques like using metal denture bases and soft liners. Frequent recalls are needed due to increased risk of denture

The human mandible can be related to the maxilla in several positions in the horizontal plane. Among these centric relation is a significant position, because of its usefulness in relating the dentulous and edentulous mandible to maxilla, where the teeth , muscles and temporomandibular joint function in harmony. It is a position of occluso-articular harmony.

Fixed partial dentures transmit forces through the abutments to the periodontium. Failures are due to poor engineering, the use of improper materials, inadequate tooth preparation, and faulty fabrication. Of particular concern to prosthodontist is the selection of teeth for abutments. They must recognize the forces developed by the oral mechanism, and resistance.
Successful selection of abutments for fixed partial dentures requires sensitive diagnostic ability. Thorough knowledge of anatomy, ceramics, the chemistry and physics of dental materials, metallurgy, Periodontics, phonetics, physiology, radiology and the mechanics of oral function is fundamental.
There have been several changes since inception in the field of dental ceramics. Need for newer materials with improved aesthetics, flexural strength and optical properties made it necessary for introduction of advanced technology in fabrication of dental ceramics.
The patient who requires a single denture opposing a natural or restored dentition challenges the clinician even more than the completely edentulous patient does. This is due to the biomechanical differences in the supporting tissues of the opposing arches. So a proper evaluation, correction of the existing factors and proper sequence of denture construction is necessary to give a more stable prosthesis.

Is the Physiological Mobility of the Teeth Disturbed Because Of the Retainer...Abu-Hussein Muhamad
Abstract: In Orthodontics The Stability Of The Achieved Result Remains A Fundamental Issue Of Concern And Debate. Tirk Has Said “The Result Of Orthodontic Therapy – Good, Bad Or Indifferent Is Only Evident Many Years Out Of Retention” . Maintaining Teeth In Their Corrected Positions After Orthodontic Treatment Has Been And Continues To Be A Challenge . Usually A Retention Phase Is Required After Active Orthodontic Tooth Movement To Hold Teeth In Its Ideal Aesthetic And Functional Relation And Prevent The Teeth To Return To Their Former Position.Fixed Appliance Therapy Often Lasts Several Years. Gladness About Debonding Is Great And The Patient Consider The Occasion As The End Of Treatment Many Times. Patients As Well As Their Parents Often Underestimate The Importance Of The Following Retention Period And How Quickly Negligences In This Part Of Treatment Result In Recidivisms. Fixed Retainer Guarantee A Good Long-Term Stability At Least As Long As They Are In Situ. The Reliable Attachment Of Lingual Retainers With Modern Bonding Procedures Made Them A Popular Retention Method. To Test Their Influence On Tooth Mobility Or Damping Quality The Presented Study Was Performed With A Dynamic Test Procedure (Periotest).

18. Fixed Prosthodontics
In dry environment, fixed non tissue bearing
prosthesis are preferred where indicated
FPDs should have full coverage retainers and
easily cleaned pontics and connectors

Use of gingivally approching clasp avoided
Tooth supported denture with minimal tissue
coverage
Metal denture bases are preferred
Removable Partial denture
20. Complete denture
Procedures -aim at optimizing retention and
stability
Use dentures with metal bases
Use of soft liners to improve comfort
Use of denture adhesives to augment retention
Frequent recall – As more prone to candidal
infections

25. Conclusion
Clinicians should be aware of the signs and
symptoms, diagnostic procedures, etiologies,
sequelae and appropriate therapeutic
regimens
Effective evaluation and appropriate
treatment will promote acceptable levels of
comfort and function

Editor's Notes
Xerostomia is a condition associated with both a decrease in the amount of saliva produced and an alteration in its chemical composition, therefore causing dry mouth . It can have profound negative effects on the quality of life.
Dehydration from any cause including drug-induced
Extreme discomfort in wearing denture
Dentures do not adhere to tissues and are not retained as well, Lack of lubrication increases frictional forces between dentures and oral mucosa, causing sores.
Patients should be . Frequent use of products containing aloe vera or vitamin E should be encouraged. Avoid Tobacco use, spicy, salty, and highly acidic foods that irritate the mouth
Side effect of high dose radiotherapy in head and neck (Aminofostine, selective cyctoprotector limits undesirable effect .Aminofostine, selective cyctoprotector that acts upon salivary glands, kidneys, liver, heart or bone marrow can be used to limit undesirable effects of radiotherapy of head and neck cancer
Salivary stimulation by physiologic(mastication) or pharmocological methods, organic acid like ascorbic acid, malic acid and citric acid increases salivation but has tendency of deminieralization. Sorbitol is a sugar substitute
Pilocarpine – reported since1960 and cevimeline HCL , Mechanism of action- stimulation of both local and direct cellular stimulation, parasymphathetic action(parasympathomimetic and muscarinic agonist) induces water and electrolyte flow of saliva –also stimulate mucin and other salivary constitutens(For minimum of 90 days for optimum result). Side effect- aas osmotic laxative agent can induce GIT symptoms such as dirrohea, due to its osmotic potential
examined 207 patients undergoing head and neck radiation therapy and taking pilocarpine for symptoms of xerostomia
With enzyme(e.g. Biotène® Oralbalance Dry Mouth Gel)(mucin based saliva orthona Indian J stomatology 2011 GS Chandu)
Xerostom oral hygiene cleans and relieve dryness, lubricating and replenishing moisture in the oral cavity
Contain three primary enzymes: lactoperoxidase, lysozyme, glucose oxidase and one protein that is naturally found in human saliva called lactoferrin . This enzyme system acts to penetrate the cell wall of plaque-forming bacteria, helping to maintain a healthy balance of oral flora. Candida species are sesnsitive to this enzyme
This study examined 19 patients receiving head and neck radiation treatment for advanced cancer. These patients also reported less dry mouth upon waking
Clinical procedures and laboratory procedures .Dentures with metal bases exhibit improved accuracy of fit and effective wettabiliy contributing to better retantion.Metal bases prosthesis are easy to clean and have less plaque accumulation.Use of well hydrated denture adhesive to improve retention and stability and cushinioing and lubricating effect
Saliva reservoir -Chamber incorporated into removable prosthesis - provides flow of salivary substitute for certain period of time
Unclear - maxillary or mandibular prosthesis
and is SLS-free
Successful management of prosthodontic patient is complicated by xerostomia.
