This article was presented in the morning session of MU-II, Dhaka Medical College Hospital.
Acute pancreatitis is a common medical problem. Initial phase of acute pancreatitis is characterized by inflammation. This is caused by release of cytokines and other pro inflammatory mediators. These further cause vasodilatation, intravascular volume depletion, and end organ hypoperfusion. The etiology can be varied but common causes are biliary (stone in CBD) and alcohol. Other causes are drugs, infections, trauma, idiopathic, post ERCP etc. Patients with severe pancreatitis have high risk of mortality (10%) which can go upto 30% if necrosis gets infected, which occurs in about 40% patients. Further, persistent organ failure increases the mortality up to 34–55% as compared to 0.3% with transient organ failure. Traditionally as per Atlanta classification, acute pancreatitis has been classified as mild or severe depending upon organ failure or local complications. Acute pancreatitis is a hyper-catabolic state. Moreover some of these patients may be malnourished to begin with (alcoholics). Thus their nutritional requirements are much more than ordinary person. There are good quality studies available to show that in absence of cholangitis, there is no benefit of doing early ERCP. Also, technically it is more difficult to do in such situations, and procedure related complication may be more. If in doubt, it may be worthwhile to do endoscopic ultrasound to document the presence of CBD stone before attempting to cannulate the CBD.

Introduction: The importance of the admission blood pressure (BP) for intracerebral
hemorrhage (ICH) outcome is not completely clear. Our objective was to
analyze the clinical impact of BP at hospital arrival in patients with primary ICH.
Material and methods: We studied 316 patients (50% women, mean age:
64 years, 75% with hypertension history) with acute primary ICH. The first BP reading
at admission was evaluated for its association with neuroimaging findings
and outcome. A Cox proportional hazards model and Kaplan-Meier analyses
were constructed to evaluate factors associated with in-hospital mortality.
Results: Intraventricular irruption occurred in 52% of cases. A high frequency
of third ventricle extension was observed in patients with BP readings in the
upper quartiles of the distribution (systolic, diastolic, or mean arterial pressure).
Blood pressure readings did not correlate with hematoma volumes. In-hospital
case fatality rate was 46% (63% among those with ventricular irruption). Systolic
BP (SBP) > 190 mm Hg was independently associated with in-hospital mortality
in supratentorial (n = 285) ICH (hazard ratio: 1.19, 95% confidence interval:
1.02-1.38, for the highest vs. the lowest quartile) even after adjustment for
known strong predictors (age, ICH volume, Glasgow coma scale and ventricular
extension). Blood pressure was not significantly associated with ventricular
extension or outcome in patients with infratentorial ICH.
Conclusions: A high BP on admission is associated with an increased risk of
intraventricular extension and early mortality in patients with supratentorial
ICH. However, a significant proportion of patients with high BP readings without
ventricular irruption still have an increased risk of death.

Hypertension is the most common modifiable risk factor for cardiovascular diseases, stroke and renal failure with a reported prevalence of less than 10% in 18-19 years to greater than 60% in over 70 years old Pakistani adults.
It is estimated that more than one billion adults are hypertensive worldwide and this figure is projected to increase to 1.56 billion by the year 2025, which is an increase of 60% from year 2000
Clinical evidence suggests that lowering blood pressure with antihypertensive drugs reduces the risk of myocardial infarction, stroke, heart failure , revascularization procedures and end stage renal diseases in hypertensive patients.
Various classes of antihypertensive drugs like diuretics, beta blockers, calcium channel blockers and inhibitors of renin angiotensin system have been shown to reduce hypertension complications.
Among calcium channel blockers, amlodipine is a commonly prescribed antihypertensive with proven efficacy. However, the troublesome adverse reactions of amlodipine are development of pedal edema and other vasodilation related side effects like headache, dizziness, flushing, palpitation etc.
Lercanidipine is a third generation CCB claimed to have sustained blood pressure lowering with single daily dosing and CCBs related side effects are claimed to be low.

Impact of Malnutrition on Lipid Profile in Chronic Kidney Disease Patients in...Neeleshkumar Maurya
The present study was carried out to identify the role of malnutrition and its relationship for the development of cardiovascular disease (CVD) in chronic kidney disease (CKD) patients taking hemodialysis. We conducted an analytical study with 100 patients. It was carried out over one-year period, from February 25, 2017 to March 30, 2018. The inclusion criteria were the patients who have been on hemodialysis for at least past three months period and at least more than 18-year-old. All the patients were divided into two groups: first group of patients have both CVD and CKD and other group of patients have only CKD. Patients were subjected to biochemical and anthropometric parameters. Out of hundred patients, about 60 followed the inclusion and exclusion criteria. Eight women and 52 men with the age range from 18 to 80 years with 49±10.2years as mean age. We found that higher level of cholesterol, triglyceride, low protein intake and low energy conception in CKD alone patients is directly associated with malnutrition. The association between cholesterol levels and CKD would be altered by the presence of malnutrition. Low level of protein and total energy intake also confirms the presence of malnutrition in CKD patient developed the CVD.

Indeed, increased inflammatory status pre- and post-PCI has also been associated with poor prognosis, and control of the residual inflammatory risk (RIR) in the CANTOS (Canakinumab Anti-inflammatory Thrombosis Outcomes Study) trial has recently opened new perspectives in the field of secondary prevention.
Introduction Psoriasis is a chronic inflammatory & most common dermatologic diseases. It is related to several diseases but most common is cardiovascular disease. Lipid abnormalities are observed in psoriasis patients at the earliest stages of the disease and so it may be genetically determined.Hence there is a great need to study lipid abnormalities in psoriatic patients, which will help us to evaluate the level of risk individuals may experience for developing atherosclerosis and vascular obstructive disorders, as well as associated morbidity and mortality. Aim: To estimate lipoproteins levels & related risk of cardiovascular diseases in psoriasis patients Materials and Methods: This is a community-based cross sectional analytical study. The study population was derived from the healthy ambulatory person in and around Pimpri area & was conducted in Dr. D.Y.Patil Medical College & Hospital, Pimpri Pune. Study group consist of 40 psoriatic patients with mean age of 50.68 ± 8.86 years and control group consisted of 40 healthy individual of either sex with mean age of 50.15 ± 9.6 years. 55 % males & 45 % females were present in control group while test group comprises of 59 % males & 41 % females. estimation of Serum total cholesterol, Serum triglyceride, Serum High-Density Lipoprotein-cholesterol & Serum Low density Lipoprotein levels were done the same day. Estimation of lipoproteins levels done by Enzymatic End Point Method. Tha data was analysed by calculating mean, standard deviation, p-value and Chi-square test. Results : Findings of this show that, total cholesterol ,VLDL was significantly increased in psoriasis compared to control(p<0.05).Difference between psoriasis cases(6.8 ± 1.49) and control (3.08 ± 1.3) in findings of serum TGs were highly significant(p<0.001).However, difference in between serum LDL level in psoriasis cases and control was not significant. There were no significant changes shown in serum HDL level in both psoriasis cases and control . LDL/HDL ration has not shown any significant changes in both psoriasis case and control, while in TC/HDL ratio, difference was statistically significant(p<0.05). Conclusion: Present study suggests that patients with psoriasis were associated with hyperlipidaemia. So have high risk of developing the cardiovascular diseases. We suggest that screening of serum lipid profile in psoriatic patients will help in evaluating risk at early stage. Impromptu treatment in these patients will prevent the risk of cardiovascular diseases.

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

2. INTRODUCTION
• Sepsis is defined as the presence (probable or documented) of infection together with systemic
manifestations
• It is the second leading cause of mortality in the ICU after multi-organ failure.
• Given that the reversibility of severe sepsis is poor, the need for an “Early Prognostic Marker” to
identify those at highest risk for mortality in order to optimize therapeutic options is critical
there by to reduce the ICU mortality secondary to sepsis.
• Recent evidence shows that serum High density lipoprotein cholesterol (HDL-C) may be a
useful prognostic marker of sepsis .
• Additional studies have demonstrated that patients diagnosed with severe sepsis in the ICU
have a measured decrease in circulating level of lipoproteins and an increase in triglycerides
independent of comorbidities.
3. REVIEW OF LITERATURE
Naresh Monigari et al ( July 2015 : Manipal)(1) concluded that there is significant association
of low HDL value on day 1 with mortality.
Risk of death in patients with day 1 HDL value <10mg/dl is 7.01 times the risk of death in patients with
day 1 HDL value ≥10mg/dl .
Trend of HDL correlated with clinical outcome of patients. Raising trend favors improvement in clinical
condition and decreasing trend implied worsening of the clinical condition.
SH Lee et al (2015) (3) concluded from his study that Non-survivors showed significantly higher
SOFA scores than survivors .Non-survivors had low levels of HDL, on days 0, 1, 3, and 7.
Shor R et al (2008) retrospective analysis of 204 patients showed low HDL (≤20 mg/dl) was
associated with a 17.5-fold increase in odds for death compared to HDL ≥ 65 mg/dl

density Lipoproteins–Cholesterol (HDL-C-) level in Sepsis patients. To
further correlate with prognostic outcome of patients with abnormal
HDL-C levels.
• Objectives:
• To measure HDL-C values in all patients in sepsis.
• To assess and compare the prognosis and response to treatment
in correlation with values of HDL-C.
5. INCLUSION CRITERIA
Patients with age greater than 18 years and satisfying the criteria
for Sepsis.
EXCLUSION CRITERIA
• Patients on treatment with statins
• Patients with Chronic liver disease, chronic kidney disease, thyroid
dysfunction, diabetes.
• Patients with known chronic inflammatory conditions like Human
immunodeficiency virus disease, SLE (Systemic lupus erythematous)
and RA (Rheumatoid arthritis)
• Patients diagnosed to have malabsorption disorders.
6. METHODOLOGY
• This study is being conducted in MGMC&RI.
• The study period is from November 2016 to march 2018.
• Those patients diagnosed to be in Sepsis will be the study participants.
• Whenever patient is diagnosed to be in sepsis, those patients will be
contacted immediately.
• Details of the study will be explained in detail. Permission is sought in the
form of written consent and the study is conducted.
• After ethical committee approval all the patients diagnosed to have sepsis in
High dependency units/wards of Mahatma Gandhi Medical College and
research institute will be recruited for the study.
• Blood samples from each patient will be taken for HDL-C-cholesterol levels,
creatinine, bilirubin levels, and platelet levels at the time of admission and on
the day 5 after admission and SOFA scores will be calculated accordingly on
day of admission and day 5 and assessed accordingly.

After going through the past records I selected the number of patients to be included in the study would be
100.
