Non-compliance of KOC HSEMS
procedures
 Obtain required Permit to Work in compliance with KOC SA.004 ( Permit to Work
Procedure)
 Ensure to comply with KOC HSEMS Welding and Cutting (KOC.SA.021), Use of Abrasive
Wheels and Disc Equipment (KOC.SA.012),Portable
Battery and Electrically Operated Tools & Equipment (KOC.SA.014).
Poor task planning
 Ensure all personnel have undergone Project HSE Induction and EOD training and
attended craft wise training.
 Tool box talk
 Ensure to deploy competent person to do the task.
 Ensure the equipment is certified by company as per KOC-SA-013- Mobile Equipment
Procedure and fit for the intended purpose as required
 Ensure all equipment to be used in association with gas cylinders (if any) is properly
inspected and in operationalcondition and fit for purpose.
 Ensure that PPE appropriate to hot work is correctly identified & used.
Welding,

Mis-use of equipment
 Keep the power supply cable secured and Cable management implemented
 Ensure all electricalpower sourcesare properly earthed and dead man switch of the
grinding machine is functional.
 Ensure grinding disc/ cuttingdisc fitted to the grindingmachine is of correctRPM and
size/type
 Ensure hoses connectionsfor gas cutting are fitted with flash back arresters and
color coded.
 Ensure all gas cylinders are placed in trolleys and secured on upright position.
Not wearing appropriate PPE

 Wear appropriate PPE for Welding / Grinding
i.e. face shield, welders hood, Welding / Grinding aprons, leg covering spatsand
gloves
And 100% cotton coveralls. Shoulder covers are needed for overhead worksif any

 Provide welding / grinding habitats to hot work location to prevent sparks in open
area.
 Remove all flammable and combustible materials in the area and safety barricade the
area as appropriate.

 No one should be allowed to go to remote area alone.
 Ensure availability of cold water at job site
 Drink water frequently to avoid dehydration
 Take breaks in hot weather
