Renal Tumour Angiomyolipoma - Bizarre Presentation
A 28-year old female presented with fever, chills, and flank pain. Imaging showed a large liver lesion initially thought to be a tumor or abscess. Further CT and MRI revealed the lesion to be retroperitoneal and connected to the upper pole of the right kidney. At surgery, 1.5 liters of pus was drained. Histopathology of the renal mass found it to be an angiomyolipoma, a rare renal tumor. Literature review discussed diagnostic challenges and optimal imaging techniques for diagnosing renal angiomyolipomas.

CT urography (CTU) is highly accurate for detecting bladder cancer in high-risk patients. A retrospective study of 45 high-risk patients found that CTU had a 97% sensitivity and 80% specificity for bladder cancer detection compared to flexible cystoscopy. Multivariate analysis showed that positive CTU findings were highly significant predictors of bladder cancer diagnosis. The study suggests that in high-risk patients with positive CTU findings, flexible cystoscopy could be omitted and patients referred directly for rigid cystoscopy and potential biopsy or resection. MR imaging of liver
MRI is useful for evaluating various liver conditions. It is superior to CT for detecting small liver lesions and characterizing lesions. MRI can identify diffuse liver diseases affecting hepatocytes or reticuloendothelial cells, causing homogeneous or segmental changes. Cirrhosis appears as numerous low signal regenerative nodules on T2-weighted images. Hemangiomas are intensely hyperintense on T2-weighted images and enhance peripherally on contrast images. Dysplastic nodules are generally hypointense on T1-weighted images and do not enhance with contrast. MRI utilizes multiple sequences and techniques to comprehensively evaluate liver tumors, diffuse diseases, and incidental findings.
Renal Tumour Angiomyolipoma - Bizarre Presentation
A 28-year old female presented with fever, chills, and flank pain. Imaging showed a large liver lesion initially thought to be a tumor or abscess. Further CT and MRI revealed the lesion to be retroperitoneal and connected to the upper pole of the right kidney. At surgery, 1.5 liters of pus was drained. Histopathology of the renal mass found it to be an angiomyolipoma, a rare renal tumor. Literature review discussed diagnostic challenges and optimal imaging techniques for diagnosing renal angiomyolipomas.
The pathway to uroradiology
1. MDCT uses a rotating X-ray tube and row of detectors to provide stronger X-rays than conventional CT. It can perform multiphase scans and multiplanar reconstructions.
2. MRI is useful for evaluating scrotal masses when ultrasound findings are discrepant, testicular involvement is diffuse, or fibrous lesions, lipomas or hemorrhage are suspected.
3. FDG-PET is limited in detecting localized prostate, bladder, and renal cancers due to low glycolytic activity of malignant cells and urinary excretion of FDG. However, it can help evaluate changes in tumor burden and site during and after therapy.

Ultrasonography is the initial test of choice to evaluate obstructive jaundice as it is non-invasive, inexpensive and highly sensitive. It can detect dilated bile ducts suggesting extrahepatic obstruction. MRCP and ERCP provide more detailed imaging of the biliary tree but ERCP allows for therapeutic interventions. Other options include CT, PTC and EUS which provide additional information but have greater risks or limitations. The cause of obstructive jaundice can be benign such as gallstones or malignancies involving the bile ducts, pancreas or gallbladder.

Renal scans (C, D) show normal-functioning parenchyma.7

- 5. • Fig GU 9-3 Renal cyst puncture. (A) The instillation of contrast material shows the smooth inner wall characteristic of a benign cyst. (B) In another patient, the introduction of contrast material reveals the markedly irregular inner border of a necrotic renal cell carcinoma.

(A) A combined excretory urogram and inferior vena cavagram shows a large mass in the lower pole of the right kidney with displacement but no invasion of the pelvocalyceal system and inferior vena cava. (B) Arteriography shows the mass to be hypervascular. The overall radiographic appearance is indistinguishable from that of renal cell carcinoma.
