An elderly female patient underwent an endoscopic ultrasound that revealed a pancreatic cyst with an irregular thick wall. A fine needle aspiration was performed. A young boy presented with failure to thrive, abdominal pain, and distension. Endoscopy found nodular hyperplasia in the duodenum which was biopsied and treated. A second patient underwent endoscopy for upper gastrointestinal bleeding, where a bleeding duodenal ulcer was treated with endoscopic hemostasis.

Triple atresia case report publication
This case report describes a rare case of triple intestinal atresia involving the jejunum, ileum, and colon in a newborn female with Down syndrome and imperforate anus. The baby underwent emergency laparotomy which revealed atresias in the jejunum, ileum, and colon. Histopathology of the resected intestinal segments showed no specific cellular abnormalities. Unfortunately, the baby did not survive due to post-operative cardiopulmonary complications. Triple intestinal atresia is an extremely uncommon condition. The presence of Down syndrome and imperforate anus with triple atresia makes this an especially rare case.

Hirschsprung's disease is a congenital condition where the distal segments of the intestinal tract lack intrinsic ganglion cells. This causes mechanical obstruction due to failure of the affected areas to relax during peristalsis. It most commonly affects the rectum and sigmoid colon. The diagnosis is established through a combination of clinical presentation, radiological imaging like barium enema, rectal biopsy, and manometry. Treatment involves initial decompression and colostomy followed by a definitive pull-through surgery like Swenson or Soave procedure to remove the aganglionic segment. Complications can include anastomotic leak, postoperative enterocolitis, constipation and incontinence.

Treatment is surgical removal of the affected portion of intestine. The most common procedures are Swenson, Duhamel and Soave operations, which have mortality rates of 1-6% and risks of complications like leakage or enterocolitis.
Chronic osteomyelitis
A child presented with a 1 month history of swelling around the left elbow associated with evening rise in temperature. They had fallen and injured the elbow 2 months prior. Examination found a 4 by 4 cm soft, swollen area over the proximal forearm with local rise in temperature. Tests revealed elevated ESR and CRP. MRI and biopsy showed evidence of tuberculous inflammation. The child underwent incision and drainage of the collection and was started on anti-tubercular treatment.

2) It occurs due to abnormal development of the tracheoesophageal septum during embryogenesis.
3) The main treatment is surgery to connect the ends of the esophagus and separate it from the trachea, which requires delicate techniques.

A 12-year-old Chinese boy was admitted with abdominal discomfort and distension. During surgery, his enlarged spleen was removed due to a cystic mass. The spleen filters the blood, produces antibodies, and stores platelets. Its blood supply is the splenic artery and drainage is through the portal vein. The ligaments that hold it include the splenogastric, splenophrenic, and splenocolic ligaments. Common reasons for splenectomy are trauma and hematological/immunological disorders. The most worrisome complication is sepsis from encapsulated bacteria, which can be prevented through pre-surgery vaccinations.

Autoinflammatory disorders (AIDs) are characterized by aberrant regulation of the innate immune system leading to recurrent episodes of systemic inflammation without evidence of autoimmunity. AIDs can be hereditary monogenic disorders caused by mutations in genes regulating the inflammasome and interleukin 1 beta processing. Common symptoms include periodic fevers and rashes. Treatment may involve inhibition of interleukin 1 or tumor necrosis factor alpha.

Dietary fiber from whole foods like grains, legumes, vegetables, and fruits has demonstrated benefits for gastrointestinal (GIT) health. Isolated and extracted fibers also show promising regulatory effects on the gut and microbiome. However, fibers have varying physicochemical properties depending on their origin and processing that influence their functional characteristics and clinical applications. More research is needed, including well-designed randomized controlled trials, to determine which fiber sources, characteristics, doses and durations optimize GIT health benefits and manage gastrointestinal disorders. Combining fibers with different physiological effects may be a promising therapeutic strategy.

- Coronaviruses typically cause common colds but SARS-CoV and MERS-CoV can cause pneumonia, respiratory failure, and death. A novel coronavirus, SARS-CoV-2, emerged in Wuhan, China in late 2019 and caused a global pandemic.
- SARS-CoV-2 spreads mainly through respiratory droplets when people cough, sneeze or talk within 6 feet of each other. Asymptomatic and pre-symptomatic people are highly infectious.
- COVID-19 symptoms range from mild to critical illness. The elderly and those with pre-existing conditions are at higher risk for severe disease. Diagnosis involves PCR testing of respiratory samples.

- Sexual dysfunction and infertility are more prevalent in men with IBD compared to the general population.
- Depression is the most consistent negative predictive factor of sexual function among men with IBD.
- Sulfasalazine can reversibly reduce male fertility, so it is recommended to discontinue 3-4 months prior to conception. Most other IBD medications do not significantly impact fertility.
- Men with IBD have an increased risk of prostate cancer and prostate cancer screening guidelines for higher risk patients should be followed.

Upper and lower GI endoscopies have diagnostic, therapeutic, and screening indications. For upper endoscopy, common diagnostic indications include dyspepsia in patients over age 60 or with red flags, upper GI bleeding, and dysphagia. Therapeutic uses include treatment of bleeding, removal of foreign bodies, dilation of strictures, and stenting. Screening is done for portal hypertension, Barrett's esophagus, eosinophilic esophagitis, and cancers. Lower endoscopy indications involve diagnostic evaluation of bleeding, abdominal pain, diarrhea, and screening of average-risk adults over 50 and high-risk groups. Therapeutic colonoscopy is used for polypectomy, bleeding treatment, and stricture dilation

Dr. Hiwa Abubakir and Dr. Mohamed Alshekhani discuss the management of nonvariceal gastrointestinal bleeding. They recommend endoscopy within 24 hours, or earlier for high-risk patients, to identify and treat the bleeding source. A variety of endoscopic tools can be used for hemostasis, including injection of adrenaline combined with clips, bands, electrocautery, or laser coagulation. Newer tools like over-the-scope clips are best for high-risk or recurrent bleeders. Doppler probes help ensure complete vessel occlusion. Additional prothrombotic agents or interventions may be needed in some cases.
