ENTITLEMENT COORDINATOR – REHAB (JHL Manhattan Division)
 Maintain a comprehensive list of newly admitted patient, discharged patient and patients on leave of absent.
 Keep a tracing record of facility that referred patients to the Nursing Home
 Keep record of where patients are been discharged to.
 Making sure we have the correct payer for each patient on record
 To keep the movement of patients from the Nursing Home to Hospital, transfer from room to room in the
facility
 Keep the list of all patient in the facility, their room and the bed numbers.
 Keeping day to day treatment and progress record of all patients in the facility
 Verifying all payments with patients payers and the Account department of the facility
 Keep records of time-in and time-out of all Rehab workers for salary purpose.
 Maintain good and friendly relationship with patients and members of their families.
 Providing assistance and support with respect to the financial aspects of the department to the Director of
Rehabilitation
 Perform any other duties as may be directed by the Director of Rehabilitation

 Responsible for initial outreach to the Potential Enrollee within 24 hours time period.
 To educate the Potential Enrollee on what is needed to complete a New Medicaid Application, if there is an
indication a New Medicaid Application.
 To educate the Potential Enrollee on what is needed to have an identified Medicaid Issue resolved, such as
 Recertification, *New Medicaid Application, *Surplus Issues *Pooled Trust *Nursing Home Restriction,
 Spend Down *RVI-3 Status. Provide assistance to consumers during emergency situations as necessary.
 Work on E-PACES to identifies coverage problems or restrictions. Identifies current Providers or service
information. Provides coverage end date of a Medicaid eligibility
 Review information in the Entitlement Work tab in E-Champ completed by the Membership Development
Coordinator/Members Service Coordinator or Data entry staff to confirm the reason for referral.
 Develop Individualized Service Plans with and for consumers in accordance with OPWDD and agency
regulations using a person-centered planning process.
 Maintain accurate up to date records, reports, and documentation which are required by state and agency
regulations. Comply with all agency standards, rules, practices, and procedures.
 Provides information and answers to all questions from potential members regarding benefits, rights and
responsibilities, and plan operations. Meets and reviews monthly sales-related goals and objectives
 Coordinates and relays information to staff related to the pre-enrollment process

*Identify and educate potential members on all aspects of the plan including answering questions regarding plan's
features and benefits and walking client through the required Blue List disclosures.
*Responsible for identifying and assisting current members who are due to re-certify their healthcare coverage for
Medicaid, FHP and CHP by completing member's annual recertification application including addition on additional
eligible family members.
*Responsible for meeting new application goals and maintaining acceptable rejection levels while adhering to
regulatory and state compliance
*To initiate conversations with the general public on the importance of healthcare and the services that a
manage care organization provides.
*Maintain ample supply of Access New York Applications (ACNY), Addendums, Blue listings and Medicaid Long
Forms at all times. Maintain and carry Facilitated Enroller Reference Binder (FERB) at all times.
*Attend events, schools, sites, and meetings.
*Manage and maintain positive relationships with community organizations, provider offices
* Participation in weekly, bi-weekly, and monthly full staff and team meetings.
* On-line application.

*Ensures the accuracy and quality of enrollment applications and documents submitted by Enrollment Sales

development.

information and departmental updates, strategize marketing activities and develop personnel, conduct
weekly team meetings to address strategies in new enrollment and retention efforts, in addition to
disseminating departmental updates.
*Assist in establishing and maintaining productive and cooperative relationships with clients to ensure
that profitability and membership goals are maintained.
