An 87-year-old man presented with progressive bilateral leg edema for two days. He had a history of pulmonary TB and osteoporosis. On examination, he appeared pale and weak with coarse breathing sounds and pitting edema of both legs. Chest X-ray and EKG were performed along with blood tests which showed microcytic anemia. The doctor's impressions included anemia, congestive heart failure, osteoporosis and malnutrition. Plans were made for further diagnostic tests and treatment including blood transfusion and iron supplementation while stabilizing the patient's condition.
