1. PURPOSE
This document provides the requirements to ensure adequate management support is available
to support the operations of the Pengerang Refining Company Sdn Bhd and Pengerang
Petrochemical Company Sdn Bhd to be known after this as PRefChem. The purpose of this
procedure is to establish the process and requirements to conduct inspection, testing and
preventive maintenance of firefighting and emergency equipment in the process area.
2. SCOPE
The requirements in this document apply to all facilities within the PRefChem including
projects and construction sites.
3. PERFORMANCE MEASUREMENTS
 Compliance to inspection, testing and preventive maintenance schedule.
 Gaps closing based on defect items of the firefighting and emergency equipment.

General Requirements
ITPM for firefighting and emergency equipment for PRefChem shall be conducted
according to the frequency fixed by this procedure and the party conducting the ITPM
activity shall comply with the specific procedures according to the specific equipment.
Inspection, Testing and Preventive Maintenance (ITPM) of Fire Fighting
Equipment
Inspection, testing and maintenance shall be conducted as Table 1: ITPM Table for
Firefighting and Emergency Equipment and Table 2: Table of Specific Roles by Equipment
and Frequencies.
The equipment that shall be covered by this procedure are as follows:

Any activity for inspection, testing and maintenance of firefighting and emergency
equipment shall follow at least minimum of the frequency as per Table 1. If the activity is
outsourced to a third party or any contractors, they shall also obey to the minimum
requirement of this procedure.

Record Keeping
Records of Inspection, Testing and Preventive Maintenance of Firefighting and Emergency
Equipment shall be kept by Fire Safety and Emergency Response Section or any parties
that applies this procedure as a guideline to conduct his/her program such as Daily
Structured Round.
Specific Roles and Responsibility
The execution of ITPM activity and programs shall be conducted by respective person in
charge as per Table 2. He/she shall use the procedures related to this document to
perform his/her ITPM activity and programs to ensure emergency preparedness level is
at a satisfying level.

shall be managed through PRefChem-03_05-L3-046778 - Bypassing of Safety Critical
Element (SCE).
Bypass or override certificate shall be obtained prior to disabling any fire protection
system. No fire protection system shall be disabled without written notification to affected
parties. Maintenance plans and schedules shall be developed and approved prior to work
initiation.
Inventory of spare components and consumable items required by fire protection systems
(e.g. portable fire extinguishers and clean agent cylinders) shall be provided based on
manufacturer’s recommendations, considering procurement and delivery period. An
updated inventory list shall be maintained for ease of inspection.
ITPM activities which may involve handling of hazardous items (e.g. testing of gaseous
fire suppression system or replacement of highly pressurized agent cylinders) shall be
assessed in accordance with Job Hazard Analysis. ITPM activities which may cause
temporary loss of fire protection function (e.g. removal of firewater pump for service)
shall be treated with additional mitigations. A risk assessment shall be conducted,
endorsed by Fire Safety & Emergency Response, and mitigations implemented prior to
the activity. These additional mitigations should be adopted into the abnormal operation
procedure.
Fire Safety & Emergency Response should identify and analyze reports of defects, failures
and issues affecting performance of fire protection systems and develop intervention plans
to be endorsed by the Management. Fire Safety & Emergency Response should verify
completion of ITPM activities carried out by third parties or contractors.
Requirement of Competency and Training
Only competent personnel shall be allowed to carry out ITPM on fire protection systems.
Persons performing servicing of equipment may be required by local fire authorities to be
registered and/or in possession of valid competency certificate. For example, the servicing
of portable fire extinguishers in Malaysia requires JBPM’s FECP & eFEIS certification.

Personnel performing ITPM shall be provided with formal training and the relevant PPE,
tools and equipment to carry out the ITPM activities. Fire Safety & Emergency Response
Section shall verify the competency of any third party performing ITPM activities, including
during early contracting phase.
Deferral of ITPM for Firefighting and Emergency Equipment
In any event of that inspection, testing and preventive maintenance firefighting and
emergency equipment is required to be deferred, deferral form from procedure
PRefChem-06_03_02-L3-049259 – ITPM Deferral Management with attachment of the
risk assessment for the deferred equipment. Deferring of inspection, testing and
preventive maintenance of firefighting and emergency equipment shall be made according
to the steps/flow/method embedded in the procedure. See Attachment A for Deferral
Form.

Misuse of Firefighting and Emergency Equipment
The deployment of these equipment should only be carried out by competent persons
from the Emergency Response Team (ERT), Fire Safety and Emergency Response
personnel or authorized Fire Service Contractors/Service Provider appointed by Fire
Safety and Emergency Response personnel for inspection and testing purposes. Measures
should be in place to ensure that the equipment is not exposed to damage, theft, sabotage
or misuse. Safety precautions shall be practiced by personnel operating/inspecting/testing
this equipment.

 Assist the periodic audit which includes necessary audit at
contractor’s site to ensure compliance to relevant procedures.
 Report any fire safety issues requiring management attention
with a proposed intervention plan
Area Owner  Conduct inspection of firefighting and emergency equipment in
his / her area of control (e.g. OSB, Process Unit, FAR,
Substation).
 Notify and report to Fire Safety and Emergency Response
Section immediately for any defect or any replacement required
to any of the firefighting and emergency equipment within his /
her area of control.
Plant/Shift Supervisor  Ensure firefighting and emergency equipment are properly
maintained and operating in good conditions based on the
requirement.
 Ensures that all inspection of firefighting and emergency
equipment are conducted as per schedule.
 Assist Fire Safety and Emergency Response Section to conduct
testing and preventive maintenance of firefighting and
emergency equipment.
 Assist Fire Safety and Emergency Response section on the
renewal of Fire Certification (FC) issued by Jabatan Bomba &
Penyelamat Malaysia (JBPM).
 Ensure that all personnel (staff and contractors) operating in
his/her area are trained and being briefed on the operation of
firefighting and emergency equipment within his/her area of
control.

PURPOSE This document provides the requirements to ensure adequate management support is available to support the operations of the Pengerang Refining Company Sdn Bhd and Pengerang Petrochemical Company Sdn Bhd to be known after this as PRefChem. The purpose of this procedure is to establish the process and requirements to conduct inspection, testing and preventive maintenance of firefighting and emergency equipment in the process area. 2. SCOPE The requirements in this document apply to all facilities within the PRefChem including projects and construction sites. 3. PERFORMANCE MEASUREMENTS  Compliance to inspection, testing and preventive maintenance schedule.  Gaps closing based on defect items of the firefighting and emergency equipment. 4. PROCEDURE No. Procedure 4.1 4.2 General Requirements ITPM for firefighting and emergency equipment for PRefChem shall be conducted according to the frequency fixed by this procedure and the party conducting the ITPM activity shall comply with the specific procedures according to the specific equipment. Inspection, Testing and Preventive Maintenance (ITPM) of Fire Fighting Equipment Inspection, testing and maintenance shall be conducted as Table 1: ITPM Table for Firefighting and Emergency Equipment and Table 2: Table of Specific Roles by Equipment and Frequencies. The equipment that shall be covered by this procedure are as follows:  Portable Type Fire Extinguisher  Fire Hose Box and Accessories

If the activity is outsourced to a third party or any contractors, they shall also obey to the minimum requirement of this procedure. Bypass or override certificate shall be obtained prior to disabling any fire protection system. No fire protection system shall be disabled without written notification to affected parties. Maintenance plans and schedules shall be developed and approved prior to work initiation. Inventory of spare components and consumable items required by fire protection systems (e.g. portable fire extinguishers and clean agent cylinders) shall be provided based on manufacturer’s recommendations, considering procurement and delivery period. An updated inventory list shall be maintained for ease of inspection. ITPM activities which may involve handling of hazardous items (e.g. testing of gaseous fire suppression system or replacement of highly pressurized agent cylinders) shall be assessed in accordance with Job Hazard Analysis. ITPM activities which may cause temporary loss of fire protection function (e.g. removal of firewater pump for service) shall be treated with additional mitigations. A risk assessment shall be conducted, endorsed by Fire Safety & Emergency Response, and mitigations implemented prior to the activity. These additional mitigations should be adopted into the abnormal operation procedure. Fire Safety & Emergency Response should identify and analyze reports of defects, failures and issues affecting performance of fire protection systems and develop intervention plans to be endorsed by the Management. Fire Safety & Emergency Response should verify completion of ITPM activities carried out by third parties or contractors. Requirement of Competency and Training Only competent personnel shall be allowed to carry out ITPM on fire protection systems. Persons performing servicing of equipment may be required by local fire authorities to be registered and/or in possession of valid competency certificate. For example, the servicing of portable fire extinguishers in Malaysia requires JBPM’s FECP & eFEIS certification.

Fire Safety & Emergency Response Section shall verify the competency of any third party performing ITPM activities, including during early contracting phase. Deferral of ITPM for Firefighting and Emergency Equipment In any event of that inspection, testing and preventive maintenance firefighting and emergency equipment is required to be deferred, deferral form from procedure PRefChem-06_03_02-L3-049259 – ITPM Deferral Management with attachment of the risk assessment for the deferred equipment. Deferring of inspection, testing and preventive maintenance of firefighting and emergency equipment shall be made according to the steps/flow/method embedded in the procedure.
