A 35-year-old woman was admitted to the hospital with recurrent melena after treatment for a gastric ulcer positive for Helicobacter pylori. She received multiple courses of treatment over 17 months to eradicate H. pylori, but her symptoms returned each time treatment ended. A fiberoptic endoscopy showed a large ulcer on the stomach wall covered by a clot, and a white submucosal plaque was seen opposite the ulcer. The patient's condition was worsening with ongoing bleeding.
