- 3. www.dhp.virginia.gov/nha/nha_forms.htm AIT Application – State Board • 480 Hour Requirement • Application found on VBLTCA website • Complete with your Preceptor • App requires an official copy of transcripts • $215 Application Fee • Due prior to your practicum semester Practicum Proposal – VCU • 600 Hour Requirement • Template found on Blackboard Student Org • Complete with your Preceptor • Due Monday before Final Exam week the semester before you begin
- 4. www.dhp.virginia.gov/nha/nha_forms.htm AIT State Board Requirements • Monthly Report • To be completed by you EACH MONTH and approved by your preceptor • Turn original in to the Board • Send copy to Jen Pryor + Jenny Inker with your reflections VCU Gerontology Requirements • Monthly or Bi-Monthly check in with: Dr. Welleford, Jen Pryor + Jenny Inker • Reflective process about your learning • Midterm report from preceptor: template found on Blackboard Student Org

AIT State Board Requirements • Completion of AIT form • Due upon completion of your AIT hours or practicum VCU Gerontology Requirements • Practicum Final Report – Student • Practicum Final Report – Preceptor • Copies of your Deliverables • DUE the Monday before Final Exam week the semester you intend to graduate www.dhp.virginia.gov/nha/nha_forms.htm
