A person's cognitive system can overall be very effective, though occasionally mistakes occur resulting in generalized cognitive failures. Cognitive
failures, also known as absent–mindedness, are errors or mistakes people make due to attention slips or memory failures (Reason, 1982). These failures
refer to all possible errors within the cognitive system and can be traced to problems with attention, memory, errors created by distractions and errors in
the execution of intentions. These include lapses of attention, mind–wandering, action failures, etc. Examples of cognitive failures include daydreaming
during an exam, forgetting the name of the person you just met a short time before and forgetting where you placed your keys or your glasses. These
are mistakes that most people would agree they had experienced. How often these mistakes occur however is dependent on the person. When I kept
track of my own personal memory lapses, I found that I often make mistakes on a daily basis.
