cartilage และ matrix มีความไมสมดุล
ระหวางการสรางและทําลาย articular

10. NSAIDs
• Analgesic และ anti-inflammatory effects
อาการที่เกิดขึ้นใน OA สวนหนึ่งอาจเกิดจาก mild synovitis, หรือ
periarticular inflammation
ระยะสั้น NSAID ลดอาการปวดไดดีกวา placebo
ระยะยาว ไดผลดีกวา placebo แตมีปญหาการติดตามการใชยา และ
ไมสามารถใชยาไดติดตอกันนาน

Cochrane Database Syst Rev. 2005
• In the placebo-controlled RCT, acetaminophen was shown to be
clearly superior to placebo with a similar safety profile.
• Acetaminophen was less effective overall than NSAIDs in term of
pain reduction and global assessment BUT both drugs had similar
efficacy in term of improvement in functional status.

Overall, ...... IA HA being modestly effective in treatment for pain
associated with moderate knee OA, and the effect lasts for several
months, but saline injections also produce large effect sizes over the
same period of time.........

Recommodation:
1. ใชในรายที่ไมตอบสนองตอ non-pharmacologic therapy
และการใชยาแกปวดทั่วไป
2. รายที่มีขอหามในการใช NSAID หรือมีผลขางเคียงตอการใช NSAID
(ควรใชในรายที่ moderately severe OA)
ผลที่ไดคงอยูประมาณ 2-6 เดือน

25. Intra-articular hyaluronic acid for the treatment of OA of the
knee: systematic review and meta-analysis.

22 trials from 1159 papers until April 2004
1. Pain reduction during movement (VAS 100 mm.)
- 3.8 mm after 2-6 weeks
- 4.3 mm after 10-14 weeks
- 7.1 mm after 22-30 weeks
This effect was NOT compatible with a clinically “meaningful difference”.
(expected to be about 15 mm on the visual analogue scale)
2. No improvement in knee function was observed at any points.

Content up to July 2003, and Cochrane Central Register of Controlled
Trials. Specialised journals and reference lists of RCTs and pertinent
review articles up to April 2004 were handsearched.
63 trials were identified. (37 trials: HA vs. placebo, 9 trials: HA vs. IA
steroids, and 5 trials: HA vs. NSAIDs)

28. Viscosupplementation for the treatment of OA of the knee.
Bellamy N et al. Cochrane Database Syst Rev 2005 Apr
Main results:
1. The pooled analyses of the effects of HA against placebo controls
generally supported the efficacy of this class of intervention.
5-13 week post inj. showed improvement from baseline
(11-54% for pain and 9-15% for function)
2. Comparable efficacy was noted against NSAIDs and longer-term benefits
were noted in comparisons against IA steroids
Overall, the aforementioned analyses support the use of the HA class of
products in the treatment of knee OA.

1. Oral GS และ CS ประสิทธิภาพในการในการลดอาการปวด ชวยให
การทํากิจกรรมและเคลื่อนไหวขอไดดีขึ้น เมื่อเปรียบเทียบกับ placebo
2. GS มีประสิทธิภาพในการชะลอการดําเนินของ OA เมื่อเทียบกับ
placebo แตผลมีนอยมาก (0.27 มม. ใน 3 ป) ไมมีขอมูลสวนนี้ที่
ชัดเจนใน CS
3. ขอมูลประสิทธิภาพของยาทั้งสองเปรียบเทียบกับ placebo ที่มี
นัยสําคัญทางคลินิก คือ ประสิทธิภาพในการลดอาการปวดและการชวย
การเคลื่อนไหวของขอใหดีขึ้นเทานั้น

Towheed TE et al, Cochrane Database Syst Rev. 2005 Apr 18
8 trials with adequate allocation concealment failed to show benefit
of glucosamine for pain and WOMAC function.
The 20 analysed RCTs found glucosamine favoured placebo with an
improvement in pain and function the Lequesne Index. WOMAC
pain, function and stiffness outcome DID not reach statistical
significance.
The results are not uniformly positive….. reasons …unexplained.
Rotta preparation, glucosamine was found to be superior for pain,
and function using the Lequesne Index. …… (but not WOMAC)

Authuor’s Conclusions:
Update 20 trails with 2750 patients; pooled results from studied using non-Rotta
preparation or adequate allocation concealment failed to show benefit of
glucosamine for pain and WOMAC function.
Studies evaluating Rotta preparation show that glucosamine was superior to
placebo in the treatment of pain and functional impairment resulting from
symptomatic OA. the Lequesne Index. …… (but not WOMAC)
WOMAC outcomes of pain, stiffness, and function DID NOT show a superiority
of glucosamine over placebo for both Rotta and non-Rotta preparations.

35. Diacerein
• ลดการสรางหรือหลั่ง IL-1 จากเยื่อบุขอ ลดการทํางานของ IL-1 receptor ที่มี
ตอchondrocyte
• Diacerein vs. tenoxicam in OA hip 288 ราย: ประสิทธิภาพของขอพอๆ กัน
เมื่อประเมินที่เดือนที่ 2 แตกลุมที่ได diacerein มีความตองการยาแกปวด
มากกวากลุมที่ได tenoxicam
• การใชยาระยะปานกลาง diacerein vs. placebo: diacerein ไดผลดีกวา placebo
ในการลดอาการปวด
• การใชยาในระยะยาว ใน hip OA เทียบกับ placebo พบวายาสามารถชะลอการ
สูญเสียของความกวางของชองขออยางมีนัยสําคัญทางสถิติ (50% vs. placebo
60%; p = 0.036) โดยการวิเคราะหขอมูลพบวายาสามารถชะลอการสูญเสีย
ความกวางของชองขอได 0.15 มม. ตอป แตอยางไรก็ตามการศึกษานี้ไมพบ
ความแตกตางของอาการที่ดีขึ้นเมื่อเทียบกับ placebo

It was not possible to draw firm conclusions from a single study.
2 combined studies of avocado/ soybean unsaponifiables showed beneficial
effects on functional index, pain, intake of NSAIDs and global evaluation.
Single studies using willow bark preparation (Reumalex), topical capsaicin
and tipi tea, were inconclusive.
