Comment 1 Community health nurses can apply the strategies of cultural competence by being culturally aware. Nurses should start by assessing the patient to determine the person’s knowledge and beliefs before providing care. This will give the nurse insight as to how to proceed with education and treatment. An example of a nurse practicing cultural preservation would to acknowledge the use of traditional healers within the Hispanic community. Latinos term illnesses as either “hot” or “cold” and treat such illnesses with herbs or other measures, such as massage on the affected area or steam baths. The treatment of such illnesses is termed cultural accommodation and should be encouraged if these forms of treatment have been proven harmless. An example of cultural repatterning would be for the nurse to work with the patient to change a person’s behavior(s) that would be harmful to him/her. Cultural brokering is recognized as connecting differing cultural backgrounds so that a change in behavior can be made to produce a successful outcome. Barriers to applying any of these strategies is a lack of understanding on the nurse’s part and recognizing that family plays a major role in influencing behavior. For example, research shows that Latino adolescents that battle substance abuse are more successful with their treatment and have better outcomes with family support. This is due to, in part, because Hispanic families rely on the entire family (aunts, uncles, cousins, grandparents, etc.) as a unified support system (Burrow-Sanchez). However, some barriers to Latinos obtaining treatment could be a lack of access to needed healthcare/treatment, language barriers, etc. Therefore, it is very important for nurses to be culturally aware and practice patience and understanding while maintaining respect when it comes to providing care for people that come from different cultures. As an Advanced Practice Registered Nurse (APRN) who plans to reside in such a culturally diverse city such as Miami, Fl it is extremely important that we are not just familiarized and conscientious of our own values, beliefs, culture, and self-concept, but that we are aware cognizant of others’ cultures, beliefs and values and how they could potentially impact not just the patient’s overall health, but the interventions we plan to implement to improve their health. As the percentage of patients with type 2 diabetes and the epidemic of obesity on the continual rise in Hispanics and African Americans, it is imperative that we as providers understand the factors contributing to these alarming numbers. Clinical trials have demonstrated that understanding the factors contributing to these health concerns and providing culturally tailored interventions can be efficient and effective (Joo & Liu, 2021).
Research conducted demonstrated the positive impact of several culturally tailored interventions on type 2 diabetes and these included open discussions of cultural beliefs about diabetes and their treatments, employing use of their native language, integrating cultural dietary preferences, and encouraging family participation and support (Joo & Liu, 2021). With respect to obesity, research has been conducted to examine culturally influenced interventions geared towards family genetics, behaviors, and the environment by using e-health as an opportunity to deliver interactive, culturally diverse, tailored information, however this was found to be largely dependent on literacy levels (Gustafson et al., 2010). Introduction
Nursing is not all about giving medications or treating patients. It is the responsibility of the nurses to educate patients on how to prevent illnesses and how to manage certain medical conditions. Nurses can do these by interacting and communicating with patients. By doing this, they will help patients understand how to take control of their health care. When patients take part in their health care, they are likely to change their behaviors and do things that are likely to improve their general health.
My role and the environment I will utilize for teaching
According to Burke and Mancuso (2012), learning is very important in any nursing environment. Effective education of patients happens from the time they are admitted at the hospital and goes on until the patients are discharged from the hospital. For out-patients, I will educate them during their waiting time. As a nurse I will take every opportunity I will come across during the patients’ visit to the hospital and throughout their admission in the hospital to educate them about their health care. I will provide patients with instructions to follow on self-care and how to maintain certain problems. Some of the self-care instructions include;
· How to follow the steps of self-care
· How to know early signs of certain illnesses
· How to go about emergency problems
· Who to contact in case of problems
The intended audience
I will educate people of all populations in my education program regardless of their age, culture, illness, ethnicity, and gender. General education will be provided to all patients on how to take care of themselves when they leave the hospital. This important because sometimes patients go home, neglect themselves, resume their unhealthy practices, and forget to manage their medical conditions. For patients suffering from diabetes, I will educate and provide them with instructions on how to inject themselves with insulin. For new mothers, they will learn how to take care of their new born babies and how to bath the infants. I will provide instructions on how to change a colostomy pouching system for the concerned patients.

It is agreed widely in the health care field that an individual's culture is a critical factor to be considered in treatment. The Surgeon General's report, Mental Health: Culture, Race, and Ethnicity, states, "Substantive data from consumer and family self-reports, ethnic match, and ethnic-specific services outcome studies suggest that tailoring services to the specific needs of these [ethnic] groups will improve utilization and outcomes” (U.S. Department of Health and Human Services 2001, p. 36). The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) (American Psychiatric Association 1994) calls on clinicians to understand how their relationship with the client is affected by cultural differences and sets up a framework for reviewing the effects of culture on each client.
Because verbal communication and the therapeutic alliance are distinguishing features of treatment for both substance use and mental disorders, the issue of culture is significant for treatment in both fields. The therapeutic alliance should be informed by the clinician's understanding of the client's cultural identity, social supports, self-esteem, and reluctance about treatment resulting from social stigma. This paper is therefore, written with the objective of analyzing the effect of various aspects, for instance, increased employee turnover rates, on the organization as well as their contribution to the increasing rates of employees opting to desert their jobs.

My role and the environment I will utilize for teaching
According to Burke and Mancuso (2012), learning is very important in any nursing environment. Effective education of patients happens from the time they are admitted at the hospital and goes on until the patients are discharged from the hospital. For out-patients, I will educate them during their waiting time. As a nurse I will take every opportunity I will come across during the patients’ visit to the hospital and throughout their admission in the hospital to educate them about their health care. I will provide patients with instructions to follow on self-care and how to maintain certain problems. Some of the self-care instructions include;

I will educate people of all populations in my education program regardless of their age, culture, illness, ethnicity, and gender. General education will be provided to all patients on how to take care of themselves when they leave the hospital. This important because sometimes patients go home, neglect themselves, resume their unhealthy practices, and forget to manage their medical conditions. For patients suffering from diabetes, I will educate and provide them with instructions on how to inject themselves with insulin. For new mothers, they will learn how to take care of their new born babies and how to bath the infants. I will provide instructions on how to change a colostomy pouching system for the concerned patients.

2. What It Means To Be a Culturally Competent Clinician
It is agreed widely in the health care field that an individual's culture is a critical factor to be considered in treatment. The Surgeon General's report, Mental Health: Culture, Race, and Ethnicity, states, "Substantive data from consumer and family self-reports, ethnic match, and ethnic-specific services outcome studies suggest that tailoring services to the specific needs of these [ethnic] groups will improve utilization and outcomes” (U.S. Department of Health and Human Services 2001, p. 36). The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) (American Psychiatric Association 1994) calls on clinicians to understand how their relationship with the client is affected by cultural differences and sets up a framework for reviewing the effects of culture on each client.
Because verbal communication and the therapeutic alliance are distinguishing features of treatment for both substance use and mental disorders, the issue of culture is significant for treatment in both fields. The therapeutic alliance should be informed by the clinician's understanding of the client's cultural identity, social supports, self-esteem, and reluctance about treatment resulting from social stigma. A common theme in culturally competent care is that the treatment provider--not the person seeking treatment--is responsible for ensuring that treatment is effective for diverse clients.

When the living will was discussed with him, he insisted that the patient had not intended for the document to be used in a situation like the present one. Further discussion with him revealed that he understood that the patient would not be able to recover any meaningful brain function but he argued that the living will did not apply because her condition was not imminently terminal. He further indicated that he did not consider his wife to be in a permanent unconscious condition. The immediate family members (the couple’s adult children) disagreed with their father’s refusal to withdraw life support.

You will be analyzing two short stories, “Battle Royal” (which is the first chapter in Ralph Ellison’s novel,
Invisible Man
) and “The Birthmark.” As you read, reflect on the ways each depicts characters that are deemed socially unacceptable because of their outward appearances.
Please write a
comparison/contrast essay of 1000 words
or more discussing the questions below. Remember to begin your paper with an engaging introduction and clear thesis statement, develop each point in the body of your paper using examples and quotes from the stories, and conclude your paper with a restatement of your thesis and closing remarks. Also, be sure to maintain your credibility by including in-text citations and a reference list correctly formatted in APA style.
Setting: Describe the settings of both pieces and identify how the eras in which they take place—with their distinct societal attitudes and customs—affect the main characters?
Characters:
“Battle Royal:” Discuss the young man and his grandfather. Why do we never learn the young man’s name? What do the grandfather’s dying words reveal about him?
“The Birthmark:” Describe the main characters: Aylmer, Georgiana and Aminadab. What is important to each?
Point of View: In which point of view (first- or third-person) is each piece told? If the point of view in “Battle Royal” was changed, would it have made the story more effective, or less so?
Symbolism:
“Battle Royal:” Analyze the deeper meaning of the following: the “battle royal” itself, the naked blonde, and the young man’s dream at the end of the story.
“The Birthmark:” What does Georgiana’s birthmark signify, first to her and then to Aylmer? What does alchemy represent in the story?
Themes: What are the main themes/messages of each piece? What, in other words, do you think the authors, Ralph Ellison and Nathaniel Hawthorne, are trying to communicate about life and human nature in their respective stories?
Fear: What role does fear play in both pieces?
Discrimination: Both stories address physical appearance, specifically one’s skin, and the way people may discriminate against others because of external characteristics they deem inferior. Compare and contrast how discrimination and prejudice are presented in “Battle Royal” and “The Birthmark.”
Final Thoughts: Author Tim O’Brien wrote, "That's what fiction is for. It's for getting at the truth when the truth isn't sufficient for the truth." Talk about how literature might give us “truer” insights into the human experience by appealing to our senses, emotions and empathy. Describe a situation in which you or someone you know may have been discriminated against because of appearance, gender, race or another attribute. What did the experience teach you?

“Minority Report Steven Spielberg, Director (2002)
Minority Report is set in the year 2054, and John Anderton (Tom Cruise) is chief of the Department of Pre-Crime in Washington, D.C. There has not been a murder there in 6 years, because of an operation Anderton supervises where three pre-cognitive humans (“pre-cogs”) drift in a flotation tank and have their brain waves tapped by computers. These brain waves can pick up thoughts of premeditated murders, so the police can be warned and arrest the would-be perpetrators before they can kill (hence the “Department of Pre-Crime”). Of course, it is not as easy as this, because the pre-cogs can provide only the time and date of the murder, the murderer’s name, and the victim’s name—the other facts can be learned only by clues derived from the various images generated by the pre-cogs around the time of murder. The film is based on a short story by Philip K. Dick.
The pre-crime police strategy is going to expand nationwide, and there is personal and bureaucratic jealousy toward Anderton and the pre-crime unit. The movie focuses on a rare disagreement among the “pre-cogs” over a pending murder, and one of the pre- cogs apparently files a minority report disagreeing with the others. Normally, the minority report is disregarded, but the twist is that Anderton himself becomes a target for arrest as a predicted killer, and he is apparently warned of this by one of the pre- cogs. Anderton then is chased by police, assisted by one of the pre-cogs named Agatha (Samatha Morton), and the futuristic police pursuit and search is interesting and compelling. For example, everyone is subject to retina scans, so Anderton has to get an eye transplant to hide his identity while being pursued. The story has several twists, and
it raises important issues about the certainty of knowledge and how knowledge should be used.
QUESTIONS
1. If we were able to predict premeditated murders in the future by somehow scanning the minds of citizens, is this enough evidence to apprehend and take legal action against them? What would the major ethical perspectives say?
2. The accurate prediction of criminal behavior is not yet possible, but profiles are sometimes used by police for targeting suspects. Under what circumstances is the use of a profile to take police action morally permissible?

- 1. Comment 1 Community health nurses can apply the strategies of cultural competence by being culturally aware. Nurses should start by assessing the patient to determine the person’s knowledge and beliefs before providing care. This will give the nurse insight as to how to proceed with education and treatment. An example of a nurse practicing cultural preservation would to acknowledge the use of traditional healers within the Hispanic community. Latinos term illnesses as either “hot” or “cold” and treat such illnesses with herbs or other measures, such as massage on the affected area or steam baths. The treatment of such illnesses is termed cultural accommodation and should be encouraged if these forms of treatment have been proven harmless. An example of cultural repatterning would be for the nurse to work with the patient to change a person’s behavior(s) that would be harmful to him/her. Cultural brokering is recognized as connecting differing cultural backgrounds so that a change in behavior can be made to produce a successful outcome. Barriers to applying any of these strategies is a lack of understanding on the nurse’s part and recognizing that family plays a major role in influencing behavior. For example, research shows that Latino adolescents that battle substance abuse are more successful with their treatment and have better outcomes with family support. This is due to, in part, because Hispanic families rely on the entire family (aunts, uncles, cousins, grandparents, etc.) as a unified support system (Burrow-Sanchez). However, some barriers to Latinos obtaining treatment could be a lack of access to needed healthcare/treatment, language barriers, etc. Therefore, it is very important for nurses to be culturally aware and practice patience and understanding while maintaining respect when it comes to providing care for people

Comment 2 A community health nurse has a great challenge in applying cultural competence because they see and treat such a variety of culturally different people. It is key for the community health nurse to assess their own beliefs and be aware that they may greatly vary from the people in their care. Being open minded and setting aside personal biases while implementing the four strategies of cultural competence is needed to achieve success (Maurer & Smith, 2013). Midwives work in facilities, but also work in the community as well assisting with birthing in homes. Cultural preservation can be seen by only using a female midwife in the home birth of a Muslim baby as well as respecting that the father will most likely not be present for the birth. Cultural accommodation example would be to allow a mother soon after giving birth to eat a ratb, which is a type of date. This is thought to help promote excellent breast milk for the baby. Cultural repatterning would be discussing that the tradition of placing khakhe shafaa, a type of mud, in the infant’s mouth after birth as unhealthy. Cultural brokering involves another person of the persons culture to help negotiate between the community health nurse and the patient. In the case of the khakhe shafaa, there is an additional practice of using Euphrates water in the baby’s mouth. Using someone based in the same culture will increase the trust level and possible help repattern the behavior that is unhealthy for the infant (Ahlul Bayt Digital Islamic Library Project [Islamic Library], 2018). Another example of cultural accommodation would be to show support of fasting during Ramadan. Unfortunately, if the
- 3. Muslim client is diabetic and being seen in a community setting, the nurse may recommend monitoring blood glucose levels and “breaking the fast” if the blood sugar levels were to drop too low. This may create a barrier for applying this strategy (Rassool, 2015).
