Evidence-Based Practice
This randomized controlled trial evaluated the effectiveness of a tailored web- and text-based smoking cessation program called iQuit in Practice compared to usual care among smokers in primary care. The intervention led to significantly higher quit rates at 4 weeks compared to usual care. However, the differences in quit rates were not sustained at 6 months.
The study used a randomized controlled trial design, which is considered the gold standard for evaluating interventions. As a Level I evidence on the hierarchy, this provides the highest level of evidence that the iQuit in Practice program leads to short-

Week 3: Epidemiology, the Basis for Public Health
The study of epidemiology includes the examination of infectious disease, mental health and health-related events such as accidents or violence, and occupational and environmental exposure and their effects, as well as the examination of positive health states (Stanhope & Lancaster, 2016). Additionally, the study of epidemiology includes research into the morbidity and mortality of chronic illnesses that are found in the United States and across the globe. Infectious diseases like polio, TB, measles, and malaria have been almost eliminated in the United States; however, these diseases continue in other parts of the world for a variety of reasons. Highly contagious infectious diseases can and do lead to death despite the fact that many are preventable. The United States has used childhood vaccinations to prevent and eradicate illnesses such as smallpox and polio. In order to reduce the incidence of malaria in the United States, DDT—a known carcinogen in humans that is also toxic to birds—and other pesticides were used in the past and have since been replaced with nontoxic insecticides and larvicides to reduce the numbers of mosquitoes.
Unfortunately, developing countries often do not have the resources to engage in the same sorts of prevention programs and may have to resort to unsafe or toxic means to control vectors. They may not have the financial resources or health care personnel available to engage in mass vaccination campaigns. In addition, countries that experience frequent wars, acts of terrorism, and political instability face additional challenges to the provision of health care to their populations, especially when faced with an outbreak of an infectious disease.
This week, you will focus on the epidemiology of infectious and communicable disease and how nursing practices change in response to threats and outbreaks.

A.1 Healthcare problem
Worldwide estimates have shown that greater than 1.4 million patients have acquired nosocomial infections. Adherence to hand hygiene policies are shown to be the most effective way to help prevent these healthcare-associated infections; sadly research shows that healthcare workers have suboptimal compliance with their facilities hand hygiene policies due to lack of education and compliance monitoring. Patients in our healthcare settings are under the assumption that we are doing our best to promote their healing when in fact 7% of them will be subjected to a nosocomial infection with that rate climbing to 10% in developing countries (Finco et al., 2018).
A.2 Significance of the problem
The cost of care that is associated with nosocomial infections is estimated to be over ten billion dollars putting a burden on both patients and health organizations alike. It is estimated that 38% of all infections are caused by cross-contamination due to noncompliance with hand hygiene policies. These infections lead to approximately 99,000 deaths a year in the United States alone (Sickbert-Bennett et al., 2016).
A.3 Current healthcare practices related to the problem
Most healthcare facilities have an educational program that simply teaches how to achieve proper hand hygiene and use the WHO five moments of hand hygiene as their standard. However, this does not educate the healthcare workers on why it is important, nor does it address the far-reaching consequences for noncompliance. Along with the lack of foundational education, most facilities do not monitor for compliance.
A.4 How the problem affects the organization and patients’ cultural background
Inadequate hand hygiene leading to nosocomial infections can affect the organization's cultural background by leading to dissatisfaction in the workplace as staff becomes frustrated by their feelings of inadequacy and helplessness in dealing with patients getting sicker instead of better. The staff may also be feeling stress in the burden of caring for sicker patients. The patient's cultural background may be affected as they may be feeling despair or depression at their inability to get better, and some may feel it is punishment according to their cultural or religious beliefs.
B. Two research evidence sources and two non-research evidence sources considered
In searching for my research evidence sources, I start with the Western Governors University Library online. Once in the library, a boolean phrase was used, which allowed me to search for research articles that contain more than one topic in the same paper. Phrases I used in this search were “nosocomial infections,” “hand hygiene compliance,” and “ hand hygiene education.” With these phrases, thousands of articles were available to peruse.

Avoiding Epidemiologic Traps

Ecologic Fallacy
The ecologic fallacy, simply stated, is the error made when one makes incorrect inferences about an individual or small group’s probability of having a certain characteristic, based on the probability of that characteristic in the population from which that individual or small group comes.
Let’s look at an example:

Let’s say that we know that in a University with lower and upper campuses, that because upper campus houses graduate and honors programs, that the average GPA of students on upper campus is higher.
Does it follow that in a particular introductory psychology class offered to students from across the campus, if the professor tabulates the GPA of students enrolling in that class, that the ones from upper campus will have a higher GPA than those from lower campus?

Ecologic Fallacy
Not necessarily!
The mistake the psychology professor would make with such an assumption would be to substitute risk at the population level for risk at the individual level. The population-level difference in GPA, in this case, is attributable to the influence of students who probably are not going to take introductory psychology.
Confounding
Consider the following: 7 of 110 women, and 17 of 100 men, are positive for a new viral antibody “N” on when screened with a blood test. The odds ratio is 3.01, easily significant with a chi-square test.
However … look what happens when the data are analyzed by who has or has not ever lived outside the U.S.:

Confounding
Among those having lived outside the U.S., 15/50 men and 3/10 women (both 30%) have the N antibody. Among those never having lived outside the U.S., 2/50 men and 4/100 women have N antibody (both 4%).
The odds ratio in each group is 1.00 – there is actually no relationship at all between gender and having the N antibody. It only appeared so because of a statistical fluke.
Confounding
This is an example of what is known as confounding. The relationship between gender and the N antibody is confounded by whether the person has lived outside the U.S. Confounding occurs when the relationship between “A” and “C” is distorted (in either direction) by “B”, which is associated with “A”, and, independent of its relationship with A, is associated with “C”.

Bias
Confounding is an illustration of what is known as “bias” – that is, a situation where an unrelated factor obscures the true outcome.
Bias can come from a number of sources: four of the most common are recall bias, nonresponse bias, selection bias, and publication bias.
