Discussion: Cultural Considerations in Nursing
Many factors influence maternal health. In many countries, an unacceptable number of women die in childbirth or shortly after giving birth. As discussed by Dr. Leslie Mancuso in this week’s first media presentation, culture and religious beliefs influence childbirth practices.
To prepare for this Discussion:
Review Case 6, “Saving Mothers’ Lives in Sri Lanka.”
Review Dr. Mancuso’s comments on cultural beliefs that influence maternal health.
In addition, consider the following questions:
What cultural and historical features of Sri Lanka contributed to the success of the country’s maternal health program?
What are some assumptions that a person might make about you based on your appearance or cultural background? All cultural diversity is not about ethnicity, religious affiliation, or traditional backgrounds. When you consider what cultural issues affect you and/or your community, think about whether you represent diversity of another kind, such as; rural versus urban, east coast versus west coast, employed in a small hospital versus a large research center, or providing nursing practice to young versus elderly, acute care versus clinic care or mental health care. Each of these "types" of diversity contribute to you as a person and how you practice nursing.
How might that affect the nursing care you receive?
What cultural information would you want a nurse or doctor to know about you?
What would you want a health promotion program to include that addresses a health concern for your cultural or ethnic community?
.
Identify any genetic predisposition your chosen population ( Hispanifideladallimore
Identify any genetic predisposition your chosen population ( Hispanics) has to a particular disease ( Diabetes) and develop primary practice interventions that reflect the cultural considerations of the population.
Then, develop culturally appropriate, measurable interventions to help your population members maintain an optimal state of health, avoiding the problem that you identified them being at risk for developing.
Culturally aware nurses recognize that states of health are revealed differently across cultures and ethnicities. Culture and ethnic background will affect the way each individual responds to health, illness, and death (Stanhope & Lancaster, 2016). These nurses are also aware of their own biases, which may affect the care they provide to others (Stanhope & Lancaster, 2016). Because most nurses work in institutions with individual patients, they are accustomed to delivering culturally competent care on a one-on-one basis. When a public health nurse deals with a population, he or she must consider how the population culture affects the ways in which the community nurse may interact. This can be with regard to the provision of education or mass health care needs such as those required in a foodborne illness, if mass vaccinations are needed for a communicable disease outbreak, or if education is required to prevent heart disease. In addition to understanding the nuances of the culture of a population, community health nurses must understand the role genetics play in health. Some disorders, such as glaucoma and diabetes, have a genetic link, as do some cancers, such as breast and ovarian.
Please discuss the following questions in no less than 4-5 paragraphs
· Provide a few examples of community resources that should be put in place to assist your population in resolving their health care needs. What gaps in service do you see that affect your population?
· Are there any cultural considerations that might inform your approach to caring for this population?
· Does your population have a genetic predisposition to the health care problem you have identified?
· Identify at least one evidence-based, culturally competent behavior change that would promote health for your selected population and for the specific health care problem you are addressing?

Learning to adapt to different culture is a difficult task that each person has to give a try. As for John, this was the only way he could have survived to live in this region. Therefore, this was a choice he could have made earlier or later in life and there was no way out of it. The most interesting of all was being asked to be shaved at the age of thirteen to symbolize that he was about to be recognized by the society as a fully grown man. In addition, understanding different cultures was to his advantage because he was now capable of interacting with different cultures(Castillo, 2006).

Patients and their families are given a multitude of information about their health and commonly must make important decisions from these facts. Obstacles that prevent easy delivery of health care information include literacy, culture, language, and physiological barriers. It is up to the nurse to assess and evaluate the patient's learning needs and readiness to learn because everyone learns differently.

Comment 1
Community health nurses can apply the strategies of cultural competence by being culturally aware. Nurses should start by assessing the patient to determine the person’s knowledge and beliefs before providing care. This will give the nurse insight as to how to proceed with education and treatment.
An example of a nurse practicing cultural preservation would to acknowledge the use of traditional healers within the Hispanic community. Latinos term illnesses as either “hot” or “cold” and treat such illnesses with herbs or other measures, such as massage on the affected area or steam baths. The treatment of such illnesses is termed cultural accommodation and should be encouraged if these forms of treatment have been proven harmless. An example of cultural repatterning would be for the nurse to work with the patient to change a person’s behavior(s) that would be harmful to him/her. Cultural brokering is recognized as connecting differing cultural backgrounds so that a change in behavior can be made to produce a successful outcome. Barriers to applying any of these strategies is a lack of understanding on the nurse’s part and recognizing that family plays a major role in influencing behavior. For example, research shows that Latino adolescents that battle substance abuse are more successful with their treatment and have better outcomes with family support. This is due to, in part, because Hispanic families rely on the entire family (aunts, uncles, cousins, grandparents, etc.) as a unified support system (Burrow-Sanchez). However, some barriers to Latinos obtaining treatment could be a lack of access to needed healthcare/treatment, language barriers, etc.
Therefore, it is very important for nurses to be culturally aware and practice patience and understanding while maintaining respect when it comes to providing care for people that come from different cultures.
Comment 2
A community health nurse has a great challenge in applying cultural competence because they see and treat such a variety of culturally different people. It is key for the community health nurse to assess their own beliefs and be aware that they may greatly vary from the people in their care. Being open minded and setting aside personal biases while implementing the four strategies of cultural competence is needed to achieve success (Maurer & Smith, 2013).
Midwives work in facilities, but also work in the community as well assisting with birthing in homes. Cultural preservation can be seen by only using a female midwife in the home birth of a Muslim baby as well as respecting that the father will most likely not be present for the birth. Cultural accommodation example would be to allow a mother soon after giving birth to eat a ratb, which is a type of date. This is thought to help promote excellent breast milk for the baby. Cultural repatterning would be discussing that the tradition of placing khakhe shafaa, a type of mud, in the infant’s mouth after birt ...

It is essential to note that we actually live in a diverse nation and overcoming the shortcomings associated with cultural preferences and beliefs is a big obstacle for healthcare professionals today. In the article “Health Disparity and Cultural Conflict” cultural diversity is singled out to be more than one race. It is important for healthcare workers to know that addressing cultural diversity is far more than just understanding the values, practices, customs, and beliefs (Hughes, 2011). Apart from national origin and racial classification, there are various kinds of cultural diversity and these include language, age, religious affiliation, and geographical location.
Healthcare providers are expected to diligently accommodate the varying needs of the individuals they meet in their service provision. The needs actually range from the different cultural backgrounds to the learning styles and preferences as well as the mixed opinions that define their health and well-being (Lochman, 2012). The issue of language barrier is a big problem to many healthcare providers.
Nurses are normally exposed to various beliefs and cultures as they work in their healthcare fields. Apart from diversity, it is important
to consider the level of education plus the style of learning. This will enable the nurse to formulate the best teaching method relevant for the patient. Patients with various cultural differences can sometimes not be willing to take part in education or take in the teachings. In order to initiate any formal kind of teaching, trust must be developed (Campinha, 2003). Nurses should assess the values and ideas related to the illness and health of the patients, treatment provided, medication being taken, and the expectations to ensure effective teaching or learning.
Healthcare professional should pay very close attention to the cultural needs of their patients including expression and language. For example, a Chinese patient who has been disregarding signs unusual signs and symptoms of rectal bleeding for a long time. He has come to the hospital and diagnosed with colon cancer. The early treatment process however needs colon resection. The treatment process is discussed with the patient and risk also explained, particularly blood loss. In the process of surgery, the patient loses a lot of blood and the level of haemoglobin reduces below the normal range. This required immediate blood transfusion. However, the culture of the patient did not accept blood transfusion. The transfusion process had to be delayed since the patient refused. Such kind of cultural affiliations can affect the manner which care delivery is done and providers must be aware of them and teach the patients. Teaching should actually be a multidisciplinary team work which includes social work, dietician, and therapy department. Nurses should also be aware of their own values and beliefs before interacting with the various different culture.
