Mumps
Mumps is a viral disease caused by the mumps virus. Initial
symptoms of mumps are non-specific and include fever,
headache, malaise, muscle pain, and loss of appetite. These
symptoms are usually followed by painful swelling of the
parotid glands, called parotitis, which is the most common
symptom of a mumps infection.
Definition
Etiology
• Mumps is caused by the mumps virus, which is an RNA virus,
Rubulavirus.
• Rubulavirus is within the genus Paramyxovirus and is a
member of the family Paramyxoviridae.
• Humans are the only natural host of the mumps virus.
Mode of Transmission
• The virus is primarily transmitted by respiratory secretions such as
droplets and saliva, as well as via direct contact with an infected person.
• Mumps is highly contagious and spreads easily in densely populated
settings.
• Transmission can occur from one week before the onset of symptoms
to eight days after.
Incubation Period
• The incubation period of mumps is 16-18 days; however, it can vary from 14-25 days.
Mostly children and young adults get infected with this virus.
Risk Factors
• Risk factors for mumps include immunodeficiency, international travel and
lack of vaccination.
Statistics
• Most cases reported in 2006 and 2009-2010 were associated with a few large,
localized outbreaks. However, since 2014, more than 1,000 mumps cases have been
reported each year, and in 2019 nearly every state reported mumps cases.
Pathophysiology
Respiratory Transmission of Virus
Replication in nasopharynx and regional lymph nodes
Viremia 12-25 days after exposure with spread to other tissues
Multiple tissues infected during viremia
Mumps
Clinical
Manifestations
• Is a highly infectious child-hood disease
• Incubation period usually 16-18 days (range, 12-25 days)
• Mumps starts with moderate fever, malaise, pain on chewing or swallowing,
particularly acidic liquid.
• Followed by the inflammation of salivary glands, particularly the parotid
glands.
• The swelling appears in front of the ear.
• Orchitis, oophoritis, mastitis, pancreatitis, hearing loss, meningitis, and
encephalitis
• More common among adults than children
• Less likely in vaccinated persons compared to unvaccinated persons
• Meningitis, encephalitis, pancreatitis, and hearing loss 1% or less among
infected persons in the postvaccine era
Complications
Therapeutic Interventions
(Medical and Pharmacological Management)
There's no specific medical treatment for mumps.
Treatment is supportive by treating symptoms,
using antipyretics and analgesics.
Nursing Management/Intervention
• The patient must rest in bed until the fever goes away.
• Isolate the patient, to prevent spreading the disease to others.
• Use antipyretics and analgesics to ease symptoms.
• Avoid giving food that require chewing.
• Avoid giving sour foods that stimulates saliva production.
• Encourage patient to drink plenty of water.
• Use cold compress to ease the swelling glands.
Prevention
• Vaccination is the best way to prevent mumps and mumps complications.
• This vaccine is included in the combination measles-mumps-rubella (MMR)
and measles-mumps-rubella-varicella (MMRV) vaccines.
