All Student Services personnel (PSPs) are required to submit the following information as
their ANNUAL REPORT for the 2011/12 school year.

   A. Caseload/Referral Information
   B. Activities Related to Professional Duties and Responsibilities
   C. Reporting and Documentation

   A. Caseload/Referral Information
      Please submit caseload/referral information on each of the schools you serve. Other
      relevant caseload information may also be submitted. Possible break down of caseload
      vs. work load could also be included.

   B. Activities Related to Professional Duties and Responsibilities
      Please provide a list of specific activities related to your professional duties and
      responsibilities that you were involved with during the 2011/12 school year. This may
      include things such as professional development, workshops, seminars or other PD
      activities that you conducted/attended, class presentations, PPP planning meetings,
      contact with parents, consultation and collaboration with other team members,
      participation on committees, other contributions to the school/division. Please pay
      particular attention to our three C’s – Collaboration, Capacity Building and
      Collaboration. How did you contribute to these 3 C’s this year? What was your
      contribution to your PLC? Did you move closer to consistency of practice? If not, why
      not?

   Please submit one piece of evidence with a brief explanation related to one of the
   items listed below. This can include things such as a copy of an agenda or handout form
   a class presentation, workshop , a brochure or information provided to schools or
   parents, certification or recognition earned, etc…. Preferably, you will link this item to
   the 3 C’s.

C. Reporting and Documentation
   Please submit one of the following: a sample report, a copy of your case management
   notes, an example of your data/record keeping, an intervention plan, a joint report
   developed with another PSP.


   Please submit your completed ANNUAL REPORT information along with your Annual
   Growth Plan to your area Student Services Co-ordinator on or before June 18th, 2012.

A. Caseload/Referral Information B. Activities Related to Professional Duties and Responsibilities C. Reporting and Documentation A. Caseload/Referral Information Please submit caseload/referral information on each of the schools you serve. Other relevant caseload information may also be submitted. Possible break down of caseload vs. work load could also be included. B. Activities Related to Professional Duties and Responsibilities Please provide a list of specific activities related to your professional duties and responsibilities that you were involved with during the 2011/12 school year. This may include things such as professional development, workshops, seminars or other PD activities that you conducted/attended, class presentations, PPP planning meetings, contact with parents, consultation and collaboration with other team members, participation on committees, other contributions to the school/division. Please pay particular attention to our three C’s – Collaboration, Capacity Building and Collaboration. How did you contribute to these 3 C’s this year? What was your contribution to your PLC? Did you move closer to consistency of practice? If not, why not?

This can include things such as a copy of an agenda or handout form a class presentation, workshop , a brochure or information provided to schools or parents, certification or recognition earned, etc…. Preferably, you will link this item to the 3 C’s. C. Reporting and Documentation Please submit one of the following: a sample report, a copy of your case management notes, an example of your data/record keeping, an intervention plan, a joint report developed with another PSP. Please submit your completed ANNUAL REPORT information along with your Annual Growth Plan to your area Student Services Co-ordinator on or before June 18th, 2012.
