performed by the Sean Curran Company and the Ustastshkirt Plus musical ensemble.
It was a beautiful collaboration between contemporary American dance and ancient

more so. The program was a creation of Sean Curran, a choreographer from NYC. It
was inspired by his travels Kazakhstan and the music he heard there. He came across
a small six member ensemble called Ustastshkirt Plus, meaning master apprentice ,
from the Tien Shan Mountains of the Kyrgyz Republic, whose music gave him the

I am authorized to represent the Fulton County Self Insurance Plan in the above
referenced matter. I am writing at this time in supplement to an appeal filed by
Benetech in the matter on 03/30/17. Both Benetech and the undersigned are
authorized to represent Fulton Country Self Insurance Plan in this matter. I have
enclosed letter of authorization herewith.
By way of background, Fulton County is responsible for a New York State
Workers Compensation claim, WCB# 50413999 with a date of accident of 10/01
/2003. The case has been established for an injury to the neck by Administrative

Benetech has reviewed each and every one of these treatment codes. Code M419 is
for scoliosis unspecified. Codes Z7982, Z79899, I10, Z7902 and Z981 are
unidentified codes. Code I2510 is for atherosclerotic heart disease. Code M4806 is
for spinal stenosis of the lumbar region.
The codes M419 scoliosis unspecified is not related to the claimant s established neck
condition, nor is it related to carpal tunnel syndrome or cubital tunnel syndrome. The
codes I2510 is for atherosclerotic heart disease, which is likewise in no way related
to any of the claimant s established injuries.
The codes M4806 which is for spinal stenosis of the lumbar region is not related to
the claimant s established case for the neck, right carpal tunnel syndrome and right
cubital tunnel syndrome.
The remaining billing codes cannot be identified and there is no indication they are
related to the established sites.
Fulton County and Benetech acknowledge Medicare s right to be reimbursed for
any conditional payments made, which should have been the responsibility of the
New York State Workers Compensation carrier, the primary carrier for any treatment
associated with the clai ant s 10/01/2003 injury. However, none of the billing codes,
which you forwarded to us, correspond to any of the claimant s established conditions
and we can only conclude they are not related to the 10/01/2003 injury.
