Root resorption is a condition characterized by a partial loss of root cementum and dentin.
Root resorption of the deciduous dentition is a physiological process and it is a necessary precursor to the eruption of permanent teeth.
Permanent teeth root resorption is a pathological inflammatory process and it can be affected by several factors
Apical root resorption can be also related to an orthodontic treatment and it can be present during the treatment or at the end of it.
This root resorption is called orthodontically- induced inflammatory root resorption (OIRR) and it is considered a distinct pathologic process.
Patient-related and treatment-related factors are involved in the onset and progression of this root resorption.1. Cemental or surface resorption with remodeling. In this process, only the outer cemental layers are resorbed, and they are later fully regenerated or remodeled. This process resembles trabecular bone remodeling.
2. Dentinal resorption with repair (deep resorption). In this process, the cementum and the outer layers of the dentin are resorbed and usually repaired with cementum material. The final shape of the root after this resorption and formation process may or may not be identical to the original form.
. Circumferential apical root resorption. In this process, full resorption of the hard tissue components of the root apex occurs, and root shortening is evident.
Orthodontic forces applied to the biologic system act similarly on bone and cementum, which are separated by the periodontal membrane. If there are no differences in the biologic behavior of these two organs, both would resorb equally.
Since cementum is more resistant to resorption compared with the more vulnerable bone, applied forces usually cause bone resorption, which leads to tooth movement. However, resorption of the cementum and dentin may also occur
Several theories explaining the resistance of the dental tissues, especially cemental resistance to resorption, exist.
It is documented that the uncalcified mineral tissues, osteoid, precementum, and predentin are resistant to resorption and may initially prevent loss of root tissue.
These layers might contain noncollagenic materials, eg, the cells themselves, that possess potent anticollagenase propertiesAfter extensive research in this field, mainly with tooth replantation models, Andreasen, relates surface resistance to the innermost cellular layer of the periodontal ligament.
This layer supplies the protective mechanism to the root, as well as the potential for a repair.

Etiology in orthodontics is the study of actual causes of dento – facial abnormalities.
Malocclusion is the condition where there is a deviation from the usual or accepted relationship, dental malocclusion exists when the individual teeth within one or both jaws are abnormally related to each other, this condition may be limited to a couple of teeth or involving the majority of teeth present.
Development of normal dentition and occlusion depends on a number of interrelated factors that include the dento alveolar, skeletal and the neuromuscular factors. Thus localization of the possible etiology may be a very difficult task.
A- Extrinsic factors:
1- Evolution:
With evolution, the jaws become smaller, reduction in the number and size of teeth and diminution of jaw projection together with increased in vertical height of the face and there is retrognathic tendency in man as he ascends the evolutionary scale.
2- Heredity:
Transmission of dento facial characteristic through generation by genes. The child is a product of parents who have dissimilar genetic material. Thus the child may inherit conflicting traits from both the parents resulting in abnormalities of the dentofacial region. Another reason attributed for genetically determined malocclusion is the racial, ethnic and regional intermixer, which might have led to uncoordinated inheritance of teeth and jaws.
There are three types of transmission of malocclusion from the stand point of genetics:
1- Repetitive: the recurrence of single dentofacial deviation within the immediate family.
2- Discontinuous: a tendency for a malocclusion trait to reappear within the family over several generations.
3- Variable: the occurrence of different but related types of malocclusion within several generation of the same family.
Dental defect of genetic origin include the following:
= Crowding and spacing of teeth.
= Size and characteristic of soft tissue including muscles and frenum.
= Macrognathia and micrognathia.
= Macrodontia and microdontia.
= Oligodontia.
= Tooth shape variations.
= Median diastemas.
= upper face height, nose height, and bigonial width.
= Bimaxillary protrusion.
4- Congenital:
Those are deformities of hereditary or non-hereditary origin but exciting at birth.
The congenital abnormalities that cause malocclusion:
= Cleft lip and palate:
lack of fusion between the two palatal processes to each other. From one third to one half of all cleft palate children have familial history of this deformity.
As with the non-cleft child, palatal, pharyngeal and perioral musculature is well developed at birth to meet the demand of suckling, deglutition and mastication. Primary etiologic sites:
1- Neuromuscular system:
The muscle group that serve most frequently as primary etiologic sites are:
== muscles of mastication
== muscles of facial expression
== tongue
The neuromuscular system plays its primary role in the etiology of dentofacial deformity by the effect of abnormal contraction of bony skeleton and the dentition. Both bones and teeth are affected by the many functional activities of orofacial region
2- Bone:
Since the bone pf maxilla and mandible serve as bases of dental arches, changes in dental arches growth may alter the occlusal and functional relationship.
3- Teeth;
The teeth may be primary sites in the etiology of dentofacial deformity in many ways
Gross variation in size and shape are encountered frequently and always are of concern
Decrease or increase in the regular number of teeth will give rise malocclusion
Etiologic factors:
A- Extrinsic factors:

Most authors between 1900-- 1920 did not completely determine the role of inheritance in determination of the form, size and proportion of dentofacial skeleton, but they stress their work upon the effect of the environmental factors, and at this time they were hardly belief that the effect of local lack of function is more important.
Bennet statement: the size, form and density of bones such as maxilla and mandible varies according to the extent to which these structure are used during period of growth – (function stimulate growth)
Walk Joff statement: the form and degree of development of maxilla and mandible depends upon the magnitude of functional stimuli of muscles acting upon these structures.
Baker: his study was performed on animals by unilateral amputation of muscles of mastication, he found lack of growth on the affected side.
Brash: studied the facial form and the dental development in twins on genetic bases, he also emphasized the genetic facial pattern of some royal families in Europe where they had been inter-marriage, his studies gave the best evidence to support the role of inheritance
Axel Lundstorm:1925 showed that, the form and size of dental bases and the teeth are genetically determined, when the size of the teeth and their basal arches are not correlated, problems of crowding or spacing will be arising.
Broadbent and Hofrath 1931: developed standardized cephalometric x-ray technique which permit serial longitudinal studies of facial growth, by this studies the concept of inheritance growth pattern arises

The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

55. These dog experiments turned out to be
misleading, because many breeds of small dogs carry
the gene for achondroplasia.
Most of the malocclusions produced in Stockard’s
breeding experiments can be explained not on the
basis of inherited jaw size but by the extent to which
achondroplasia was expressed in that animal.
So these experiments have been severely criticized
on the basis that the gene for achondroplasia is likely to
have contributed.

The classic way to determine to what extent a
characteristic is determined by inheritance is to
compare monozygotic twins (identical) with
dizygotic twins (fraternal).
Studies of this type are limited because it is
difficult to establish zygosity and confirm that
environments were in fact the same for both
members of a twin pair.
Lauweryns et al ; concluded that 40% of dental
and facial variation that lead to malocclusion can
be attributed to hereditary factor.

The other classic method of estimating the
influence of heredity is to study family
members by observing similarity and
differences between father-child, mother-
child, and sibling pairs.
For most measurements of facial skeletal
dimensions (i.e. length of the
mandible), correlation coefficients for parent-
child pairs are about 0.5 .
For dental characteristics, the parent – child
correlations are lower, ranging from a
maximum of nearly 0.5 for overjet to a

Nakasima et al. found high correlation co-
efficient values between parents and their
offspring in the class II and class III malocclusion
groups suggesting a strong familial tendency in
the development of these malocclusions.
The genetic effects of determining the
craniofacial form of offspring by the father were
equivalent to those by the mother, but daughters
were more affected by parents than were sons
because of this finding they suggested a major
influence of X-linked genes from 0.5 to 0.9.

slightly less than 20% of the isolated cleft palate cases appear to

nasorespiratory distress, mouth breathing, tonsillitis, or pharyngitis.
•When the tonsils are inflamed, the root of the tongue may encroach
on the enlarged facial pillars.

If the postural position is normal, the tongue thrust swallow has
no clinical significance because tongue thrust swallowing simply
has too short a duration to have an impact on tooth position.
• Pressure by the tongue against the teeth during a typical
swallow lasts for approximately 1 second.
• A typical individual swallows about 800 times/day while
awake but has only a few swallows /hour while asleep. The
total/day therefore is usually under 1000.
• One thousand seconds of pressure, of course, totals only a
few minutes, not nearly enough to effect the equilibrium.

• For the great majority of patients with endogenous

serious clinical problem, but on rare occasions, quite serious
open bites are present.

• Usually it is the adaptation to enlarged tonsils,
pharyngitis, or tonsilitis.
• When the acutely inflamed throat (pharyngitis or
tonsilitis) is anesthetized, the adaptive protracted posture
of the tongue may spontaneously correct to a more
normal position.
• As long as the precipitating pain mechanism is present,
the tongue will posture itself forward, and repositioning
of the incisors will not be stable. Therefore, it is best to
refer such patients to a physician for correlative therapy.

descends in the neck, air can be taken in through the
mouth, but this is normally prevented by 3 sphincter
mechanism:
Anterior sphincter : formed by lips.
Intermediate : formed by tongue and hard palate

Failure of these barriers causes mouth
breathing, which is mostly intermittent and more
common at night.

adenoids obstruct the airway causing mouth

• Humans are primarily nasal breathers but everyone
breathes partially through the mouth under
physiologic conditions.
• The most prominent being an increased need for air
during exercise.
• At rest, minimum airflow is 20-25 L per minute, but

conversation lead to increased airflow and transition
to partial mouth breathing.
• For the average individual there is a transition to
partial oral breathing when ventillatory exchange
rates above 40-45L/min are reached.

Normally the lower lip at rest covers the lower 1/3 or
¼ of the crowns of upper incisors .
When the upper lip is short, habitual lip closure is
difficult and may contribute to functional
chewing, swallowing, speech etc. and non functional
contacts between upper teeth and lower lip.
Instead of exerting a light continuous force on the
labial segments of upper anterior teeth, the lower lip
cushions against the lingual surfaces and is often
implicated as the principle etiological factor in
labially tipped upper incisor.
