The proposal requests $100,000 to develop a patient navigation toolkit and resource website, and deliver an interactive day-long education and training conference for 200 Chicagoland breast health providers in November 2012. The conference will provide comprehensive training through education presentations and interactive workshop sessions. It will also develop a resource website and workbook toolkit covering program planning, implementation and evaluation to help providers develop accredited patient navigation programs.
