What is Patient- Centered Care?
Fundamentally different from traditional care delivery
Based on value, not necessarily volume
A care team is responsible for providing all the patient's health care needs
Emphasis is placed on increased accessibility: •open scheduling •expanded hours •strong communication
Patient-Centered Care Delivery Models
Patient-Centered Medical Home (PCMH)
Accountable Care Organization (ACO)
Other care delivery models involving key tenets of patient-centered care
How Does Policy Support Patient- Centered Care?
The Affordable Care Act (ACA) mentions patient- centeredness 36 times.
Meaningful Use Stage 2: 7 of 23 measures are directly grounded in patient- centered care.
Accountable Care Organizations (ACOs): 7 of 33 performance measures directly relate to patient engagement.
CMS Public Measure Transparency: Introduces the Physician Compare Website, which encourages quality and transparency.
CMS Value-based Purchasing: CMS' value- based modifier payment program will implement differential payment based on quality of care.
Other agency initiatives support key tenets of patient-centered care: CMS PCORI, HRSA PCMH and VA PACT
