A 62-year-old male patient presented with difficulty breathing, frequent nighttime urination, chest pain, and tightness. Examinations found high blood pressure, low hemoglobin, and increased blood urea nitrogen. Tests showed the patient had heart failure due to reduced left ventricular function and vascular insufficiency from longstanding hypertension. The patient was diagnosed with heart failure and hypertension. Treatment included diuretics, beta blockers, and digoxin to improve heart function as well as lifestyle counseling regarding weight, fluid, and sodium management.
