A worker was seriously injured while using a grinder during a tank repair operation.
The draw-string from the hood of his jacket became entangled in the grinder, causing the grinder to be
pulled towards his head; he subsequently suffered serious lacerations on his temple and neck.
The first lesson learned from this incident is that there should be no drawstrings or loose items of
clothing, PPE, etc dangling when operating a grinder or other rotating equipment. Such items can
easily become entangled in the rotating mechanism. This would include items such as hair, scarf, ID badge,
jewellery, or any other article that could cause a person to get drawn into a machine. In some cases this
would include PPE such as lanyards, etc.
The second lesson learned from this incident:
• reinforces policy that prohibits grinders from having a locking trigger. Please insure that all grinders at
your projects and facilities have NON LOCKING triggers.
• update applicable JSAs to include this potential hazard.

500 hilang kerja kilang terbakar
KUALA LUMPUR: Lebih 500 pekerja tempatan dan asing hilang mata pencarian apabila kilang polisterin
pembungkus makanan tempat mereka bekerja musnah dalam kebakaran di Jalan 222, Kampung Jaya, di
Sungai Buloh dekat sini, malam tadi.
Kebakaran yang bermula kira-kira jam 8 malam itu dipercayai berpunca daripada bahagian stor
menyimpan pelbagai bahan kimia menyebabkan bahagian pengeluaran dan stor tempat alat pendingin
hawa musnah.
Kebakaran itu dipercayai menyebabkan kerugian lebih RM2 juta.Difahamkan, ini adalah kali kedua kilang
itu terbakar pada tahun ini, selepas kali pertama beberapa bulan lalu.
Bagaimanapun, tidak ada kemalangan jiwa atau kecederaan dilaporkan dalam kejadian berkenaan kerana
ketika itu kilang terbabit tidak beroperasi.
Katanya, empat jentera bomba dari Sungai Buloh, Damansara, Bu kit Jelutong dan Tentera Darat Kem
Sungai Buloh bersama 35 anggota bergegas ke tempat kejadian dan tiba pada jam 8.16 malam.
“Sebaik tiba kami dapati api sedang marak dengan 50 peratus daripada stor menyimpan bahan polisterin
dan stor alat pendingin hawa sudah terbakar.“Api merebak terlalu cepat kerana semua bahan yang ada
dalam kilang berkenaan mudah terbakar,” katanya ketika ditemui di tempat kejadian.

LIFT INCIDENT

KUALA LUMPUR: Kepala seorang jurutera berkecai apabila dia tergelincir dan jatuh dari tingkat 18
ke tingkat 11 ketika membaiki lif di bangunan Bank Negara Malaysia (BNM) di sini, semalam.
Mangsa, Chang Can Syang, 29, berasal dari Taiping, Perak dikatakan jatuh di tengah lubang lif
ketika membaikinya sebelum tersangkut di tingkat 11.
Ketua Polis Daerah Dang Wangi, Asisten Komisioner Kamal Pasha Jamal, berkata mangsa yang
cedera parah di kepala, meninggal dunia di tempat kejadian. Katanya, kejadian berlaku kira-kira
jam 11.30 pagi dan mangsa dipercayai tidak memakai kelengkapan keselamatan ketika membaiki
lif berkenaan. Ketika mangsa membaiki lif dalam bangunan berkenaan, tiba-tiba dia tergelincir dan
terjatuh ke tingkat 11 sebelum meninggal dunia.
Mayat mangsa kemudian dihantar ke Rumah Mayat Hospital Kuala Lumpur (HKL) untuk bedah
siasat, katanya di sini. Polis meminta orang ramai yang mempunyai maklumat tampil di balai polis
berhampiran memberi keterangan bagi membantu siasatan atau menghubungi talian Rakan Cop
03-21159999.
Sementara itu, jurucakap Bomba dan Penyelamat berkata pihaknya mengambil masa kira-kira
sejam untuk mengeluarkan mayat mangsa yang berada di tingkat 11 bangunan berkenaan.
Bomba terpaksa menggunakan tangga dan tali dalam operasi mengeluarkan mayat berkenaan.
Mayat mangsa dapat dikeluarkan jam 12.54 tengah hari dan diserahkan kepada polis untuk
tindakan selanjutnya,katanya.
