6. ACUTE INVASIVE FUNGAL RHINOSINUSITIS
• An angioinvasive fungal infection of the nasal cavity and paranasal sinuses
• Immunocompromised patients
• The initial presenting symptoms: eye or facial pain and facial numbness,
followed by soft tissue swelling, blurry vision , …
• Can spread into the orbit or mouth
• Blepharoptosis, internal or external ophthalmoplegia, multiple cranial
nerve palsies, headache, and acute vision loss
7. ACUTE INVASIVE FUNGAL RHINOSINUSITIS
• A high index of suspicion is necessary to make the diagnosis
• Imaging studies should be considered as an adjunct to assess for extent of
disease
• Nasal endoscopic evaluation
• The gold standard for diagnosing is biopsy of infected tissues
• Management of AIFR includes antifungal therapy, surgical resection, and
reversal of immunocompromise
